Metingen van Cilia Beat Frequency (CBF) en patroon zijn vastgesteld als diagnostische hulpmiddelen voor respiratoire aandoeningen zoals primaire ciliaire dyskinesie (PCD)1. Bij Cystic Fibrosis (CF) veroorzaakt disfunctie van het CF Transmembraangeleidingsregulator (CFTR) chloridekanaal uitdroging van de luchtwegoppervlakvloeistof en verminderde mucociliaire klaring2. De ciliaire functie is in vitro onderzocht in primaire luchtwegcelmodellen als indicator voor CFTR-kanaalactiviteit3. Er bestaat echter een aanzienlijke variabiliteit van patiënt tot patiënt in CBF als reactie op CFTR-modulerende geneesmiddelen, zelfs voor patiënten met dezelfde CFTR-mutaties 3. Bovendien is de impact van disfunctionele CFTR-gereguleerde chloridesecretie op de ciliaire functie slecht begrepen. Er is momenteel geen uitgebreid protocol dat de monstervoorbereiding van in vitro luchtwegmodellen, beeldacquisitie en analyse van CBF aantoont.
Nasale epitheliale platen geïsoleerd van nasale mucosale borstelingen worden direct gebruikt voor metingen van de ciliaire functie voor PCD-diagnose4. Hoewel er geen controle is over de grootte of kwaliteit van de verkregen nasale epitheliale vellen, varieert CBF afhankelijk van of het wordt gemeten op enkele cellen of celbladen en op epitheelplaat trilhaarranden die verstoord of ongestoord zijn5. Als zodanig kan secundaire dyskinesieën veroorzaakt door schade aan cellen tijdens het verzamelen van neusslijmvliesborstels CBF beïnvloeden. Primaire celkweek van nasale epitheelcellen en hun differentiatie bij Air-Liquid Interface (ALI) of in driedimensionale keldermembraanmatrix in trilhaarepepitheelorganoïden geven aanleiding tot trilhaartjes die vrij zijn van secundaire dyskinesieën 4,6,7,8. Luchtwegepitheelcellen gedifferentieerd bij ALI (voortaan ALI-modellen genoemd) worden beschouwd als een belangrijk secundair diagnostisch hulpmiddel dat de ciliaire slagpatronen en frequentie van ex vivo neusslijmvliesborstelsrepliceert 6 en analyse van ciliaire ultrastructuur, beatpatroon en slagfrequentie mogelijk maakt met behoud van patiëntspecifieke defecten9 . Toch bestaan er discrepanties in de methodologieën die worden gebruikt om deze pseudostratified, mucociliair gedifferentieerde celmodellen te maken. Verschillende kweekuitbreidings- of differentiatieprotocollen kunnen verschillende epitheliale fenotypen (trilhaar of secretoir)10 induceren en resulteren in significante verschillen in CBF11. CBF is gekwantificeerd in nasale epitheliale borstels 4,6,12,13,14,15,16, luchtwegepitheel organoïden 14,17,18 en ALI modellen 3,4,6,13,19,20, 21. Toch zijn er onder deze protocollen grote variabiliteiten en vaak worden veel parameters niet gecontroleerd. In sommige studies wordt CBF bijvoorbeeld in situ afgebeeld terwijl de cellen van het ALI-model op de permeabele steuninzet 3,19,20,21 blijven, terwijl anderen de cellen van de permeabele steuninzet schrapen en ze in media 4,6,13 ophangen.
Bovendien wordt de bredere toepassing van technieken die de ciliaire functie meten beperkt door de extreme gevoeligheid van de ciliaire functie voor veranderingen in omgevingsfactoren. Omgevingsfactoren zoals temperatuur22,vochtigheid 23,24 en pH25,26 beïnvloeden de ciliaire functie en moeten worden gereguleerd om CBF nauwkeurig te kwantificeren. De verschillende fysiologische parameters die in verschillende laboratoria worden gebruikt en hoe ze CBF beïnvloeden, zijn eerder beoordeeld27.
Verschillende beeldvormingstechnologieën en benaderingen van CBF-metingen worden gerapporteerd in de literatuur. Voor PCD-diagnostiek wordt videomicroscopie gebruikt om ciliaire functie28,29 te meten. Onlangs werd een video-analysealgoritme op basis van differentiële dynamische microscopie gebruikt om zowel CBF- als cilia-coördinatie in luchtwegepitheelcel ALI-modellen 3,30 te kwantificeren. Deze methode maakt de karakterisering van ciliaire kloppen in luchtwegepitheelcellen op een snelle en volledig geautomatiseerde manier mogelijk, zonder de noodzaak om regio's te segmenteren of te selecteren. Verschillende methoden voor beeldvorming en kwantificering van CBF kunnen bijdragen aan de verschillen die in cbf in de literatuur worden gerapporteerd (aanvullend dossier 1).
