1. 考试前准备

</ol>2. 阻抗测试负载验证

</ol>4. 确定可接受的测量值

与肺活量测定法不同，肺活量测定法建议最多尝试八次以避免疲劳，而FOT可能没有建议的最大重复次数，可能是因为它与努力无关的方法。在实践中，一些研究人员获得多达八次重复测量18，我们的实验室使用多达10次测量的类似经验法则。建立上限对于定义测试会话的结束实际上很重要。这样做对于患有呼吸道疾病的个体尤其重要，其中大于10%的CoV可能反映潜在的疾病过程，而不是努力不足。Harkness等人最近描述了他们与这些患者群体的经历，并建议更自由的临界值（CoV高达20%）仍然可以报告用于临床解释。每个诊所和研究实验室都应该在实际决策之间取得平衡，例如时间限制，考生的能力和疲劳程度，以及达到CoV截止值的可能性。需要考虑的一种方法是实施评分系统。例如，一旦从最多10次尝试中获得至少三次无伪影的重复测量，应用对应于CoV水平的字母等级 - 即"A"≤10%;"B">10%和≤15%;"C">15%和≤20%;和"D">20%。要考虑的其他策略可能包括修改软件和硬件采集参数，以实现更完整的呼吸。例如，一些制造商的设置可以适应更长的记录持续时间和/或延长的记录时间，以实现超过ERS建议的至少三次完整呼吸。在报告FOT结果时，必须披露所有采集参数，以便于解释和与其他已发表文献的比较。FOT采集参数将继续积极研究，并可能导致未来对FOT性能和测量的修改。
本文旨在重点介绍FOT的最新技术和应用，并为成人测试提供标准化方案。然而，重要的是要认识到FOT的相关局限性。首先，阻抗测量对胸外影响等伪影尤其可疑4。因此，目前的协议侧重于最大限度地减少这种影响，例如确保在采集过程中适当的脸颊支撑。此外，流量中断（例如，舌头覆盖烟嘴、吞咽、误呼吸）妨碍了准确的测量，并导致 Zrs 计算的有效呼吸次数减少19。其次，尽管从患者的角度来看，FOT很容易执行，但对于技术人员和临床医生来说，识别这些伪影以及解释输出是一项挑战20。例如，当前的FOT设备产生大量数据来表征个人的呼吸力学;然而，缺乏参考值和围绕关键变量的共识是减缓其临床采用的因素。同样，虽然建议至少获得三个无伪影试验4，但如果进行了三个以上的试验并发现可接受，则目前对于选择使用哪些试验来代表测试会话的推荐方法没有达成共识。因此，FOT在各种气道疾病中的临床应用仍在继续积极研究。最后，从技术角度来看，FOT制造商在以下方面存在异质性：i）频率波形，ii）错误检测算法，以及iii）呼吸间和呼吸内分析2，21，22，23，24。上述许多限制可以通过遵循标准化协议以及输出和记录参数的透明报告来解决。

FOT不能替代肺活量测定法，而是肺功能的补充评估。然而，与肺活量测试相比，它可能具有几个优点，包括在潮汐呼吸期间进行的测量（与努力无关），以及评估肺活量测定法不可行的远端或小气道的潜力5。因此，FOT在儿科环境中获得了相当大的普及6，7，以及用于评估肺活量测定正常或保留的有症状患者8，9，10，11。FOT 在支气管激发试验期间也显示出临床效用，其中症状与 FOT 的相关性比肺活量测定法更强12。此外，FOT 需要较低剂量的支气管游走剂，以诱导可测量的呼吸功能差异13。

employing the forced oscillation technique for the assessment of respiratory mechanics in adults

