Dextrocardia는 심장의 축이 흉강의 오른쪽에 색인화되는 희귀 한 선천성 심장 기형입니다. 좌상 좌골 좌근은 심장을 포함한 모든 내장 기관이 거울에 비추는 것을 말하며 극히 드문경우입니다 1,2. 심방 세동 (AF)은 수백만 명의 사람들에게 영향을 미치고 특히 뇌졸중 위험 증가와 관련하여 상당한 이환율과 사망률을 유발하는 가장 흔한 부정맥입니다3.
문헌 조사는 병용 Cox-Maze 절차 (CMP)와 좌상 반전이있는 심낭 심근에서의 판막 수술에 대한 결정적인 접근법을 입증 할 수 없었다. 좌상 좌상을 동반 한 덱스 트로 맥의 경우 승모판 막 치환에 대한보고는 거의 없으며 4,5,6,7,8,9 대 좌상에서는 훨씬 적습니다. 그러나 이러한 전략은 삼첨판 막 수술에는 적용되지 않으며 심낭의 복잡한 CMP-IV는 말할 것도 없습니다.
이 논문은 제자리 판막 절제술과 함께 냉동 절제 또는 고주파 절제를 사용하는 CMP-IV 수술의 심방 병변 세트에 관한 세 가지 사례의 수술 기술과 경험을 보고합니다. 모든 수술은 각 환자에 대한 12 개월 추적 관찰과 이전 2 명에 대한 48 개월 추적 관찰에서 부비동 리듬 (SR) 유지에 성공했습니다. 심방 냉동 절제의 transmurality를 조사하기 위해 전자 현미경을 사용하였다.
사례 발표:
환자 및 수술 전 검사 2016년 2월부터 2020년 9월까지 48세 여성 환자, 55세 남성 환자, 39세 남성 환자가 심계항진, 호흡곤란, 수년간 지속되는 운동에 대한 쉬운 피로감에 대한 유사한 불만으로 심장 센터에 연속적으로 입원했습니다(표 1). 그들은 모두 좌상 대 심근 또는 기타 심장 건강 동반 질환의 친숙한 역사를 부인했습니다. 모든 환자는 수술 전 검사를 위해 정기적으로 심전도(ECG), 흉부 방사선 촬영(그림 1), 컴퓨터 단층 촬영(CT) 및 도플러 경흉부 심초음파(TTE)를 의뢰했습니다.
사례 1 : 48 세의 한 여성이 수년간 지속되는 운동에 대한 심계항진, 호흡 곤란 및 쉬운 피로감에 대한 불만을 나타 냈습니다. 기억 상실은 눈에 띄지 않았습니다. 신체 검사 중에 오른쪽 쇄골 정중선 외부의 다섯 번째 늑간 공간에서 3 등급 수축기 불어 잡음이 들렸습니다. X선 일반 필름과 CT 스캔은 덱스트로맥의 윤곽이 확대되고 계상-대결이 있음을 보여주었습니다. 경식도 심 초음파는 중등도에서 중증의 승모판 막 탈출증 및 삼첨판 환형 확장 후 중등도에서 중증의 승모판 막 폐쇄 부전과 경미한 삼첨판 막 폐쇄 부전을 나타 냈습니다. 혈전증은 없었고 좌심방의 직경은 5.3cm였으며 심실 속도가 빠른 AF가 ECG에 의해 감지되었습니다.
사례 2 : 55 세의 남성이 수년간 비슷한 증상을 보였습니다. 심장 건강 동반 질환의 병력은 없었습니다. 그러나 그는 약 6 개월 전에 뇌졸중을 앓 았습니다. 신체 검사에서 왼쪽 두 번째 늑간 공간에서 이완기 부는 잡음이 목에 발산되는 소리가 들렸습니다. 일반 X- 레이 및 CT 스캔은 윤곽과 좌위 전체가 확대 된 덱스트로 맥을 나타 냈습니다. 경식도 심 초음파는 경증 대동맥 탈출증 및 삼첨판 환형 확장 후 중등도 대동맥 판막 역류와 경증 내지 중등도의 삼첨판 막 역류를 보였다. 좌심방의 직경은 심방 혈전증없이 4.5cm였습니다. 빠른 심실 속도를 갖는 AF가 ECG에 의해 검출되었다.
사례 3: 진행성 운동성 호흡곤란 및 간헐적 심계항진이 나타난 39세 남성. 신체 검사 중에 오른쪽 쇄골 정중선 외부의 다섯 번째 늑간 공간에서 3 등급 수축기 불어 잡음이 들렸습니다. X 선 일반 필름과 CT 스캔은 덱스 트로 비아의 윤곽이 확대되고 좌상 대 전체가 있음을 보여주었습니다. 경식도 심 초음파는 환형 확대 후 심한 승모판 막 폐쇄 부전과 삼첨판 역류를 보였다. 심방 혈전증은 없었으며 좌심방 직경은 5.8cm였습니다. 24 시간 외래 ECG는 총 하중이 165 분 인 발작 AF를 시연했습니다.
진단, 평가 및 계획 사례 1 : 기계식 승모판 치환술과 삼첨판 고리 성형술을 통한 심방 냉동 냉동 콕스 - 미로 IV 절차 (cryo-CMP-IV)가 동시에 수행되었습니다.
사례 2 : 병용 기계 대동맥 판막 (AV) 교체 및 삼첨판 고리 성형술을 병행하는 심방 냉동 -CMP-IV 절차가 수행되었습니다.
사례 3 : 투심 CMP-IV 절차, 승모판 막 성형술 및 삼첨판 고리 성형술을 동시에 수행했습니다.

cox-maze iv procedure concomitant with valvular surgery in situs inversus dextrocardia: a single-center experience in china