Een protocol van cultuur tot kwantificering om bestaande methoden, standaardisatie van cultuuromstandigheden en beeldacquisitie te stroomlijnen, uitgevoerd in strikte milieugecontroleerde omstandigheden, zou een consistente, reproduceerbare kwantificering van CBF binnen en tussen individuen mogelijk maken.
Dit protocol biedt een volledige beschrijving van de verzameling epitheelcellen, expansie- en differentiatiecultuuromstandigheden en kwantificering van CBF in drie verschillende luchtwegepitheelcelmodelsystemen van nasale oorsprong: 1) inheemse epitheelbladen, 2) ALI-modellen afgebeeld op permeabele steuninzetstukken en 3) Extracellulaire Matrix (ECM)-ingebedde driedimensionale organoïden (figuur 1) ). Nasale epitheelcellen verkregen uit nasale inferieure turbinaatborstels worden gebruikt als vertegenwoordigers van het luchtwegepitheel, omdat ze een effectief surrogaat zijn voor bronchiale epitheelcellen31 terwijl ze de invasieve procedure overwinnen die gepaard gaat met het verzamelen van bronchiale borstels. De Conditional Reprogramming Cell (CRC) methode wordt gebruikt om primaire luchtwegepitheelcellen uit te breiden voor het maken van ALI-modellen en driedimensionale organoïden. Voorwaardelijke herprogrammering van luchtwegepitheelcellen tot een stamcelachtige toestand wordt geïnduceerd door cocultuur met groei-gestopt fibroblast feedercelsysteem en Rho-associated kinase (ROCK) -remmer32. Belangrijk is dat de CRC-methode de bevolking verdubbelt in luchtwegepitheelcellen met behoud van hun weefselspecifieke differentiatiepotentieel33,34. In alle luchtwegepitheelcelmodellen wordt de ciliaire functie vastgelegd in een temperatuurgecontroleerde kamer met behulp van een hogesnelheidsvideocamera met gestandaardiseerde beeldacquisitie-instellingen. Op maat gemaakte scripts worden gebruikt voor de kwantificering van CBF.

Metingen van de trilhaarfunctie (slagfrequentie, patroon) zijn vastgesteld als diagnostische hulpmiddelen voor aandoeningen van de luchtwegen, zoals primaire ciliaire dyskinesie. De bredere toepassing van deze technieken wordt echter beperkt door de extreme gevoeligheid van de ciliaire functie voor veranderingen in omgevingsfactoren, zoals temperatuur, vochtigheid en pH. In de luchtwegen van patiënten met Cystic Fibrosis (CF) belemmert slijmaccumulatie het kloppen van trilharen. De Cilia-functie is onderzocht in primaire luchtwegcelmodellen als een indicator van CF Transmembrane conductance Regulator (CFTR) kanaalactiviteit. Er is echter een aanzienlijke variabiliteit van patiënt tot patiënt in de frequentie van trilharen gevonden als reactie op CFTR-modulerende geneesmiddelen, zelfs voor patiënten met dezelfde CFTR-mutaties . Bovendien is de impact van disfunctionele CFTR-gereguleerde chloridesecretie op de ciliaire functie slecht begrepen. Gestandaardiseerde kweekomstandigheden en beeldacquisitie uitgevoerd in een milieugecontroleerde toestand zouden een consistente, reproduceerbare kwantificering van CBF tussen individuen en in reactie op CFTR-modulerende geneesmiddelen mogelijk maken. Dit protocol beschrijft de kwantificering van CBF in drie verschillende luchtwegepitheelcelmodelsystemen: 1) native epithelial sheets, 2) lucht-vloeistof interfacemodellen afgebeeld op permeabele steuninzetstukken, en 3) extracellulaire matrix-ingebedde driedimensionale organoïden. De laatste twee repliceren in vivo longfysiologie, met kloppende trilharen en productie van slijm. De ciliaire functie wordt vastgelegd met behulp van een hogesnelheidsvideocamera in een omgevingsgestuurde kamer. Voor de analyse van CBF worden op maat gemaakte scripts gebruikt. Vertaling van CBF-metingen naar de kliniek is bedoeld als een belangrijk klinisch hulpmiddel voor het voorspellen van de respons op CFTR-modulerende geneesmiddelen per patiënt.

