在商业DNA合成之后，可以快速组装脚趾开关，然后通过对对应于靶基因组短区域（200-300 nt）的合成靶触发序列进行初始筛选来进行测试。为了筛选基于脚趾开关的传感器的性能，最好以RNA的形式添加靶序列。在本文中，概述了添加 体外 转录触发RNA所需的步骤。但是，如果可用，可以使用全长基因组模板，例如定量的病毒RNA提取物或商业合成RNA基因组或标准品。使用全长RNA基因组进行初始脚趾开关筛选是有益的，因为它可以告知其他因素（如RNA二级结构）是否会影响传感器性能。为了优化候选开关的开/关比，可以将脚趾开关DNA滴定到无细胞反应中。此步骤还可用于识别高性能的脚趾保持开关（折叠扩增或高 ON/OFF 比），并从下游表征步骤中省略泄漏的脚趾保持开关（在没有靶 RNA 的情况下的高信号）。
为了提高性能最佳的脚趾开关候选物的检测限，NASBA用于将靶标寨卡病毒RNA的临床相关浓度提高到可以通过脚趾开关检测到的水平6。筛选正向和反向引物组的不同组合，以确定最佳的NASBA引物和脚趾开关组合，以便在临床相关浓度下进行检测。一旦确定了理想的引物组和脚趾开关组合，该测定将进行临床验证。需要注意的是，脚趾开关和NASBA筛选阶段可能是劳动力和资源密集型的，因此测试开发最适合资源充足的研究地点。虽然我们还没有应用流程自动化，但这很可能会加速迭代设计、构建和测试周期32。幸运的是，从传感器设计和测试到部署的周转时间可能非常短（不到一周），这使得该策略非常适合时间紧迫的情况，例如流行病爆发6。

正如我们在 COVID-19 大流行期间所看到的那样，用于执行 RT-qPCR 的仪器本身可能成为瓶颈并限制患者获得检测的机会。这一因素在很大程度上也是财务因素，导致集中测试模式可以限制诊断访问。例如，在2015/2016年寨卡疫情期间，巴西只有五个国家参考实验室，这导致患者检测延迟。在不考虑规模经济的潜在好处的情况下，便携式酶标仪目前的商品成本为~500美元，即使考虑到劳动力和商业利润而增加五倍，仍然提供了一种负担得起的工具。相比之下，RT-qPCR 仪器的成本从 15，000 美元到 90，000 美元不等，34 美元不等。此外，拉丁美洲无细胞检测的每次检测费用约为5.48美元，而在寨卡病毒爆发时，巴西RT-qPCR的每次检测费用为~10-11美元36。除了设备成本外，便携式酶标仪占地面积小（20 cm3），自动分析， 数据 通过互联网上传到云端，并且可以使用电池供电。这些功能极大地扩展了可以部署测试的潜在设置，并同时扩大了可以服务的患者群体。
迄今为止，最常见的商用大肠杆菌CFPS平台是S30和PURE系统37;然而，改善低收入和中等收入国家获得诊断服务的一个关键考虑因素是这些试剂的国内供应有限。解决这一挑战的一个重要步骤是发展当地的CFPS生产。Federici实验室最近在开发一个非商业平台方面取得了重大进展，该平台在基于裂解物的无细胞系统中实施基于脚趾开关的传感器，寨卡病毒RNA14达到2.7 fM LOD。这一成就不仅允许试剂在使用国生产，避免进口关税和延误，而且劳动力成本也随着当地费率的调整，因此总体成本可以显着降低。在Federici小组概述的工作中，智利生产CFPS表达反应（5μL）的成本为6.9美分（美元）35，38，为实施基于裂解物的系统提供了双重激励（改善物流和成本）35，38。
将RT-qPCR可比检测放入分布式诊断网络可以带来比当前依赖于将样本运输到集中式RT-qPCR设施的做法的显着优势。在寨卡病例集中的城郊地区，患者与诊断机构之间的物理距离会减慢诊断速度，并增加结果无法在临床相关时间到达患者的风险。我们希望，这里介绍的工作能够有助于使研究界能够通过知识转让，为人类健康、农业和环境监测创造分散的生物技术和便携式硬件。

