Aβ1-42, en veletableret biomarkør for AD, er en CSF-kernebiomarkør relateret til Aβ-metabolisme og amyloidplaquedannelse i hjernen og har været meget udbredt i kliniske forsøg og klinikken26. Amyloid-β (Aβ) og tau er vigtige CSF-biomarkører. Aβ består af 42 aminosyrer (Aβ1-42), som er produktet af transmembran amyloid precursor protein (APP) hydrolyseret af β- og γ-sekretaser1. Aβ1-42 kan aggregeres til ekstracellulære amyloidplaques i hjernen på grund af dets opløselighedsegenskaber1,2. Tau er et mikrotubuli-associeret protein, der hovedsageligt er til stede i axoner og er involveret i anterograd aksonal transport3. Unormal tau hyperphosphorylering induceres hovedsageligt af ubalancen mellem kinaser og fosfataser, hvilket resulterer i løsrivelse af tau fra mikrotubuli og dannelsen af neurofibrillære tangles (NFT)1. Koncentrationen af tau stiger i CSF, fordi tau- og phosphorylerede tau-proteiner (pTau) frigives i det ekstracellulære rum under den neurodegenerative proces. Tidligere undersøgelser har vist, at CSF-biomarkører er relevante for de tre vigtigste patologiske ændringer i Alzheimers sygdom (AD) hjerne: ekstracellulære amyloidplaques, intracellulær NFT-dannelse og neurontab4. Unormale koncentrationer af Aβ og tau til stede i det tidlige stadium af AD, hvilket muliggør tidlig AD-diagnose5,6.
I 2016 fandt Tsai et al., at ikke-invasiv lysflimmer (40 Hz) reducerede niveauerne af Aβ1-40 og Aβ1-42 i den visuelle cortex af præ-deponerende mus7. For nylig rapporterede de yderligere, at auditiv tonestimulering (40 Hz) forbedrede genkendelse og rumlig hukommelse, reducerede amyloidproteinniveauer i hippocampus og auditiv cortex (AC) hos 5XFAD-mus og reducerede pTau-koncentrationer i P301S tauopatimodellen8. Disse resultater indikerer, at ikke-invasive teknikker kan påvirke Aβ- og tau-metabolismen.
Som et ikke-invasivt værktøj kan transkraniel magnetisk stimulering (TMS) elektrisk stimulere neuralt væv, herunder rygmarven, perifere nerver og hjernebark9. Desuden kan det ændre excitabiliteten af hjernebarken på det stimulerede sted og i de funktionelle forbindelser. Derfor er TMS blevet anvendt til behandling af neurodegenerative lidelser og prognostiske og diagnostiske tests. Den mest almindelige form for klinisk intervention i TMS, rTMS, kan inducere cortexaktivering, ændre cortexens excitabilitet og regulere kognitiv / motorisk funktion.
Det blev rapporteret, at 20 Hz rTMS havde en in vitro neuroprotektiv virkning mod oxidative stressorer, herunder glutamat og Aβ og forbedrede den samlede levedygtighed af monoklonale hippocampale HT22-celler i mus10. Efter 1 Hz rTMS-stimulering blev det β-site APP-spaltningsenzym 1, APP, og dets C-terminale fragmenter i hippocampus reduceret betydeligt. Især svækkelsen af langsigtet potensering, rumlig læring og hukommelse i hippocampus CA1 blev vendt11,12. Bai et al. undersøgte effekten af rTMS på den Aβ-inducerede gammaoscillationsdysfunktion under en arbejdshukommelsestest. Tidligere undersøgelser har vist, at et ikke-invasivt lysflimrende regime og auditiv tonestimulering kan påvirke Aβ- og tau-stofskiftet i hjernen. Som en ikke-invasiv teknik er gentagen transkraniel magnetisk stimulering (rTMS) blevet anvendt til behandling af neurodegenerative lidelser. Denne undersøgelse undersøgte virkningerne af rTMS på Aβ- og tau-niveauer i rhesus abe cerebrospinalvæske (CSF). Dette er en enkeltblind, selvkontrolleret undersøgelse. Tre forskellige frekvenser (lavfrekvent, 1 Hz; høje frekvenser, 20 Hz og 40 Hz) af rTMS blev brugt til at stimulere rhesusabens bilaterale-dorsolaterale præfrontale cortex (DLPFC). En kateteriseringsmetode blev brugt til at indsamle CSF. Alle prøver blev udsat for væskechipdetektion for at analysere CSF-biomarkører (Aβ42, Aβ42 / Aβ40, tTau, pTau). CSF-biomarkørniveauerne ændrede sig med tiden efter stimulering med rTMS. Efter stimulering viste Aβ42-niveauet i CSF en opadgående tendens ved alle frekvenser (1 Hz, 20 Hz og 40 Hz) med større forskelle for højfrekvenserne (p < 0,05) end for den lave frekvens. 
