급성 췌장염(AP)은 가장 흔한 소화기 질환 중 하나이며 환자에게 막대한 의학적 및 경제적 부담을 줍니다. AP 환자의 약 20%는 감염된 괴사 또는 지속적인 장기 기능 장애로 복잡해지는 중증 급성 췌장염(SAP)을 앓고있습니다1. SAP는 일반적으로 더 높은 이환율 및 사망률(최대 30%)과 관련이 있습니다1. 지속적인 장기 기능 장애가 있거나 경피적 배액 (PCD) 후 회복되지 않거나 위장 또는 담즙 폐쇄로 고통받는 감염된 괴사가있는 SAP 환자의 경우 수술 적 괴사 조직 제거를 고려해야합니다 1,2.
최소 침습 시대에는 내시경 경막 괴사 절제술, 복강경 또는 개방 위 괴사조직 제거술, 스텝 업 접근법 1,2의 일부인 비디오 보조 복막 괴사조직 제거(VARD)를 포함하여 개복 수술 외에 수술 괴사조직 제거에 대한 여러 접근 방식이 있습니다. VARD는 감염된 괴사의 왼쪽 분포가 부실성 시궁창으로 확장되거나 후복막깊숙이 확장되는 환자에게 선호되는 접근 방식입니다2. 광범위한 후 복막 유착으로 인해 괴사 성 파편과 인접한 염증성 정상 조직 사이의 복강경 검사에서 백색광 이미지에서 시각적으로 명확한 분리 표면이 거의 없기 때문에 VARD는 필연적으로 혈관 손상, 외부 췌장 피부 또는 장 피부 누공 3,4,5의 위험을 동반합니다.
인도시아닌 그린(ICG)을 사용한 실시간 수술 중 근적외선 형광 영상은 장 6,7의 관류 평가와 담즙 및 혈관 해부학 8,9의 시각화를 용이하게 하기 위해 적용되었습니다. VARD의 위와 같은 단점을 고려하여 VARD에서 ICG를 사용한 실시간 근적외선 형광 이미징을 적용하여 관류가 잘 된 인접한 정상 조직 및 혈관 구조를 시각화 할 수 있습니다. 이 변형 된 기술 (ICG 유도 VARD)은 수술 적 괴사 조직 제거 중에 명확한 분리 표면을 제공하고 혈관 또는 장 손상의 위험을 줄일 수 있습니다. ICG 유도 VARD는 외과의가 중증 급성 괴사성 췌장염을 치료할 때 보다 안전한 괴사조직 제거를 수행하는 데 도움이 될 수 있습니다.

이 프로토콜은 중증 급성 괴사 성 췌장염 치료를위한 인도시아닌 녹색 유도 비디오 보조 후 복막 괴사 조직 제거를 제공합니다.
