Als een innovatieve onderzoeksbehandeling is FMT de afgelopen jaren een hot topic geworden bij de behandeling van verschillende aandoeningen, omdat dysbiose van de commensale microbiota betrokken is bij de pathogenese van meerdere menselijke ziekten, waaronder IBD, obesitas, diabetes mellitus, autisme, hartaandoeningen en kanker26. Het menselijke darmmicro-ecosysteem is uiterst complex, met meer dan 1000 soorten bacteriën in de darm van een gezond persoon1. De darmflora is betrokken bij het behoud van de normale fysiologische functies van de darm en de immuunrespons en heeft een onafscheidelijke relatie met het menselijk lichaam. Accumulerend bewijs suggereert dat het darmmicrobioom het laatste menselijke orgaan vormt, dat deel uitmaakt van het menselijk lichaam, niet alleen een groep parasieten2. Een 'gezonde' symbiotische relatie tussen de darmmicrobiota, hun metabolieten en het immuunsysteem van de gastheer dat in het vroege leven is vastgesteld, is van cruciaal belang voor het behoud van de darmhomeostase. Bij sommige abnormale aandoeningen zoals chronische ontsteking verstoren veranderingen in de interne en externe omgeving van het lichaam de darmhomeostase ernstig, wat resulteert in een aanhoudende onbalans van de microbiële gemeenschap van de darm, dysbiose genaamd3. In feite kan blootstelling aan meerdere omgevingsfactoren, waaronder voeding, medicijnen en pathogenen, leiden tot veranderingen in de microbiota.
Dysbiose is geassocieerd met de pathogenese van een verscheidenheid aan darmziekten, zoals inflammatoire darmziekte (IBD), prikkelbare darmsyndroom (IBS) en pseudomembraneuze enteritis, evenals een groeiende lijst van extra-intestinale aandoeningen, waaronder hart- en vaatziekten, obesitas en allergie4. Microbiotaprofilering toonde aan dat patiënten met IBD een dramatische afname van de bacteriële diversiteit hebben, evenals duidelijke veranderingen in de populaties van sommige specifieke bacteriestammen 5,6. Deze studies toonden minder Lachnospiraceae en Bacteroidetes aan, maar meer Proteobacteria en Actinobacteriën bij IBD-patiënten. Er wordt aangenomen dat de pathogenese van IBD gerelateerd is aan verschillende pathogene factoren, waaronder abnormale darmflora, ontregelde immuunrespons, omgevingsuitdagingen en genetische varianten7. Overvloedig bewijs suggereert dat darmbacteriën een rol spelen in de initiatie- en toepassingsfasen van IBD 8,9, wat aangeeft dat het corrigeren van darmdysbiose een nieuwe benadering kan zijn voor de therapie en / of onderhoudsbehandeling van IBD.

In 1958 behandelden Dr. Eiseman en zijn collega's met succes vier gevallen van ernstige pseudomembraneuze enteritis met fecale materie van gezonde donoren via klysma, waarmee een nieuw hoofdstuk in de moderne westerse geneeskunde werd geopend met behulp van menselijke uitwerpselen om menselijke ziekten te behandelen11. Clostridium difficile-infectie (CDI) is de belangrijkste oorzaak van pseudomembraneuze enteritis12 en FMT is zeer effectief bij de behandeling van CDI. In de afgelopen acht jaar is FMT een standaardbehandelingstherapie geworden voor de behandeling van recidiverende CDI13, wat aanleiding is geweest voor verdere studies naar de rol van FMT bij andere aandoeningen, zoals IBD. In de afgelopen twintig jaar hebben talloze case reports en cohortstudies het gebruik van FMT bij patiënten met IBD14 gedocumenteerd. Een meta-analyse, waaronder 12 onderzoeken, toonde aan dat 62% van de patiënten met de ziekte van Crohn (CD) klinische remissie bereikte na FMT, en 69% van de CD-patiënten had klinische respons15. Ondanks deze bemoedigende bevindingen blijft de rol van FMT bij de behandeling van IBD onzeker en zijn de mechanismen waarmee FMT darmontsteking verbetert slecht begrepen. Verder onderzoek is nodig voordat FMT zich kan aansluiten bij het huidige armamentarium van behandelingsopties voor IBD in de klinieken.