Om de efficiëntie van dit protocol bij het kwantificeren van CBF aan te tonen, worden de resultaten van CBF gemeten in luchtwegepitheelcel ALI-modellen afgeleid van drie deelnemers met CF en drie gezonde controledeelnemers gepresenteerd. Op dag 14 van de cultuurdifferentiatie waren kloppende trilharen aanwezig (figuur 6). Collection, Expansion, and Differentiation of Primary Human Nasal Epithelial Cell Models for Quantification of Cilia Beat Frequency

We beschrijven de verzameling luchtwegstamcellen uit neusslijmvlies. Deze cellen worden in het lab uitgebreid en gedifferentieerd tot een pseudostratified epitheel. Dit is in vitro expansie en differentiatie, wat vergelijkbaar is met onze luchtwegen in vivo, wat betekent dat er gespecialiseerde cellen, zoals trilhaarcellen, aanwezig zijn die kunnen worden gebruikt voor het kwantificeren van de trilhaarslagfrequentie.Om een consistente en reproduceerbare kwantificering van de trilhaarslagfrequentie tussen verschillende individuen en in reactie op verschillende geneesmiddelen mogelijk te maken, standaardiseren we kweekomstandigheden en beeldacquisitie. Cilia beat frequentiemetingen worden gebruikt als klinische hulpmiddelen bij ziekten zoals primaire ciliaire dyskinesie. We hopen dit vast te stellen als een klinische marker voor cystische fibrose.Katelin Allan en Laura Fawcett, promovendi uit mijn lab, en Dr.Sharon Wong, een postdoc uit mijn laboratorium, demonstreren de procedure. Begin met de deelnemer te vragen om door zijn mond te ademen. Neem vervolgens een cytologieborstel in de dominante hand en laat het vijfde cijfer op de kin van de deelnemer rusten om de hand te verankeren, steek de cytologieborstel in de neusholte van de deelnemer in een hoek van 45 graden ten opzichte van het gezicht van de deelnemer en passeer deze door de neusvleesus.Nadat u de borstel rechtop hebt gedraaid, zodat deze loodrecht op het gezicht van de deelnemer staat, beweegt u de borstel voorzichtig maar stevig tegen de zijwand van de neus onder het inferieure terbinaat totdat deze zich in het midden tot achterste deel van het inferieure turbinaat bevindt. Draai het penseel maximaal drie keer 360 graden. Verwijder het vervolgens voorzichtig en keer de inbrengmanoeuvre om, zodat de cellen niet loskomen van de borstel.Plaats de borstel in de voorbereide opvangbuis met nasale celverzamelmedia en plaats de opvangbuis op ijs. Na voorzichtige vortexing om cellen van de borstel los te maken, brengt u de opvangbuis op ijs terug naar de bioveiligheidskast. Breng met behulp van een serologische pipet de media over van de verzamelbuis naar een nieuwe buis en laat de cytologieborstels achter.Centrifugeer vervolgens het monster. Na centrifugatie resuspenseert u de celpellet in één milliliter voorwaardelijk herprogrammerend celmedium. Vervolgens passeer je met een serologische pipet van vijf milliliter de cellen door een celzeef die in een cirkelvormige beweging bovenop een buis van 50 milliliter is geplaatst.Om een suspensie met één cel te verkrijgen, verzamelt u de suspensie van de onderkant van de buis en passeert u deze meerdere keren door de zeef. Gooi vervolgens de celzeef weg. Om de luchtwegepitheelcellen op de voorbereide permeabele steuninzetstukken te zaaien, mengt u de cellen goed zonder bubbels te creëren, zodat ze homogeen en in suspensie zijn.Voeg vervolgens 150 microliter van de celsuspensie toe aan de apicale kant van de voorbereide permeabele steuninzetstukken. Om de luchtwegepitheelcellen op de lucht-vloeistofinterface te differentiëren, kweekt u ze in differentiatie of ALI-media gedurende twee dagen. Adem vervolgens de media op en voeg 750 microliter ALI-media alleen toe aan het basale compartiment om een lucht-vloeistofinterface te creëren.Om de luchtwegepitheelcellen in ECM-koepels te zaaien, resuspensie gedissocieerde luchtwegepitheelcellen met het juiste volume van 90% ECM. Houd vervolgens de pipet verticaal in een hoek van 90 graden zo dicht mogelijk bij de bodem van de put en dispenseer 50 microliter van de ECM-celsuspensie naar het midden van de put. Incubeer de plaat bij 37 graden Celsius gedurende 20 minuten totdat de ECU stolt.Terwijl de ECU stolt, verwarmt u de Airway Organoid Seeding Media, of AOSM, tot kamertemperatuur om te voorkomen dat deze bij toevoeging opnieuw liquificatie en desintegratie van de