急性膵炎(AP)は、その異常に活性化された酵素によるその後の管膨満および膵臓自己消化を伴う主要な膵管の閉塞を特徴とする膵臓の急性炎症である。その臨床症状には、局所的または全身性の炎症、腹痛、および血清アミラーゼの上昇が含まれる1,2。重症度分類3によると、APは軽度、中等度、重篤な形態で存在する可能性があり、その中でも、重度の急性膵炎(SAP)は、30%を超える高い死亡率のために最も懸念される状態である4。米国では、AP通信が入院の最も一般的な理由の1つであり、20万人以上の患者に影響を与えています5。さらに、AP、特にSAPは年々増加しており、若い年齢層に影響を与えています6。しかし、現在の臨床現場では効果的な治療法の選択肢が不足しています6,7。そのため、APに関与する分子機構を探究する必要があり、治療改善が容易になる。
APに関与するメカニズムを研究し、さまざまな治療モダリティの有効性を評価するためには、十分に確立された実験動物モデルが必要です。トランスジェニック株およびノックアウト株の容易なアクセス性と、in vivo評価に必要な薬物の用量を最小限に抑える小さなサイズにより、マウスはAP研究に好まれています。したがって、APのいくつかのモデルがマウスで開発されている8,9。
Niederauらは、カエルレイン10の静脈内投与によって誘導された軽度の膵炎ラットモデルから研究し、注射経路と同じ薬剤を用いて誘導された腺房細胞壊死を提示するSAPマウスモデルを開発しました11。このモデルには、非侵襲性、迅速な誘導、広い再現性、および適用性を含むいくつかの利点がありますが、主な欠点は、ほとんどの場合、軽度の形態のAPのみが開発され、それによってその臨床的関連性が制限されることです。アルコールはAPの主要な病因の1つと考えられています。しかし、Foitzikらは、外分泌過剰刺激などの他の要因と組み合わされた場合にのみ膵臓損傷を引き起こすと報告した12。さらに、アルコール誘発APモデルは、異なる投与経路および薬物用量を介して開発されたことが報告されているが13、14、15、それらの主な欠点は、それらを再現することの難しさである。L-アルギニンの腹腔内投与はまた、マウスにおいてAPを誘導することができる16;しかし、その低い臨床的関連性は、その適用を妨げる。胆汁酸塩であるタウロコール酸塩は、膵管注入を介してヒトAPに似た状態を誘導するために、1965年にCreutzfeldらによって最初に提案された17。病態生理学におけるその臨床的関連性については論争が存在しますが18,19、タウロコール酸誘発膵炎は依然としてSAPにとって不可欠なモデルです。

急性膵炎(AP)、特に重度の急性膵炎(SAP)の有病率は、若い年齢層で毎年増加しています。しかしながら、現在の臨床現場では有効な治療法が不足している。トランスジェニック株およびノックアウト株の入手が容易で、そのサイズが小さく、 in vivo 評価に必要な薬物の投与量を最小限に抑えることができるため、マウスにおける十分に確立された実験モデルがAP研究に好まれます。さらに、タウロコール酸ナトリウム(TC)を介して誘導されたSAPは、現在、最も広く使用されており、最も特徴付けられるモデルの1つである。このモデルは、APのプロセス中の新規治療法および可能な分子事象について調査されている。ここでは、タウロコール酸ナトリウムと簡単な自家製マイクロシリンジを用いたAPマウスモデルの作製を紹介します。さらに、その後の組織学および血清学的検査のための方法論も提供する。