In dit protocol pasten we FMT toe op IL-10-/- muizen, die spontaan colitis ontwikkelen na het spenen en hebben gediend als een gouden standaard om de multifactoriële aard van IBD 16,17,18 te weerspiegelen. IL-10−/− muizen zijn op grote schaal gebruikt om IBD-etiologie te ontleden omdat ze vergelijkbare moleculaire en histologische kenmerken vertonen als IBD-patiënten, en net als patiënten kan de ziekte worden verbeterd met anti-TNFα-therapie16. Ouder wordende IL10−/− muizen (>9 maanden oud) hebben een grotere hartomvang en verminderde hartfunctie in vergelijking met leeftijdsgematchte wild-type muizen19, waardoor het een uitstekend model is voor het bestuderen van colitis-geïnduceerde hartaandoeningen. Andere muizenmodellen van colitis, zoals het dextran natriumsulfaatmodel en het T-cel-geïnduceerde colitismodel, kunnen echter ook worden gebruikt. We dienden fecale suspensie toe via orale maagsonde, waarvan bewezen is dat het een effectieve en betere route is dan klysma bij mensen20.

Met de ontwikkeling van micro-ecologie in de afgelopen jaren heeft de relatie tussen darmbacteriën en inflammatoire darmziekten (IBD) veel aandacht getrokken. Het verzamelen van bewijs suggereert dat dysbiotische microbiota een actieve rol speelt bij het activeren of verergeren van het ontstekingsproces bij IBD en dat fecale microbiotatransplantatie (FMT) een aantrekkelijke therapeutische strategie is, omdat het overbrengen van een gezonde microbiota naar IBD-patiënt de juiste gastheer-microbiota-communicatie zou kunnen herstellen. De moleculaire mechanismen zijn echter onduidelijk en de werkzaamheid van FMT is niet erg goed vastgesteld. Verdere studies in diermodellen van IBD zijn dus noodzakelijk. In deze methode pasten we FMT toe van wild-type C57BL / 6J-muizen naar IL-10-deficiënte muizen, een veelgebruikt muismodel van colitis. De studie gaat dieper in op het verzamelen van fecale pellets van de donormuizen, het maken van de fecale oplossing / suspensie, het toedienen van de fecale oplossing en het monitoren van de ziekte. We ontdekten dat FMT de hartinsufficiëntie bij IL-10 knock-out muizen aanzienlijk verminderde, wat het therapeutische potentieel voor IBD-beheer onderstreepte.

We voerden 3 keer gezonde donor FMT uit (eenmaal per maand gedurende 3 maanden) op 2 maanden oude C57BL / 6J wild type (WT) en IL-10 knock-out muizen. Leeftijdsgematchte C57BL/6J-muizen (leeftijdsverschil moet <2 maanden) dienden als fecale donoren en telkens werden verse fecale pellets gebruikt. Drie klinische experimenten suggereerden dat fecale microbiota-transplantatie het therapeutische potentieel heeft voor een aantal menselijke ziekten. Verdere mechanistische studies zullen helpen om FMT van bank naar bed te brengen. Deze methode neemt orale maagsonde aan voor de leveringsroute, waardoor een nauwkeurigere volumelevering en de snelle absorptie mogelijk is.Het vereist slechts matige technische vaardigheden. De procedure wordt gedemonstreerd door Xioying Zhong, een senior onderzoeksmedewerker van ons laboratorium. Begin met het bereiden van steriele papieren handdoeken, stompe eindtangen en conische buizen van 50 milliliter.Plaats wat papieren handdoeken en tangen in aparte autoclaafzakken en autoclaaf ze op 180 graden Celsius in droge hitte gedurende 30 minuten. Weeg de steriele kegelvormige buizen en noteer hun gewicht op de buizen. Zet de bioveiligheidskast in de dierenkamer aan.Neem een geautoclaveerde schone muizenkooi zonder beddengoed en plaats deze in de bioveiligheidskast. Verwijder de deksel en het voedselrek en plaats ze in de kast. Leg wat steriele papieren handdoeken op de bodem van de kooi en plaats het metalen rek terug op de bovenkant