当暴露于黑暗、闪烁的光线6或含咖啡因的饮料7 时，健康人的血管密度会发生变化，因为神经血管耦合会根据具有最高活性的视网膜层在浅表、中层和深部毛细血管丛之间重新分配血流。由这种再分布引起的血管密度降低在刺激停止后返回到基线值，并且不代表毛细血管丢失，这是在视网膜病变出现在血管疾病（如糖尿病8 或动脉高血压）之前报告的病理变化9。
毛细血管减少可以通过小动脉血管扩张部分补偿。仅测量百分比或灌注面积并不能深入了解是否存在血管扩张，当毛细血管达到最小阈值时，血管扩张就会出现。测量血管密度无助于检测血管扩张引起的循环面积增加。可以使用血管密度和灌注密度之间的决定系数来间接估计小动脉循环对灌注密度的贡献，并定义与毛细血管或其他血管相对应的循环面积的百分比。
此技术背后的基本原理是，回归分析可以确定独立数值的变化在多大程度上导致从属数值的变化。在使用 OCTA 的黄斑血管成像中，毛细血管循环是一个自变量，它影响循环区域，因为评估区域中很少有较大的血管。然而，副中央凹具有较大的血管，可以扩张并改变循环面积的百分比，这无法通过当前的自动OCTA指标直接识别。使用决定系数的优点是，它测量两个现有指标之间的关系，以产生另外两个：对应于毛细血管的循环面积的百分比，以及对应于其他血管的百分比。这两个百分比都可以使用带有成像软件的像素计数直接测量。但是，可以使用 OCTA 设备自动生成的数字计算样品的确定系数10，11。
Pathak等人使用决定系数从使用人工神经网络的人口和人体测量测量中估计瘦肌肉和脂肪质量。他们的研究发现，他们的模型的 R2 值为 0.92，这解释了其大部分因变量的变异性12。O'Fee及其同事使用决定系数排除了非致命性心肌梗死作为全因和心血管死亡率的替代物，因为他们发现R2 为0.01至0.21。这些结果表明，自变量解释了因变量变化的不到80%，作为代孕标准（R2 = 0.8）13。
确定系数用于评估变量、一组变量或模型的变化对结果变量变化的影响。1 和 R2 值之间的差值表示其他变量对结果变量变化的贡献。将差异归因于单个变量并不常见，因为通常有两个以上的变量对结果有贡献。然而，具有循环的黄斑区域的比例只能来自毛细血管覆盖的区域和较大血管覆盖的区域，因为较大的血管比毛细血管扩张更多。此外，反应性血管舒张被认为很可能起源于视网膜小动脉，因为毛细血管循环减少会减少氧气供应。
只有两个来源贡献了黄斑中循环面积的百分比：毛细血管和大于它们的血管。血管密度和灌注密度之间的决定系数决定了毛细血管对循环区域的贡献，剩余的变化（1和R2 值之间的差异）代表了唯一代表循环区域（在较大的视网膜血管内）的其他变量的贡献。本文描述了在健康人群中测量这种贡献的方法（第1组）以及它在视网膜血管疾病患者中的变化：无高血压性视网膜病变的动脉高血压（第2组）和无糖尿病性视网膜病变的糖尿病（第3组）。