このビデオの全体的な目標は、ビリオ膵管へのタウロコール酸ナトリウムの逆行性注射による急性膵炎のマウスモデルを確立することです。動物を含むすべての実験は、蘇州大学の動物倫理委員会によって承認されました。すべての外科的処置は、完全麻酔下で行った。鎮痛薬は、以前の文献によって証明され、蘇州大学の動物倫理委員会によって承認された病気の自然な経過を妨げないために使用されなかった。手術の8〜12時間前にマウスを速くする。5%タウロコール酸ナトリウム溶液フィルターを0.22マイクロフィルターを通して調製する。オートクレーブは、手術器具および材料を、鉗子、はさみ、ブレードホルダー、針ホルダー、止血クリップ、血管クランプ、および滅菌ドレープを含む。マイクロシリンジを照射により滅菌した。マウスでは、1kgあたり50ミリグラムの用量で1%ペントバルビタールナトリウム溶液の腹腔内注射で全身麻酔を誘発する。テープで照射されたフォーム平板の上にマウスを仰臥位に固定します。脱毛クリームでマウスの毛皮を剣状突起レベルから両側前上腸骨脊椎の接続線まで下ろし、左右の側面を前腋窩線に平行にする。0.5%ヨードフォアで皮膚を2〜3回拭き取り、毎回1〜2分間乾燥させます。マウスを滅菌ドレープで覆います。皮膚層を切り開き、皮下に開きます。約2〜3センチメートルの長さの中央腹部切開を行う。胃に沿って十二指腸を左から右に見つけます。手術用鉗子と消毒剤を浸した綿棒を使用して十二指腸を静かに左側に引き出し、膵臓、胆道膵管、および十二指腸乳頭を露出させます。8-0 を使用します。縫合糸は乳頭周辺の十二指腸と膵臓との接続を通過するように、縫合糸をマウス本体の左側に引っ張って十二指腸を固定する。皮膚と肝臓の下端をそっと脇に引き寄せます。一般的な肝管をクランプします。マイクロインジェクターを遠位胆道膵管に逆行的に突き刺す。十二指腸乳頭付近の針先をクランプで固定する。10マイクロリットルの5%タウロコール酸ナトリウムを毎分5マイクロリットルの割合でビリオ膵管に注入する。安定した圧力を達成するために胆道膵管を5分間閉じたままにする。クランプを取り外します。マイクロインジェクターを引き出し、もう一方のクランプを外して縫合糸を固定します。十二指腸を元の解剖学的位置に戻し、出血を確認する。6-0の縫合糸で傷口を閉じます。滅菌ドレープを取り外します。切開部を0.5%ヨードフォアで消毒する。37°Cのサーマルパッドで麻酔から動物を回復させます。体液補給のために0.8ミリリットルの0.9%生理食塩水を皮下に投与する。手術後24時間で、同じ用量および注射経路を用いてペントバルビタールナトリウムでマウスを麻酔する。腹部大動脈から全血を採取し、1, 500 x gで15分間スピンして血清学的検査のために血清を単離する。組織学評価のために膵臓および肺組織を収集する。膵臓を4%パラホルムアルデヒド溶液に24時間浸漬する。一連のアルコール濃度で脱水し、パラフィンに埋め込む。ミクロトームを使用して、5つのマイクロ厚の膵臓組織切片を調製する。膵臓の組織学的評価のためにH&E染色を行う。マウスはわずかに不活性であり、手術前と比較して手術後24時間で体重が減少した。コントロールおよびSAPマウスの生存率は、手術後72時間でそれぞれ100%および72.7%であった。膵臓切片からのH&E染色結果によると、SAPマウスでは膵臓組織の髄髄壊死が観察できたが、対照マウスでは有意な変化は認められなかった。さらに、アミラーゼ、リパーゼ、膵臓、および肺MPO活性のレベルが対照マウスと比較してSAPマウスにおいて有意に増加したことが見出された。さらに、ALT、AST、BUN、およびクレアチニンを含む肝臓および腎臓指数の有意に上昇したレベルも見出された。ビリオ膵管へのタウロコール酸ナトリウムの逆行性注射を用いて、マウスにおいて重度の急性膵炎を誘発することができた。成功した穿刺を確実にするために胆道膵管のよく露出.穿刺中, 胆道膵管の穿刺を避けるために、適切な角度と力を確保します.ここで述述するTC誘導SAPモデルは、重症急性膵炎に関与するメカニズムを研究するのに好適なモデルである。これは、より安価な試薬を使用した経済モデルであり、このビデオは、初心者の外科医をより速く訓練するのに役立ちます。