按照计算设计流程，通过PCR构建了三个脚趾开关。使用琼脂糖凝胶电泳分析PCR产物（图2）。存在大约3，000 bp的清晰条带，大约与脚趾开关偶联的 lacZ 基因的大小大致相同，通常表明反应成功。或者，没有条带、多个条带或条带大小不正确的泳道表示 PCR 失败。在PCR失败的情况下，应优化反应条件和/或引物序列。
对组装好的脚趾开关进行筛选，以评估每个传感器各自的 体外 转录触发RNA（图3）。虽然所有三个传感器都显示出OD570 吸光度的增加，但开关27B（图3A）具有最快的导通速率。在没有靶标触发RNA的情况下，开关33B和（在较小程度上）47B显示出OD570 吸光度增加，表明这些开关具有一些背景活性或泄漏性（图3B，C） - 这是候选传感器不需要的特性，因为它会降低特异性。为了更清楚地识别具有最高ON/OFF信号比的传感器，计算了OD570 吸光度的倍数变化（参见补充信息第5节）并绘制了图（图4）。从该分析中可以清楚地看出，开关27B是具有最佳性能的传感器，开/关比约为60。
然后通过确定与NASBA反应偶联时激活脚趾开关所需的最低RNA浓度来评估性能最佳的脚趾开关（27B）的灵敏度。该图表明，性能最佳的寨卡传感器可以检测浓度低至每μL1.24个分子（相当于~2 aM; 图5）。
在确定并验证开关27B后，传感器材料被分发给巴西伯南布哥州累西腓的团队成员。在巴西，使用寨卡病毒患者样本评估寨卡病毒诊断平台的临床诊断准确性，并结合RT-qPCR进行比较。为了验证纸质寨卡诊断平台，使用了便携式酶标仪，该酶标仪能够孵育和读取纸基传感器的比色输出。颜色从黄色变为紫色用于识别阳性样品，而阴性样品保持黄色（图6）。可视化便携式酶标仪生成的结果（图8）的另一个选项是绘制每个纸基反应随时间变化的比色响应。样品一式三份进行测试，超过阈值（红线设置为1）的样品被认为是阳性的，而低于阈值的样品被认为是阴性的（图6 和 图7）。
最后，为了评估和比较寨卡脚趾开关传感器与当前诊断寨卡病毒感染的金标准方法的临床性能，所有患者样本均与RT-qPCR并行测试。将两个代表性患者样本的扩增图一式三份用于通过RT-qPCR检测寨卡病毒（图9）。当循环阈值 （Ct） 值为 ≤38 时，样品被视为阳性;红线表示阳性样本，蓝线表示寨卡病毒阴性样本。

我们的协议描述了基于基因回路的诊断的设计、组装和验证。这些基于纸张的分子诊断方法成本低且灵敏，能够检测临床相关浓度的核酸，并且可以设计用于检测几乎任何序列。这是一种平台技术，可以由用户根据他们的需求设计，并有可能将临床级诊断带入更分散的牙科资源设置。无细胞脚趾开关传感器几乎可以设计用于任何基于核酸的靶标。最近的工作已经证明了埃博拉、诺如病毒、SARS-CoV-2、艰难梭菌和致伤寒细菌的无细胞诊断。演示该程序的将是博士后研究员Severino Jefferson Rivero da Silva和我实验室的博士生Pouriya Bayat。要设计脚趾开关，请从寨卡病毒基因组中识别靶序列，并从扩增子中选择寨卡病毒靶序列，如文本协议中所述。下载脚趾开关设计软件包。打开 MATLAB 并导航到设计软件文件夹。将目标序列输入到位于输入子文件夹中的设计输入文件 csv 文件中。选择要用于设计功能的参数。运行设计功能，为感兴趣的目标生成脚趾开关设计。完成后，导航到final_designs文件夹，并在 csv 格式的电子表格中找到顶部脚趾开关设计序列和相应的目标序列。确保算法生成的脚趾开关 DNA 序列在五个素数端包含 T7 启动子序列，在三个素数端包含保守的 21 个核苷酸接头序列。使用 NCBI BLAST 通过检查序列同源性来筛选针对其他常见病毒的顶部脚趾开关设计序列。