Het harttransplantatiemodel van muizen draagt bij aan het onderzoek naar afstotingsmechanismen na harttransplantatie en draagt bij aan de ontwikkeling van unieke benaderingen om de overleving op lange termijn van cardiale allograftontvangers te verbeteren. Harttransplantatie bij muizen is echter een complexe en uitdagende taak, die een hoog niveau van microchirurgietechnieken vereist, vooral bij vasculaire anastomose11,12,13. He...

Harttransplantatie is de standaardbehandeling geworden voor terminaal hartfalen. Meer dan 5.500 harttransplantaties per jaar worden uitgevoerd in de organisaties die zijn geregistreerd onder de International Society for Heart and Lung Transplantation. Onder de ontvangers van allogene harttransplantaties is het 1-jaars afstotingspercentage nog steeds >10%, terwijl het 3-jaars afstotingspercentage steeg tot 36%1,2. Effectieve profylactische behandelingen voor patiënten met cardiale allograftafstoting ontbreken echter. Daarom zijn diermodelstudies gerechtvaardigd die de fysiologische mechanismen ophelderen die ten grondslag liggen aan de immunologische en pathologische afstoting van cardiale allografts. Dergelijke studies zouden bijdragen aan het onderzoek naar nieuwe doelwitten die nodig zijn om effectieve geneesmiddelen te ontwikkelen, die cardiale allograftafstoting zouden helpen voorkomen en de overlevingskansen in die patiëntenpopulaties zouden verbeteren.
Enkele potentiële immunologische en pathofysiologische mechanismen van cardiale allograftafstoting zijn onlangs voorgesteld in muismodelstudies van heterotope harttransplantatie 3,4,5. Bijgevolg werd heterotope harttransplantatie bij muizen een ideaal preklinisch model om de mechanismen van immuunafstoting en pathologisch letsel te onderzoeken die optreden bij cardiale allografts na harttransplantatie vanwege hun hoge homologie met menselijke genen. Het gangbare concept is om heterotope transplantatie uit te voeren in een muismodel door een abdominale end-to-end anastomose in de ontvangende aorta en inferieure vena cava met behulp van hechtingen, vergelijkbaar met de normale menselijke anatomie. Deze procedure kan echter interfereren met de bloedtoevoer van de ontvanger en veneuze reflux van het onderlichaam6. Daarom wordt hier een aangepaste heterotope harttransplantatieprocedure in een muismodel voorgesteld.
Het hart van de donor wordt geïmplanteerd met de halsslagader en halsslagader van de ontvanger door een end-to-end cervicale anastomose met behulp van een gemodificeerde Cuff-techniek. Deze aangepaste procedure vergemakkelijkte de operatieve haalbaarheid en zorgde voor de overleving van het harttransplantaat zonder de bloedtoevoer en veneuze reflux van het onderlichaam te verstoren.

Cardiale allograftafstoting beperkt de overleving op lange termijn van patiënten na harttransplantatie. Een harttransplantatiemodel van muizen is ideaal voor het onderzoeken van het mechanisme van cardiale allograftafstoting in preklinische studies vanwege hun hoge homologie met menselijke genen. Dit inzicht zou helpen bij het ontwikkelen van unieke benaderingen voor het verbeteren van de overleving van patiënten op lange termijn die worden behandeld met cardiale allografts. In een muismodel wordt abdominale donorhartimplantatie vaak uitgevoerd met een end-to-end anastomose naar de aorta van de ontvanger en inferieure vena cava met behulp van hechtingen. In dit model wordt het hart van de donor geïmplanteerd door end-to-end anastomose in de halsslagader en halsslagader van de ontvanger door de gemodificeerde manchettechniek. In dit cervicale heterotope harttransplantatiemodel van de muis was de overlevingskans van ontvangende muizen ongeveer 95,2% (20 van de 21 muizen overleefden). A Modified Cuff Technique for Mouse Cervical Heterotopic Heart Transplantation Model

In het huidige protocol wordt een muisharttransplantatiemodel gebruikt voor het onderzoeken van het mechanisme van cardiale allograftafstoting. Een harttransplantatiemodel van muizen helpt bij het onderzoeken van cardiale allografts immunologische en de parasagittale afstotingsmechanismen. Dit inzicht zou helpen bij het ontwikkelen van een unieke aanpak om de overleving van de patiënt op lange termijn te verbeteren. Deze cervicale transplantatie operatie wordt uitgevoerd zonder hechtingen met behulp van een manchetmethode.Daarom vermindert het de complexiteit van vaatanastomose en vermijdt het de interferentie met de bloedtoevoer en veneuze reflux van het onderlichaam. De procedure wordt gedemonstreerd door Dr.Xin Mao, een chirurg uit ons laboratorium. Plaats om te beginnen de ontvangende muis op een verwarmingskussen op de operatietafel en scheer het haar over het operatiegebied.Maak na het desinfecteren van het operatiegebied een incisie van 1,5 tot 2 centimeter parallel aan de cervicale middellijn van de rechter onderkaakhoek tot het staarteinde. Ontleed ongeveer een centimeter van de rechter externe halsader met behulp van een elektrocoagulator en een microtang. Sluit de cervicale incisie met continue hechtingen met behulp van een 5-0 polyamide monofilament hechtdraad.