首先，将健康成年人的案例作为数据采集的实际示例，以及技术人员如何选择单个测量值进行报告（案例示例1）。其次，提供了一个临床示例，该患者在支气管扩张剂之前和之后因不明原因的呼吸困难而被转诊以进行FOT获取，重点是解释（病例示例2）。请注意，在这些案例示例中，已特意使用了来自两个不同制造商的 FOT 器件来说明通用方法。材料 表中提供了其他详细信息。
案例示例 1 FOT在一名健康的25岁西班牙裔女性（身高：164厘米，体重：84.9公斤）中进行。参与者从不吸烟，否认呼吸道症状，并且没有肺部疾病史或其他重要的既往病史。她戒掉了咖啡因（≥8小时）和剧烈运动（≥24小时）。她最近进行了一次肺活量检查，结果显示正常，没有梗阻或限制体征：FEV1/FVC：0.88，FEV1：3.30 L（98%预测），FVC：3.70 L（97%预测）。
在解释和演示测试程序后，在记录之间大约1-2分钟的时间内获得了三个FOT测量值。目视检查和软件的质量控制算法没有发现任何伪影。然后检查前三次测量的5 Hz Rrs以确认会话内的CoV（单个测量值：3.06，3.79，3.46 hPa·s ·L-1;平均： 3.44 百帕·秒·L-1，标准偏差：0.36 hPa·s L-1，CoV = 标准偏差/平均值 = 0.36 / 3.44 = 0.105 * 100 = 10.5%）。
由于前三次测量的CoV为>10%，因此需要进行额外的测量。获得第四次测量（5 Hz时的RR = 3.40 hPa·s·使用所有测量值（单个测量值：3.06，3.79，3.46，3.40 hPa·s·）重新计算L-1）和会话内CoVL-1;平均： 3.43 百帕·秒·L-1;标准偏差：0.30 hPa·s·L-1;CoV = 标准偏差 / 平均值 = 0.30 / 3.43 = 0.087 * 100 = 8.7%）
由于满足了会话内CoV标准，因此将平均FOT指数计算为测量的平均值。这些测量结果如图 1所示，并在 表 1中报告。此外，为了便于与预期值进行比较， 表 2 使用考虑年龄、性别和体重的标准参考方程，列出了所有 FOT 指数（其中预测值可用）、正常下限 （LLN）、正常上限 （ULN）、预测百分比和 Z 评分的预测值14。
案例示例 2 一名 48 岁高加索男性（身高：185 厘米，体重：89 kg）被转诊至我们的中心，评估无明显原因（例如药物、呼吸道或心血管疾病或精神健康合并症）的慢性咳嗽和劳力性呼吸困难。他一生从不吸烟，但在伊拉克7个月的军事部署期间，他赞成接触蒸气，气体，灰尘和烟雾。进行完整的肺功能检查（即体体积描记法、支气管扩张剂肺活量测定法和一氧化碳肺扩散能力），所有结果均在正常范围内。在给予支气管扩张剂之前和之后15分钟进行FOT（通过带垫片的定量吸入器4喷100μg沙丁胺醇）（图2）。个别试验数据和平均值在表3支气管扩张剂给药前后呈现;由于每项试验在技术上都可接受，因此支气管扩张剂前后的测量结果及其绝对和相对差异报告于表4中。此外，还使用考虑年龄、性别和体重的标准参考方程报告预测值、预测值百分比、LLN 和 ULN14。
我们划分了 表 3 和 表 4 中报告的变量，以简化两个概念的说明：1）确定异常与正常反应，以及2）支气管扩张剂的可逆性。对于RR测量，超过ULN的值（即，升高的电阻）被认为是异常的。在这里，支气管前扩张剂RRS在4赫兹（3.32小时帕·秒·L-1）超过ULN（2.59 hPa·s·L-1），并且是预测值的155%（[3.32 / 2.14] * 100 = 155.14）。在支气管扩张剂给药后，4 Hz处的RRS降低了45.78%，超过了Oostveen等人报告的第 95百分位（即4 Hz时RR为-32%）。这种反应表明在耐药性中支气管扩张剂反应呈阳性。此外，支气管扩张后观察到的值被归一化（即，成为被认为是正常值的代表），并且是预测值的84.1%（[1.80 / 2.14] * 100 = 84.11）。
4 Hz时的Xrs有不同的解释，因为观察到的值为负数。因此，异常值是那些超过LLN的值（即，更多的负电抗）。在这里，该个体有一个支气管前期扩张剂（-0.98 hPa·s·L-1）和支气管扩张后（-0.83 hPa·s·L-1） 高于 LLN 的值 （-1.11 hPa·s·L-1）。支气管扩张剂前期与支气管扩张剂后的差异约为15%，低于Oostveen等人报告的第 95百分位（即，在4 Hz时Xrs中+33.8%）。因此，所有 Xrs 值都被视为正常值。
电抗面积（或AX）是低频电抗的积分面积，因此是正值。异常AX值是那些超过ULN的值，反映更多的负电抗。像4赫兹的Xrs一样，支气管前扩张剂AX（2.77 hPa·s·L-1）和支气管扩张后AX（1.23 hPa·s·L-1）都低于ULN。虽然支气管扩张剂前后值降低了-55%，但这低于Oostveen等人报告的第 95百分位（即AX在4 Hz时为-56.0%）。综上所述，AX也被认为是正常的。
