L5 radiculaire symptomen worden voornamelijk veroorzaakt door L4-L5 hernia of stenose. Deze symptomen kunnen ook optreden als gevolg van L5 lumbale foraminale stenose of L5-S1 laterale lumbale hernia (LLDH)9. Van alle symptomatische lumbale hernia's is L5-S1 FLDH goed voor ongeveer 3%10. Voor L5-S1 foraminale laesies wordt een posterolaterale of transforaminale benadering aanbevolen. Voor deze aanpak zijn er drie hoofdtechnieken, zoals de microscopische, buis met endoscopis...

Diagnose en operaties voor lumbale foraminale stenose (LFS) en lumbale laterale hernia (LLDH) op L5-S1-niveau zijn een uitdaging voor wervelkolomchirurgen vanwege de unieke structuur van dit niveau1. De iliacale top, het brede L5 transversale proces (TP), de kleine ruimte tussen de sacrale ala en de L5 TP en osteofyten maken het operatievenster erg smal2. Als de benige resectie niet voldoende is, kan onvoldoende decompressie naar de L5-zenuwwortel leiden tot restsymptomen. Massale benige verwijdering veroorzaakt postoperatieve instabiliteit. Deze problemen beperken de competenties van chirurgen met foraminale / extraforaminale L5-worteldecompressie. Verschillende rapporten hebben goede resultaten laten zien met minimaal invasieve wervelkolomoperaties, zoals microscopische of endoscopische procedures in dit gebied om de L5-zenuwwortel 3,4 te decomprimeren. Onlangs is het gebruik van navigatie voor foraminale decompressie van de L5-wortel gemeld met goede chirurgische uitkomsten5.
Volledig endoscopische discectomie wordt steeds populairder voor het verwijderen van laterale lumbale hernia6. Bovendien kunnen micro-endoscopische procedures in combinatie met navigatie de chirurg helpen om de L5-wortel met precisie te decomprimeren2. Meestal vereisen deze technieken intraoperatief gebruik van de C-arm. Het doel van deze methode is om de L5-wortel precies te decomprimeren met minimale benige resectie zonder een C-arm.
De indicaties voor deze techniek zijn extraforaminale lumbale hernia en herniatie / stenose van de laterale helft van de foraminale lumbale schijf. De contra-indicaties zijn herniatie/stenose van de mediale een derde van de foraminale lumbale schijf omdat de scope het beoogde gebied niet kan bereiken2.

transtubular endoscopic posterolateral decompression for l5-s1 lumbar lateral disc herniation

We rapporteren een nieuwe techniek voor C-arm vrije transtubulaire L5 zenuwdecompressie onder CT-gebaseerde navigatie om het stralingsgevaar te verminderen. Deze procedure wordt uitgevoerd onder algemene anesthesie en neuromonitoring. De patiënt wordt in buikligging op een operatieve koolstoftafel geplaatst. Een navigatiereferentiekader wordt percutaan in het contralaterale sacro-iliacale gewricht of spineuze proces geplaatst. Vervolgens worden CT-scanbeelden verkregen. Na instrumentregistratie wordt het L5-S1 foraminale niveau bevestigd met een genavigeerde sonde en wordt het toegangspunt gemarkeerd. Met behulp van een huidincisie van ongeveer 2 cm worden het onderhuidse weefsel en de spieren ontleed. De genavigeerde eerste dilatator is gericht op de driehoek van de L5-S1 Kambin en er wordt sequentiële verwijding uitgevoerd. De buis van 18 mm wordt gebruikt en aan het frame bevestigd. Het bot rond de Driehoek van de Kambin wordt verwijderd met een genavigeerde braam. Voor laterale hernia wordt de L5-zenuwwortel geïdentificeerd en ingetrokken en wordt het schijffragment verwijderd. De navigatiegeleide tubulaire endoscopische decompressie is een effectieve procedure. Er is geen stralingsgevaar voor de chirurg of het personeel van de operatiekamer.

Acht gevallen (vier mannen, vier vrouwen) ondergingen een operatie met behulp van deze nieuwe techniek. De gemiddelde leeftijd was 72,0 jaar en de gemiddelde follow-upperiode was 1,5 jaar. Er waren vijf patiënten met L5/S1 foraminale stenose, twee patiënten met L5/S foraminale discushernia en één patiënt met L3/4 foraminale hernia. We konden alle operaties uitvoeren zonder C-arm. De gemiddelde chirurgische tijd en het bloedverlies waren respectievelijk 143 min ± 14 min en 134 ± 18 ml.

Diagnose en chirurgie voor lumbale foraminale stenose en lumbale laterale hernia op L5-S1-niveau is een uitdaging voor spinale chirurg vanwege de unieke structuur. Deze studie werd goedgekeurd door de ethische commissie van het Okayama Rosai-ziekenhuis.Dit is een protocol van transtubulaire endoscopische posterolaterale decompressie voor L5-S1 lumbale laterale hernia. Evaluatie van radiogrammen en magnetische resonantie beeldvorming. Intraoperatieve CT en spinale navigatie.Dit computergestuurde flexibele full carbon frame is erg handig. Verzend de CT 3D gereconstrueerde beelden automatisch met een kabel naar het navigatiesysteem. Minimale facetresectie voorkomt extra postoperatieve spinale instabiliteit, vooral het gebruik van genavigeerde braam, helpt real-time dynamische feedback tijdens resectie van benige uitlopers.