van de kooi.Identificeer leeftijdsgematchte fecale donoren en plaats de muizenkooi in de bioveiligheidskast. Open de kooi en pak voorzichtig een donormuis bij de staart en plaats deze op het metalen rek bovenop de schone kooi. Plaats het deksel van de kooi op het rek en wacht tot het dier poept.Verzamel de fecale pellets en doe ze in een steriele conische buis van 50 milliliter. Trek de pellets op geslacht. Weeg de buis opnieuw en bereken het gewicht van de fecale pellets.Bereid een steriele oplossing door 10% glyceroloplossing toe te voegen aan de normale zoutoplossing. Voeg 10 milliliter van deze oplossing toe aan de conische buis voor elke gram fecale pellets. Homogeniseer dit mengsel drie keer op een lage snelheid gedurende 30 seconden met behulp van een benchtop homogenisator in een zuurkast om de ontlasting te resuspenderen.Filtreer de fecale suspensie door twee lagen steriel katoenen gaas. Bewaar het filtraat tijdelijk in een koelkast voor maximaal zes uur, of verpak het in steriele cryogene flacons en bewaar het in een min 80 graden vriezer. Reinig de homogenisator of blender grondig volgens een standaardprocedure.Ontdooi de bevroren fecale suspensie op ijs als u bevroren monsters gebruikt. Meng de ontdooide fecale suspensie door vortexing. Breng de verse of ontdooide fecale suspensie over in één milliliter spuiten.Weeg de muizen en kies de juiste grootte van de maaghaaal en het maximale doseringsvolume. Test de maagsonde door de lengte te meten vanaf de punt van de neus van de muis tot de onderkant van het borstbeen. Vul de spuit met 10% glycerol in zoutoplossing of fecale suspensie en verwijder luchtbellen in de spuit en de naald.Plaats één muizenkooi in de bioveiligheidskast. Verwijder de plastic kooiafdekking en laat het metalen rek op zijn plaats. Open het metalen rek met één hand.Pak een muis bij de staart en leg deze op het metalen rek. Houd de muis met één hand bij de staart en gebruik de duim en middelvinger van een andere hand om het dier in bedwang te houden door de huid over de schouders te grijpen. Strek het hoofd van het dier voorzichtig naar achteren en houd het hoofd met één hand op zijn plaats.Plaats de maagsonde bovenop de tong in de mond. Ga voorzichtig langs de bovenste pallet totdat de naald de slokdarm bereikt. Laat de naald in één beweging soepel passeren.Zodra de naald goed is geplaatst en gecontroleerd, dient u het materiaal langzaam toe door op de spuit te duwen die aan de naald is bevestigd. Trek na het doseren voorzichtig de naald eruit. Markeer de muis en breng hem terug naar zijn thuiskooi.Controleer het dier gedurende vijf tot 10 minuten door te zoeken naar tekenen van moeizame ademhaling of angst. Controleer de muizen opnieuw tussen 12 en 24 uur na de fecale microbiotatransplantatie. Enzymimmunoassay onthulde dat B-type natriuretisch peptide duidelijk verhoogd was in het plasma van interleukine 10-deficiënte muizen.Een gezonde donor fecale microbiota transplantatie verminderde aanzienlijk de toename van B type natriuretische peptide niveaus. Echocardiografie detecteerde een significante afname van de ejectiefractie van de linker ventrikel bij de interleukine 10 deficiënte muizen in vergelijking met de wildtype muizen. De afname werd significant verminderd door fecale microbiotatransplantatie.Deze bevindingen suggereren dat gezonde donor fecale microbiota transplantatie verminderde colitis geïnduceerde cardiale stoornissen. Bij het proberen van het protocol is het kiezen van de juiste maagsonde essentieel. Flexibele plastic voedingssondes kunnen een betere keuze zijn voor beginners.Het is ook belangrijk om de omgang met dieren te oefenen voordat u een maagsonde krijgt.
