考慮すべき重要な側面の1つは、腎臓における細胞浸潤である。これらの浸潤物は、サイトカインなどの可溶性因子の分泌を誘発して炎症を悪化させることにより病因を促進する12。浸潤物中にどのような細胞集団が存在するかをよりよく理解するために、有用な方法は白血球を単離することである13。ここでは、B細胞の腎浸潤の検出を例に挙げて用いる。この手順は、デオキシリボヌクレアーゼ(DNase)およびコラゲナーゼによる消化プロセスから始まり、続いて破片、赤血球、および高密度顆粒球を除去する密度勾配分離が続く。B細胞(CD19+)および形質細胞(CD138+)を単離する理由は、狼瘡腎臓がこれらの細胞14を濃縮することができるからである。腎臓の小さな凝集体中のB細胞の存在は、クローン拡張、ひいては免疫グロブリン(Ig)産生を示すことができることが示唆される。形質細胞は、これらの凝集体と同様に15中に存在することがよく知られている。白血球が単離されると、蛍光活性化細胞選別(FACS)を使用して、異なる蛍光結合抗体による染色時に目的の細胞を分析することができます。
免疫蛍光は、厚さ4 μmの腎臓組織サンプル中のタンパク質の蛍光可視化を可能にする免疫組織化学(IHC)検出方法の1つです。他のIHC検出方法は、分析物の性質、結合化学、および他の因子に依存する16。免疫蛍光は、抗原を特定の蛍光色素(または蛍光色素)で標識された対応抗体に曝露する迅速な同定方法である。励起すると、蛍光顕微鏡が検出できる光を生成します。この技術は、補体C3およびIgG2a17の沈着を観察するために使用することができる。過剰な補体カスケード活性化は、制御不能な免疫応答および機能喪失と関連している可能性がある18。腎臓における抗二本鎖DNA(抗dsDNA)自己抗体の免疫沈着は大きな懸念事項であり19、IgG2aアイソタイプを有するものはLN20と関連している。具体的には、抗dsDNA抗体は、核物質に対してより病原性および親和性を示し、免疫複合体21を形成する。IgG2aが存在すると、C3を含む補体カスケードが活性化され、免疫複合体22を消去する。C3マーカーとIgG2aマーカーを個別に定量化することも、重ね合わせて相関関係を確立することもできます。
特に、血清クレアチニン測定は、LN23を診断するために顕微鏡的血尿および腎臓生検と共に使用することができる別の信頼できる技術である。しかし、タンパク尿症の存在は糸球体損傷の強力な指標である。その意味で、狼瘡中のタンパク尿症レベルを監視することは、疾患の発症を検出し、狼瘡を診断するための他の方法を補完することができる。さらに、糸球体に沈着した免疫複合体は、炎症反応を誘導し、補体系を活性化し、より多くの炎症細胞をリクルートすることができる。このプロトコルのもう一つの注目すべき点は、腎臓におけるB細胞浸潤である。これは、浸潤したT細胞とともに、臓器損傷を引き起こす局所免疫応答を増幅する。重要なことに、LNの分類は、顕微鏡で見られる糸球体形態学的変化だけでなく、免疫蛍光で観察される免疫沈着にも基づいている。したがって、このプロトコルでは、腎機能の分析のための正確で費用対効果の高い方法が実験室の設定で提供される。
