De tanden die in deze studie werden gebruikt, maakten deel uit van een project dat werd goedgekeurd door de lokale ethische commissie (EKNZ UBE-15/111). De conventionele verlichting (bijvoorbeeld door een tandheelkundige eenheidslamp) is een onbevredigend diagnostisch hulpmiddel voor de identificatie van composietharsrestauraties. Voor superieure diagnostiek met conventionele verlichting is een vergrotingshulpmiddel, drogen of zelfs inspanningsreiniging van de tanden noodzakelijk. Zelfs onder ideale omstandigheden lijkt conventionele verlichting onvoldoende te zijn. Een studie toonde aan dat conventionele verlichting kan leiden tot een verkeerde afkeuring van composietha...

De toepassing van FIT vergemakkelijkt het onderscheid van composietharsmaterialen van een gezonde tandsubstantie in vergelijking met conventionele verlichting, bijvoorbeeld door een tandheelkundige eenheidslamp 1,2. Fluorescentie treedt op wanneer een materiaal het licht uitzendt op een hogere golflengte dan het is geabsorbeerd. Als gevolg van deze verlichting lijkt het materiaal helderder dan de tand3. De maximale fluorescentie van composietharsmaterialen treedt op wanneer deze wordt verlicht door een golflengte van 398 ± 5 nanometer3. Fluorescentie in composietharsmaterialen verschijnt als gevolg van zeldzame aardoxiden toegevoegd aan de glasvullers, enkele van de belangrijkste componenten van composietharsen 4,5. De toevoeging van deze fluorescerende stoffen is bedoeld om de optische eigenschappen van composietharsen aan te passen aan de tandstructuur om de esthetische eigenschappen van composietharsen te verbeteren 4,5. FIT is toepasbaar op veel composietharsmaterialen omdat ze deze fluorescentie-eigenschappen vertonen3. Fluorescentie neemt echter af met de veroudering van de composietharsmaterialen 6,7,8,9.
Het onderscheiden van composietharsmaterialen van tandstructuur met conventionele verlichting is een uitdaging, omdat moderne composietharsmaterialen bijna perfect overeenkomen met de optische eigenschappen van tandsubstantie10,11. De verkeerde diagnose van composiethars resulteert in onnauwkeurige tandheelkundige grafieken, valse cariësrisicobeoordeling en onjuiste behandelingsplanning11. Bovendien worden epidemiologische gegevens vervalst12.
Composiethars is het materiaal bij uitstek voor directe restauraties vanwege de eenvoudige hantering, esthetische eigenschappen en klinische prestaties13. Niettemin moeten veel composietrestauraties worden vernieuwd vanwege secundaire cariës, breuken of andere redenen14,15. Het verwijderen van resterende composietharsmaterialen kan echter veeleisend zijn onder conventionele lichtomstandigheden. Zelfs met de toepassing van een vergrotingshulpmiddel en het gebruik van tactiele sondes of uitgebreid drogen van de tanden, zijn composietresten soms moeilijk te onderscheiden van een gezonde tandstructuur. Restanten van composietresten tijdens het verwijderen van de lijmrestauratie verlagen de kwaliteit van verdere restauraties en hebben een esthetische aantasting door mogelijke verkleuring van de marges 1,16,17,18,19,20,21,22 . Integendeel, een overpreparatie als gevolg van een verkeerde diagnose van composiethars versus tandstructuur kan leiden tot onnodig stofverlies 1,2.
In de tandheelkundige traumatologie is fixatie van de gewonde tanden met behulp van traumaspalken frequent en in veel gevallen verplicht23. De traumaspalken worden meestal op de tanden bevestigd met behulp van een vloeiend composietharsmateriaal. Onvolledige verwijdering van het composietharsmateriaal in dit scenario kan leiden tot de hierboven beschreven stoornissen. Aangezien tandheelkundig trauma vooral in voortanden optreedt, zijn een aantasting van de esthetiek en voldoende hechting van verdere reconstructies cruciaal. Daarom is het doel van het artikel om de toepassing van de FIT-methode aan te tonen als een efficiënte en eenvoudige aanpak voor het detecteren en verwijderen van composietharsmaterialen.