Efter højfrekvent rTMS steg det samlede Tau-niveau (tTau) af CSF straks ved post-rTMS-tidspunktet (p < 0,05) og faldt gradvist med 24 timer. Desuden viste resultaterne, at niveauet af fosforyleret Tau (pTau) steg umiddelbart efter 40 Hz rTMS (p < 0,05). Forholdet mellem Aβ42/Aβ40 viste en opadgående tendens ved 1 Hz og 20 Hz (p < 0,05). Der var ingen signifikant forskel i tau-niveauerne med lavfrekvent (1 Hz) stimulering. Således kan højfrekvenser (20 Hz og 40 Hz) af rTMS have positive virkninger på Aβ- og tau-niveauer i rhesus monkey CSF, mens lavfrekvent (1 Hz) rTMS kun kan påvirke Aβ-niveauer.

Resultaterne viste, at rTMS kunne påvirke Aβ- og tau-niveauerne i rhesusaben CSF. CSF-biomarkørniveauerne ændrede sig med tiden efter rTMS-stimulering ved forskellige frekvenser (1 Hz, 20 Hz og 40 Hz).

Denne protokol bruger seriel cisterna magna CSF-prøveudtagningsmetode til at undersøge virkningerne af rTMS på alfa-beta- og tau-niveauer i rhesusabe. Denne teknik anvender en ny CSF-prøveudtagningsmetode, der muliggør gentagen CSF-prøveudtagning under fuldt vågne forhold med lav risiko for utilsigtede hændelser. Demonstration af proceduren vil være Ying-Qian Zhang, en forskningsassistent fra Institut for Fysiologi ved Southwest Medical University, og Hui-Xin Tan, en kandidatstuderende fra Sichuan University.Efter administration af analgesi og bekræftelse af vellykket bedøvning skal du placere den femårige mandlige rhesusabe på operationsbordet i lateral decubitusposition og desinficere området omkring lænden. På den tredje dag efter den subarachnoide kateterisering og to uger før eksperimentets start skal aben udsættes for adaptiv træning med abestolen to gange om dagen i 30 minutter hver gang.Udfør rTMS adaptiv træning eller sham-stimulering en uge efter den adaptive træning med abestolen og en uge før starten af det formelle eksperiment for at undgå at hindre eksperimentets fremskridt ved hjælp af vibrationer og lyde under stimuleringsprocessen. Til sham-stimulering skal du bruge en sham-spole, der producerer vibrationer og lyd, men ikke genererer et magnetfelt. Efter stimulering skal du tilbyde mad til aben for at hjælpe den med at tilpasse sig processen.Lokaliser den bilaterale dorsolaterale præfrontale cortex i henhold til det internationale 10 til 20-system, og udfør tre forskellige sessioner af rTMS ved hjælp af tre forskellige frekvenser uden en udvaskningsperiode på mere end 24 timer. Udfør den første, anden og tredje session to gange om dagen i tre på hinanden følgende dage. For at analysere de fire CSF-biomarkører skal du bruge en minimalt invasiv kateteriseringsmetode til at indsamle CSF.Saml CSF på fem tidspunkter med fire prøver på hvert tidspunkt med tre minutters intervaller. Efter at have indsamlet i alt 60 prøver til tre frekvenser, nummereres og opbevares dem i et minus 80 grader Celsius køleskab i op til en måned. Efter eksperimentet skal alle prøver underkastes væskechipdetektion i henhold til producentens anvisninger.Efter en hertz rTMS-stimulering steg Abeta42-niveauerne gradvist over 24 timer. Tilsvarende med rTMS ved 20 hertz steg Abeta42-niveauerne med tiden og nåede et højdepunkt seks timer efter stimulering. I modsætning hertil steg Abeta42-niveauerne efter 40 hertz-stimulering signifikant straks ved post-rTMS-tidspunktet og faldt derefter langsomt.Forholdet mellem Abeta42 og Abeta40 viste en opadgående tendens efter stimulering med en og 20 hertz rTMS og steg signifikant to timer efter stimulering. Der var dog ingen signifikant forskel i forholdet ved 40 hertz. tTau-niveauerne i CSF steg straks efter 20 og 40 hertz rTMS-stimulering og faldt gradvist.Der var dog ingen signifikant forskel efter en hertz-stimulering. PTau-niveauet steg straks efter 40 hertz-stimulering og faldt til under baseline-niveauet efter 24 timer. I modsætning hertil viste pTau-niveauet for stimuleringen af en og 20 hertz en nedadgående tendens.Forskere, der kræver flere prøver af abe CSF, kan overveje denne serielle cisterna magna CSF-prøveudtagningsmetode.
