脳卒中後の痙縮(PSS)または脊髄損傷(SCI)の評価には、ヒトの運動および痙縮の神経基盤を評価するためにH反射が使用されてきました1。測定間および被験者間のH反射の変化の定量化の改善は、H波とM波の比率(H / M比)を使用することによって達成されます。あるいは、速度依存性うつ病(RDD)は、上昇周波数のセット(例えば、0.1、0.5、1.0、2.0、および5.0Hz)を使用して測定される。RDDは、脳卒中またはSCIによって乱される可能性のある抑制性回路の完全性を反映しています。すべての神経回路が無傷の場合、H反射の均一で周波数に依存しない抑制があります。しかし、脳卒中またはSCIの結果として神経抑制が低下する場合、H反射の抑制は刺激頻度の増加とともに減少する4。
表面電極を使用した正しい電気生理学的記録は困難な場合があり、運動課題、抑制メカニズム、およびαMN興奮性の影響を受ける可能性があります5。げっ歯類の経皮的記録では、刺激電極が脛骨神経の近くに配置され、記録電極が前足の関連する筋肉の近くに配置されます。しかし、私たちの経験によれば、経皮電極の正しい配置(図1A)は、げっ歯類ではヒトの表面電極の配置よりもさらに複雑で可変です。これは、H反射を引き出すために必要な長さ、周波数、および刺激強度の違いにつながる可能性があります。これらの方法論的課題は、H反射測定研究の数が非常に限られている理由を説明することができます(たとえば、実験的脳卒中モデル3,4、およびその他の痙縮モデル6。個々の神経に対するH反射の正確な(長期の)刺激および記録は、原則として、標的神経を囲む埋め込み型電極を用いて達成することができる7,8。動物に潜在的な副作用とプローブの潜在的な不安定性を伴う困難な手術のために、このアプローチは現場では標準にはなりませんでした。ここで紹介する方法には、ある程度の外科的専門知識も必要です。しかし、それは、隣接する神経の同時刺激を回避する低刺激強度を使用して、in vivoで孤立した神経の新規で正確な刺激と記録を可能にします。

Terminal H-reflex Measurements in Mice