接受血化低于 40% 的序列。在无核酸酶水中制备浓度为10微摩尔的脚趾开关发夹DNA寡核苷酸和反向扩增引物的溶液。根据此表在冰上的PCR管中组装反应。按照本表中列出的循环条件将反应管放入热循环仪中。分析琼脂糖凝胶上的PCR产物。纯化PCR产物并在最小体积的无核酸酶水中洗脱DNA，以确保足够高的浓度。使用分光光度计定量DNA。通过将25毫克粉末溶解在一毫升无核酸酶的水中来制备CPRG储备溶液。根据此处所示的标准方案在冰上制备预混液。将无细胞预混液分配到PCR管中。对于无细胞对照和单独开关对照，加入无核酸酶水至5.94微升体积。对于反应，加入PCR纯化的脚趾开关DNA，得到终浓度为33纳摩尔。为了测试脚趾开关和靶RNA组合，将体外转录的靶RNA添加到终浓度为1微摩尔。通过移液和短暂离心彻底混合所有反应。在黑色透明底384孔板上，向反应孔周围的孔中加入30微升无核酸酶水。然后使用两毫米活检冲头和镊子，切下BSA堵塞的滤纸盘并将它们放入反应孔中。从每个反应管中分配1.8微升到384孔板中的滤纸盘上，一式三份。用透明的PCR膜覆盖板并将其放入酶标器中。每分钟在570摄氏度下测量37纳米处的吸光度，持续130分钟。在无核酸酶水中制备所有正向和反向引物组的 25 μmol 储备溶液。使用此处所示的预混液设置五微升反应。通过移液混合，直到白色沉淀溶解，然后分配到PCR管中。将正向和反向引物添加到适当的试管中，然后加入一微升无核酸酶水或两皮摩尔靶触发RNA。通过轻轻移液混合并短暂旋转试管。在热循环仪上设置孵育方案，如下所示。12分钟后，取出试管并向每个试管中加入1.25微升酶混合物，然后混合和离心。跳过41摄氏度保持步骤后，将试管放回热循环仪，开始一小时的反应孵育。然后，为了评估引物性能，组装基于纸张的无细胞脚趾开关反应并分析数据。对于灵敏度分析，鉴定候选引物对，并在无核酸酶水中制备靶RNA的连续稀释液。用生物一式三份中的选定引物组重复NASBA和无细胞反应。使用一微升提取的患者RNA，进行NASBA扩增和基于纸张的无细胞反应，如第五节所示。反应后，准备384孔反应板，并在便携式酶标仪中以37摄氏度运行测定。然后组装RT-qPCR组分，并根据该表将试剂添加到1.5毫升微量离心管中。通过移液混合反应。将 6.5 微升分配到 96 孔或 384 孔 PCR 板的每个孔中，然后每个 RNA 模板 3.5 微升一式三份。将PCR膜放在板的顶部。将 384 孔板以 600 倍 G 离心两分钟。将板放入RT-qPCR仪中并运行循环条件，如下所示。按照计算设计，构建了三个脚趾开关，并使用琼脂糖凝胶电泳进行分析。大约3，000个碱基对的条带表明反应成功。根据各自的体外转录触发RNA评估脚趾开关。虽然所有三个传感器都显示出吸光度的增加，但传感器27B的导通速率最快，而开关33B和47B的开/关比较低，表明背景活性和特异性降低。570 nm处吸光度的倍数变化表明，开关27B在开/关信号比下具有最佳性能。此外，当与NASBA结合使用时，它可以检测浓度低至每微升124个分子的RNA，表明灵敏度高。对来自巴西的寨卡病毒患者样本进行了测试，以使用便携式酶标仪评估传感器的临床诊断准确性。颜色从黄色变为紫色，鉴定出阳性样品。通过PLUM便携式酶标仪上的集成软件绘制每个纸质反应的比色响应随时间的变化。超过阈值的样本被认为是阳性的。通过与RT-qPCR进行比较，确定了传感器的临床性能。当循环阈值等于或小于38时，样品被认为是阳性的。在进行患者试验之前，重要的是要确保脚趾开关屏幕和NASBA引物灵敏度的结果是可重复和优化的。鉴于其简单性和适应性，这里描述的诊断平台为开发新的护理点工具铺平了道路，这些工具可以为卫生系统带来好处，特别是对低收入国家。
