我々は、両側性動脈硬化症のために右トランスカナルEStSを受けた56歳の女性の症例を報告する。患者はめまいや耳鳴りなしでゆっくりと進行し、両側難聴(HL)を報告した。右眼球鏡検査は正常であったが、聴力測定テストは、24dBの平均術前骨伝導純粋なトーン平均(BC-PTA)、71dBの平均術前空気伝導純粋なトーン平均(AC-PTA)、および47dBの平均術前空気骨ギャップ(ABG)を有する、右中程度の混合HLを示した。タイパノグラムは両側A型であり、ステープル反射は存在しなかった。術前評価として必要と考えられるCTスキャンはなかった。

トランスカナル排他内視鏡アプローチによるステップバイステップの散道切除術。手術室の設置や患者の位置決めから術後ケアまで、内視鏡的なテープ手術の事例を示す予定です。私は、外科医が完全な内視鏡的アプローチでステープ手術を行うことを可能にする技術的なヒントを含む外科的処置のステップバイステップの説明を案内するつもりです。内視鏡的なテープ手術は安全で、実現可能であり、伝統的なテープ切り出しと同様に好ましい結果を有することが示されている論文が増えている。しかし、内視鏡の使用に関する経験があまりない外科医にとっては、内視鏡的なテープ手術は困難である可能性がある。適切な器具の終わりが理解されれば、内視鏡と手術器具の間で外科分野を共有するという問題は容易に克服できる。1つの手渡された数十億のコントロールは、初心者の外科医にとって時々イライラする可能性があります。さらに、患者を適切に配置し、手術室を設置して、手順全体にわたって外科医に快適な位置を保証することが重要です。めまいや耳鳴りなしでゆっくりと進行した両側難聴を報告する56歳の女性の症例を報告する。右内視鏡検査は正常であったが、オーディオメトリック検査は右中程度の混合難聴を示した。また、このリンパ球はA型で、動脈反射が欠け、硬化症への両側が示唆された。CTスキャンは、準備評価として必要と考えはありませんでした。オロ気管挿管および全身麻酔の位置の後、患者は、頭部がわずかに延長された顎を有する患部の耳の反対側に向かって傾いた後に、患者の上顎を傾けた。可能な限り同じ側の肩を引き下げて、頭と肩の間に広角を作り、耳に簡単にアクセスできるようにします。局所麻酔薬の約1〜2ミリリットルを外部聴覚運河皮膚の4つの主要なポイントで希釈した予備麻酔を行う:後壁、優れた、劣った角度および後経領域。ヨドポビドホン浸漬ガーゼを使用して、外部耳全体を殺菌して無菌場を作ります。イオドポビドテンが外耳道に入り、鼓膜に到達することを保証する。高解像度モニターは、手術中に頭と首の快適な位置を保つために十分な距離で彼の目のレベルで外科医の前に置かれます。高精細デジタルカメラとキセノン光源に直径3ミリメートル、長さ15センチメートル、ゼロ度内視鏡を接続します。内耳の熱損傷を防ぐために、光源を50%の強度に保ちます。ホワイトバランスを行い、焦点を調整した後、内視鏡の先端をきれいにするために品質の脱ミストソリューションを準備します。直接内視鏡ビジョンの下で、運河の後壁の上の細胞面で術中麻酔を行う。外部聴覚運河の最も外側の部分の毛を切断した後、単極を使用して、環状から約8〜10ミリメートル離れた5時から12時までのティンパノ肉フラップ切開を説明する。丸いナイフを使用して、以前にマークされたラインに従って実際の皮膚切開を行い、エピネフリン浸したコットンイドの助けを借りてティンパノ肉のフラップを上げます。一度環状膜の白い肥厚として識別される環状膜は、その下に解剖を続ける。ハートマンの鉗子でマレウスの短いプロセスからパースフラクシダを切り離し、パーステンサをウンバスに付着させます。このステップの間に、コルダ・ティンパニをコーダル・エミネンスから出てくる白っぽい弦として識別し、そのままにしておきます。楕円形の窓領域の良好な露出を達成するために、切除および外部聴覚運河の後骨部分の可変除去はキュレットを介して行われるべきである。場合によっては、楕円形の窓領域が完全に露出している場合でも、いくつかのキュレットは、テープ上の良好な作業領域を作成する必要があります。さらにフットプレートを露出させるために、神経は通常、フックで主に変位する。テープの固定を確認した後、マイクロドリルに0.6ミリメートルのダイヤモンドバーを使用して後十分な結房を行います。その後、同じバリを使用して、フットプレートの中央後部に穴を開けます。ベルッチはさみで散テープル腱に切ります。フックの助けを借りて、テープからインカスを除去し、テープの上部構造を削除します。フィッシュフックを使用して、フットプレートホールを規則化し、適切な口径をチェックし、前庭から可能な小さな骨片を取り除きます。今度は、吸引管を使用して、外部の聴覚運河にテープの補間を挿入する時間です。ここでは、4.75ミリのプロペシスあたり0.6を使用しています。フックの助けを借りて、それをテープトミーの穴にそっと置きます。インカスの長いプロセスに固定した後、プロテシスフックを圧着します。モールのハンドルを軽く押すと、プロテシスの適切な動きを確認します。ティンパノ肉のフラップを交換し、外部監査管を再ソーブルな止血性の誓約で詰めます。接着剤石膏で耳飾りをカバーし、圧縮は必要ありません。吐き気や嘔吐が起こらなかった場合、患者は少なくとも8時間の手術後に飲み、食べ、立ち上がることができるようにします。翌日、ハウスブラックマンスケールを使用して顔面神経機能を評価し、フレンツェルゴーグルを伴う自発的な眼振の存在を評価する。感音難聴を排除するために、骨伝導純粋な音色の聴力測定を行います。患者は顔面麻痺またはめまいのない正常な術後コースを持っていた。6ヶ月間の術後聴力検査で、空気と骨の隙間が完全に閉じ込められた。腹膜検査は鼓膜の定期的な治癒を示し、患者はいかなる味覚障害も否定した。このプロトコルは、内視鏡的な散テープ手術全体で任意の組織外科医を導くことができる。内視鏡的な切り出し術を行う前に、内視鏡的な心筋切り術や心筋形成術などの基本的な手順を用いてトレーニングを行うことをお勧めします。ご注意いただきありがとうございます。
