敗血症は、感染に対する調節不全の宿主反応によって引き起こされる生命を脅かす臓器機能障害として定義され1、敗血症性ショックは敗血症の重症例の主要な死因です2。敗血症と敗血症性ショックは、毎年世界中で何百万人もの死者を出しています3。敗血症患者の転帰を改善するための鍵は、抗生物質などの治療の迅速な開始です4。敗血症の診断のためのゴールドスタンダードの方法は微生物培養です。しかし、微生物培養は時間がかかり、偽陽性および偽陰性の結果につながる可能性があり、臨床的意義が大幅に制限されます5。従って、敗血症の血液バイオマーカーを同定することが極めて望ましい。プロカルシトニンは理想的な敗血症バイオマーカーとして認識されていますが、敗血症と無菌性疾患を区別できないため、診断効果は限られています6。 
マウス盲腸結紮および穿刺(CLP)は、科学研究における敗血症のモデルを作成するために一般的に使用されます。CLPは、多微生物性腹膜炎を模倣し、炎症誘発性免疫応答と抗炎症性免疫応答の両方を活性化するため、最も広く使用されている敗血症モデルの1つです7。CLPは、細菌性エンドトキシンの注射などの代替技術よりも臨床的に関連性のある敗血症モデルを作成することは広く受け入れられています。したがって、CLPは、研究8で使用するための古典的な敗血症モデルと見なされます。ただし、CLPの主な欠点は、モデルの重症度が盲腸結紮の割合、針のサイズ、穿刺回数、開腹術などのいくつかの要因によって影響を受けるため、再現性です。したがって、CLP誘発敗血症モデルを標準化する必要がある。本研究では、CLP誘発敗血症モデルのプロトコルの詳細を説明し、標準化された手順を示し、その再現性を高めます。
炎症反応は敗血症の初期段階で起こり、好中球は臓器損傷を引き起こす過剰な量の酸化剤とプロテアーゼを放出します8。敗血症の病態生理における重要な要因は、DNA、シトルリン化ヒストン、抗菌プロテイナーゼなどの核および細胞質成分を放出する好中球細胞外トラップ(NET)の形成です9。最近の研究では、NETの過剰な生成が敗血症の病理を媒介することが示唆されています。一方、YW3-56やCl-アミジンなどの化学物質によるペプチジルアルギニンデイミナーゼ(PAD)の酵素的阻害によるNETの減少は、敗血症のマウスモデルで生存促進効果を発揮します10,11。シトルリン化ヒストンH3(CitH3)は2011年に敗血症特異的タンパク質として同定され12、その後の出版物は、循環CitH3濃度が敗血症の信頼できる診断バイオマーカーであることを実証している13,14。CitH3は、プロカルシトニンよりも感度が高く、長持ちするバイオマーカーと考えられており、炎症性サイトカインよりも敗血症の区別においてより特異的です13。
この研究では、敗血症のCLP誘発マウスモデルにおける敗血症の信頼できる診断バイオマーカーを評価しました。

この研究の主な目的は、盲腸結紮と穿刺誘発性敗血症のマウスモデルを使用して、敗血症の信頼できる血液バイオマーカーを評価することです。盲腸結紮および穿刺、または略してCLPは、敗血症の臨床的に関連するモデルです。CLPは敗血症研究で一般的に使用されるモデルです。シトルリン化ヒストンH3、またはCitH3は、敗血症誘発性NETosisに由来します。私たちの以前の研究は、CitH3が敗血症の信頼できる診断バイオマーカーであることを示しています。雄のC57BL/6マウスを選択します。マウスの体重を測定し、1.5%イソフルランを吸入してマウスを麻酔する。マウスを加熱パッドに固定します。脱毛クリームを腹部に塗り、クリームと髪を取り除く前に1分以内放置します。げっ歯類の動物の手術に適した滅菌手術器具を準備します。腹部の皮膚をヨウ素で少なくとも3回消毒します。手術部位を適切な滅菌手術用カーテンで覆います。滅菌手術用ハサミを使用して、リネアアルバに沿って腹壁を約2センチ切開します。滅菌ピンセットで腹腔から盲腸を識別して隔離します。盲腸の75巻を4-0絹縫合糸で結睭化する。腸間膜血管を結紮しないでください。盲腸結紮の割合が異なると、敗血症の重症度が決まります。25%は低悪性度敗血症につながり、50%は中性度敗血症につながり、75%は高悪性度敗血症につながります。尾端と結紮の中間点に21ゲージの針を使用して、盲腸を1回の貫通穿孔で穿刺します。浸透穴を通して盲腸から糞便をそっと絞り出します。なお、糞便も敗血症の重症度を決定するので、腹腔に押し込まれる糞便の量は一貫していなければならない。盲腸と糞便を腹腔内にやさしく交換します。腹筋層と皮膚層を6-0シルク縫合糸で閉じます。切開部をヨウ素で消毒します。ケトプロフェンの注射、キログラムあたり5ミリグラムは、盲腸を避けるために腹部の左下の象限で実行する必要があります。完全に回復するまでマウスを加熱パッドに置きます。マウスは温度管理された部屋に収容され、食物と水に自由にアクセスできます。動物は、敗血症の症状を伴う二酸化炭素の過剰摂取によって安楽死させられます、提示された私たちの研究に基づいて私たちの事前定義されたエンドポイントに関連しています。マウスは、偽群、CLP群、およびCLPプラスCl-アミジンまたはYW3-56処置群に無作為に分けられる。偽群のマウスは、盲腸結紮と穿刺を除くすべてのCLP手順を受けます。術後24時間で末梢血を採取し、遠心分離により血清を調製する。CitH3濃度は、ウェスタンブロッティングおよび自社開発のELISAキットにより測定した。図2に示すように、偽群ではCitH3は検出されなかった。CitH3レベルはCLP挿入後に上昇し、PAD阻害によって有意に減弱した。我々のデータに基づいて、循環CitH3は敗血症の信頼できる診断バイオマーカーであると結論付けました。
