엑소좀은 중요한 특징을 가진 생체 활성 소포체로, 크기는 40nm에서 200nm까지 다양합니다. 엑소좀은 세포막에서 유래하며 엔도솜의 방출로 인해 형성된다1. 이러한 구조는 세포 간 통신기 역할을 하며 이웃 세포와 상호 작용하여 활성 분자의 전달을 촉진합니다. 엑소좀은 다양한 출처에서 분리할 수 있습니다. 여기에는 혈장, 소변, 뇌척수액, 타액과 같은 체액과 체외 조건에서 배양된 세포주가 포함됩니다. 엑소좀은 지질, 단백질, 핵산과 같은 생체 거대분자를 함유하고 있기 때문에 신경 손상을 제거하는 데 중요한 역할을 합니다2. 신경계의 지지세포인 글리아(Glia)3는 엑소좀(exosome)4을 통해 단백질과 마이크로 RNA를 뉴런의 축삭돌기로 전달한다.
신경 전도의 특징인 미엘린 수초를 형성하는 지질도 엑소좀 4,5를 통해 희소돌기아교세포에서 방출됩니다. 엑소좀은 또한 시냅스 가소성, 신경 스트레스 반응, 세포 간 통신 및 뇌의 신경 발생과 같은 과정에도 관여합니다 6,7. 엑소좀은 나노 차원을 가지고 있기 때문에 BBB를 통과할 수 있습니다. 간질액에서 뇌척수액으로 이 막을 통과한 후 뇌척수액으로 전이되는 특별한 경로가 있다8. 엑소좀은 표면 특성으로 인해 약물 전달 시스템으로서 표적 세포와 효율적으로 상호 작용하고 로드된 약물을 능동적으로 전달할 수 있습니다.
엑소좀 표면에 다양한 접착 단백질(테트라스파닌 및 인테그린)이 발현되기 때문에 이러한 세포외 소포체는 숙주 세포막과 쉽게 상호 작용하고 융합할 수 있습니다9. 엑소좀은 BBB를 침투할 수 있는 능력과 표면 특성으로 인해 특히 중추신경계 질환 치료에 약물 전달 시스템으로 사용될 수 있다고 생각됩니다. 중간엽줄기세포(MSC) 유래 엑소좀은 동종 세포 치료제에 비해 면역 거부 반응 위험이 낮으며, 이러한 점에서 무세포 치료 응용 분야의 중요한 구성 요소가 될 수 있습니다10.
도파민은 파킨슨병(PD)의 특징인 뇌 결핍 분자로, 나날이 악화되고 있다11,12,13. PD는 중뇌의 흑질(substantia nigra)에서 도파민 뉴런의 퇴화 및 운동 뉴런 기능의 손실과 관련이 있는 것으로 알려져있다 14,15. 도파민성 뉴런의 사멸은 신경전달물질인 도파민이 뇌선조체에 공급되는 것을 막는다. 그 결과 PD 특이적 증상이 나타난다16. 파킨슨병의 증상은 서운동증, 자세 불안정, 경직, 특히 안정시 떨림12,13이다. 파킨슨병은 2세기 전에 처음 기술되었지만, 이 질병의 병리와 원인을 이해하기 위한 연구는 여전히 진행 중이며, 현재 파킨슨병이 복잡한 전신 질환이라는 것이 받아들여지고 있다17. 도파민 결핍이 발생할 것으로 예측되며, 뉴런의 80% 이상이 퇴화될 때 임상적 PD 증상이 관찰된다18. 질병 치료에서 불완전한 도파민 보충은 운동 증상을 줄이기 위해 선호됩니다. 생체 내 연구에 따르면 뇌에 도파민을 직접 주입하면 동물의 증상이 크게 감소하는 것으로 나타났습니다19. L-DOPA(L-3,4-디히드록시페닐알라닌)와 같은 도파민 전구체와 도파민 수용체 약물은 인간에서 뇌에 도파민을 직접 주입하는 것이 불가능하고 시스템에 들어가는 도파민이 BBB20을 통과할 수 없기 때문에 임상에서 사용됩니다. 이러한 유형의 약물은 시간이 지남에 따라 효과를 잃습니다. 그러나 파킨슨병에 대한 치료법은 아직 없습니다. 따라서 질병의 병태생리를 밝히고 파킨슨병이 환자에게 미치는 영향을 줄이기 위한 새로운 치료 전략과 치료 방식을 개발할 필요성이 매우 높습니다.