ECM-koepel veroorzaakt. Voeg na de incubatie 500 microliter verwarmde AOSM toe aan elke put door de wand van de put af te geven. Pipetteer media niet rechtstreeks op de ECM-koepel.Verander de media om de twee dagen gedurende vier tot zeven dagen. Om de media te aspirateren, kantelt u de plaat in een hoek van 45 graden en aspirateert u vanaf de onderrand van de put weg van de ECM-koepel. Start na vier tot zeven dagen organoïde differentiatie door 500 microliter verwarmde Airway Organoid Differentiation Media, of AODM, aan elke put toe te voegen en verander de media om de twee dagen gedurende zeven dagen.Plaats de kweekplaat met de luchtwegepitheelcelmodellen in de microscoopplaatinzet en sluit de microscoopomgevingskamer. Laat het monster gedurende 30 minuten in evenwicht zijn in de voorverwarmde, 37 graden Celsius, 5% met koolstofdioxide gevulde microscoopkamer. Richt je bij het microscoop-oculair op het celmodel.Klik vervolgens met behulp van de acquisitiesoftware op L100 om het lichtpad naar de poort te schakelen waar de camera is gemonteerd. Klik op de groene knop Afspelen om het gezichtsveld van de microscoop via de software te visualiseren. Controleer of de trilharen scherp zijn en pas indien nodig aan.Als u time-lapse-afbeeldingen uit het menu wilt halen, klikt u op Acquire en vervolgens op Fast TimeLapse. Selecteer in het pop-upvenster een opslaglocatie en bestandsnaam, verkrijg 1.000 frames. Om een voorbeeld van de trilharen in het gezichtsveld van de microscoop te bekijken, klikt u op Toepassen, gevolgd door de groene knop Afspelen.Pas indien nodig de Z-scherpstelling aan. Klik vervolgens op Nu uitvoeren om de Fast TimeLapse vast te leggen. Na het installeren van de benodigde software en toolboxen, kopieert u de juiste aangepaste analysescripts en de map ondersteuningsscripts naar het lokale station van de computer.Klik vervolgens in de computersoftware op het tabblad Start, gevolgd door Pad instellen. Klik in het pop-upvenster op Toevoegen met submappen. Selecteer onder het MATLAB-zoekpad de weergegeven map.Klik vervolgens op Opslaan en sluiten. Controleer of de analysescripts zijn gekoppeld aan de computersoftware door te controleren of ze in het linkerdeelvenster worden weergegeven. Breng vervolgens de voorbeeld raw-afbeeldingsbestanden over die zijn verkregen naar het lokale station van de computer.Klik vervolgens op de BeatingCiliaBatchOMEfiles_jove. m analyse scriptbestand in de computersoftware. Om het script uit te voeren, klikt u op het tabblad Editor, gevolgd door de groene knop Afspelen.Selecteer in het promptvenster dat verschijnt de RAW-afbeeldingsbestanden die u wilt analyseren. Voer de belichtingstijd voor acquisitietijd per frame in. Klik vervolgens op OK. Voer de GetFirstAmplitude uit.m script op de map die de AveSpectrum bestanden bevat. Wacht tot het script de FirstAmplitudeStacked uitvoert. xlsx-bestand, dat de hoogste amplitudefrequentie bevat en binnen het fysiologische bereik van luchtwegepitheel cilia beating ligt.Kopieer de frequentiewaarden van de FirstAmplitudeStacked. xlsx-bestand en plot ze met behulp van wetenschappelijke analysesoftware. De resultaten van de trilhaarslagfrequentie gemeten in luchtwegepitheelcel ALI-modellen afgeleid van drie deelnemers met cystische fibrose en drie gezonde controledeelnemers worden gepresenteerd.Op dag 14 van cultuurdifferentiatie waren kloppende trilharen aanwezig. Vanaf dag 14, dag 21 van de cultuurdifferentiatie, werd geen statistisch significante toename van de trilhaarslagfrequentie waargenomen tussen cohorten. Op dag 21 van de cultuurdifferentiatie was de gemiddelde trilhaarslagfrequentie voor gezonde controledeelnemers significant hoger dan die van deelnemers met cystische fibrose.Verder, toen de trilhaarslagfrequentie werd afgebeeld in dezelfde celmodellen na het verwijderen van slijm, was er een statistisch significante toename van de trilhaarslagfrequentie wanneer slijm werd verwijderd in ALI-modellen van zowel gezonde personen als mensen met cystische fibrose. De beeldvormende omgeving moet strikt worden gereguleerd, omdat de trilhaarfunctie uiterst gevoelig is voor veranderingen in omgevingsfactoren. Dit platform heeft het potentieel om te worden opgericht voor patiëntspecifieke geneesmiddelenscreening om geneesmiddelen te kunnen identificeren die de CFTR-functie moduleren.
