腹部大動脈瘤(AAA)は、血管径1の少なくとも50%の増加を伴う腹部大動脈の分節拡張として定義される。AAAは、破裂が介入2,3,4であっても非常に高い死亡率をもたらす可能性があるため、潜在的に致命的である。AAAは米国で年間約13,000人の死亡の原因となっていると報告されており、これは1,5人の死因の第10位となっています。
AAAの病因はまだ完全には理解されていない6,7,8。AAAの分子機構を調査し、潜在的な治療標的を試験するために、いくつかの実験的AAAモデルが確立されている9、10。AAAのげっ歯類モデルには、エラスターゼ、塩化カルシウム、アンジオテンシンIIおよび異種移植片モデルが含まれ、その中でエラスターゼ誘導AAAモデルは、2番目に多く使用されるモデル10、11、12、13、14、15、16、17である。このモデルは、大動脈の炎症領域へのブタ膵臓エラスターゼ(PPE)の直接注入または適用を含む。大動脈の内側層へのエラスターゼの浸透は、弾性層状の分解および炎症性細胞の浸潤を引き起こし、大動脈壁の完全性の喪失およびその後の腹部大動脈の拡張をもたらす7,18。エラスターゼ誘導AAAモデルは、1990年にAnidjarらによってラットを用いて最初に報告され、大動脈の単離された部分をエラスターゼ17で灌流した。2012年後半に、PPEの周到性適用を用いた改変モデルがBhamidipatiらによって報告された19。今日、エラスターゼ誘導AAAモデルのほとんどの手術は、バミディパティのグループに触発され、PPEの管腔内灌流ではなく外部適用で行われます。外部適用は、細かい外科的技能に対する要求が少ないが、AAAの発生率は比較的低く、管腔内灌流のそれよりも幾分小さいサイズ11,19。
AAA研究で広く使用されているが、エラスターゼ誘導AAAモデルには一定の限界がある。このモデルの1つの注意点は、手術の実施方法の必然的な変動であり、再現性の問題につながる可能性があります。例えば、大動脈の破腎セグメントがどのように単離されるか、およびセグメントのどの部分が異なる実験室間でPPE適用のために選択されるかに関して、外科的処置に相違が存在する可能性がある。PPEの酵素活性およびPPEインキュベーションの時間持続時間も変化し得る。しかし、これらはすべて、最終的なAAA形成速度および動脈瘤直径に影響を及ぼす重要な決定因子である。これらの重要な決定要因の変動は、このモデルを使用する異なるグループからのAAA研究のデータ比較を非常に困難にする。したがって、標準化された外科的処置は、様々な機関から同等の結果を得るためのツールとして必要とされている。