엑소좀 기반 연구는 최근 신경계 질환의 치료적 접근과 병리학에 대한 정보를 수집해 주목받고 있다. MSC 유래 엑소좀은 신경 손상의 염증을 줄이고 신경 재생에 기여하는 것으로 나타났습니다21,22,23. 또한, MSC 유래 엑소좀 분비체는 특히 도파민성 뉴런에 대한 신경 영양 및 신경 보호 효과를 보여줌으로써 세포 사멸을 감소시키는 것으로 보고되었습니다24,25. 최근 엑소좀을 치료제 전달 시스템으로 사용하는 플랫폼에 대한 연구가 집중적으로 가속화되고 있습니다. 수많은 연구에서 관련 약물을 엑소좀에 쉽게 캡슐화하고 표적 세포, 조직 및 장기에 안전하게 전달할 수 있는 것으로 관찰되었습니다26,27. 배양, 동결/해동 주기, 초음파 처리 및 압출과 같은 다양한 방법을 엑소좀에 약물을 로딩하는 데 사용할 수 있습니다28.
엑소좀 또는 엑소좀 유사 소포와의 배양은 친유성 소분자가 이러한 전달 시스템28,29,30에 수동적으로 캡슐화될 수 있도록 합니다. 특히 커큐민 31, 카탈라아제 30, 독소루비신32, 파클리탁셀33 등 다양한 분자가 엑소좀에 효과적으로 로딩됐다. 항산화 활성을 가진 카탈라아제 함유 엑소좀은 뇌의 뉴런과 미세아교세포에 효율적으로 축적되어 강력한 신경보호 활성을 나타내는 것이 관찰되었습니다30. 동일한 연구에서, 로딩 효율을 높이기 위해 복합체에 첨가 된 사포닌은 배양 중 약물 로딩 비율을 증가시키는 것으로 밝혀졌습니다30,34. 그러나 엑소좀에 약물을 로딩하는 기준을 확립하기 위해서는 추가 연구가 필요합니다.
본 논문은 WJ-MSC에서 분리한 엑소좀에 도파민을 캡슐화하여 나노캐리어 시스템을 개발하는 방법을 설명합니다. WJ-MSC의 배양, 엑소좀의 분리 및 특성 분석, 약물 로딩 실험, 다양한 기법을 이용한 도파민 로딩 엑소좀의 특성 분석, in vitro 세포독성 분석 등 모든 단계를 상세히 설명합니다.

크기가 40nm에서 200nm 사이인 엑소좀은 세포외 소포체의 가장 작은 하위 그룹을 구성합니다. 세포에서 분비되는 이러한 생체 활성 소포는 세포 간 화물 및 통신에 적극적인 역할을 합니다. 엑소좀은 주로 혈장, 뇌척수액, 소변, 타액, 양수, 초유, 모유, 관절액, 정액, 흉산 등의 체액에서 발견됩니다. 엑소좀의 크기를 고려할 때 혈액뇌장벽(BBB)을 통과할 수 있기 때문에 중추신경계 질환에 중요한 역할을 할 수 있을 것으로 생각됩니다. 따라서 본 연구는 와튼의 젤리 중간엽 줄기세포(WJ-MSC)에서 분리한 엑소좀에 도파민을 캡슐화하여 엑소좀 기반 나노캐리어 시스템을 개발하는 것을 목표로 했다. 특성화 과정을 통과한 엑소좀은 도파민으로 배양되었습니다. 도파민이 함유된 엑소좀은 배양이 끝날 때 재특성화되었습니다. 도파민이 함유된 엑소좀은 약물 방출 및 세포 독성 분석에서 조사되었습니다. 그 결과, 도파민이 엑소좀 내에 성공적으로 캡슐화될 수 있으며, 도파민이 함유된 엑소좀이 섬유아세포 생존력에 영향을 미치지 않는 것으로 나타났습니다.

NTA와 DLS 분석에서 밝혀진 나노 입자의 수는 서로 일치했습니다.와튼 젤리 유래 엑소좀의 제타 전위는 마이너스 15.7, 도파민이 함유된 엑소좀은 마이너스 17.6밀리볼트로 측정되었습니다. 도파민이 함유된 엑소좀의 HPLC 분석 결과 도파민의 피크가 6.45분에 검출되었습니다. 누적 약물 방출 프로파일에 따르면 캡슐화된 도파민의 74.8%가 첫 8시간 이내에 엑소좀에서 방출되었습니다.섬유아세포에 있는 도파민이 적재되고 유리된 엑소좀의 세포독성을 조사했습니다. 도파민이 함유된 엑소좀은 세포독성 효과를 나타내지 않았지만, 사포닌은 섬유아세포의 생존력을 감소시켰습니다.