Het opsporen en verwijderen van tandkleurige vulmaterialen is voor iedere tandarts een grote uitdaging. De Fluorescence-aided Identification Technique (FIT) is een niet-invasief hulpmiddel om het onderscheid tussen composietharsmateriaal en gezonde tandsubstantie te vergemakkelijken. In vergelijking met conventionele verlichting is FIT een zeer nauwkeurige, betrouwbare en snelle diagnostische methode. Wanneer composiethars wordt verlicht met een golflengte van ongeveer 398 ± 5 nm, zorgen bepaalde fluorescerende componenten ervoor dat de composiethars helderder lijkt dan de tandstructuur. Elke fluorescentie-inducerende lichtbron met de juiste golflengte kan voor deze methode worden gebruikt. Optimaal wordt deze techniek gebruikt zonder extra natuurlijk of kunstmatig licht. De toepassing van FIT kan worden gebruikt voor diagnostische doeleinden, bijvoorbeeld tandheelkundige kaarten, en bovendien voor de volledige en minimaal invasieve verwijdering van composietharsrestauraties, bracket debonding en trauma-spalkverwijdering. De beoordeling van volumetrische veranderingen na samengestelde verwijdering kan worden geleverd door overlappende pre- en postoperatieve scans en daaropvolgende berekening met behulp van geschikte software.

Het gebruik van de FIT-methode zorgt ervoor dat de meeste composietharsmaterialen helderder lijken dan een gezonde tandstructuur (figuur 2 en figuur 5). Daarom is FIT niet alleen toepasbaar bij de detectie van composietharsmateriaal, maar vergemakkelijkt het ook de verwijdering van composietharsmaterialen in het algemeen, en expliciet in achterste tanden, tijdens orthodontische beugeldebonding en bij traumaspalkverwijdering 1,2...

Het opsporen en verwijderen van tandkleurig vulmateriaal is een uitdaging. De fluorescentie-ondersteunde identificatietechniek, FIT, is een diagnostisch hulpmiddel dat de differentiatie van composiethars van tandsubstantie vergemakkelijkt. FIT maakt de minimaal invasieve verwijdering van composietharsrestauraties en composietgebonden traumaspalken mogelijk.Het is een eenvoudige, snelle en betrouwbare diagnostische aanpak. Draag om te beginnen een heldere of geel getinte veiligheidsbril met UV-bescherming en maak de kamer donkerder. Gebruik een fluorescentie-inducerende lichtbron om de tandsubstantie en tandkleurige composietharsrestauratie te verlichten.Voor een preoperatieve scan start u het intraorale scanapparaat en opent u de juiste software voor experimentele evaluatiedoeleinden. Druk op Nieuwe patiënt toevoegen om de patiënt te registreren. Vul de hiaten in, zoals achternaam, voornaam, geboortedatum en patiënt-ID, en druk vervolgens op Case toevoegen.Kies Kaakscan en vertoningen onder de sectie Indicaties en druk op Volgende. Maak de kamer donkerder en droog het werkveld om de scanprocedure te vergemakkelijken. Start de intraorale scanner en voer een digitale oppervlaktescan uit van het werkveld.Om het composietharsmateriaal te visualiseren, verlicht u de tandsubstantie en tandkleurige composietharsrestauratie. Verwijder het composietharsmateriaal met behulp van een high-speed contrahoek handstuk met diamant burs, een lijmhars remover en polijstapparaten. Voer vervolgens de postoperatieve scan uit voor experimentele volumetrische beoordeling volgens de stappen die eerder zijn aangetoond voor de preoperatieve scan.Voor experimentele volumetrische beoordeling selecteert u de patiënt en het geval, opent u vervolgens de casus en drukt u op Model. Druk vervolgens op Exporteren om de pre- en postoperatieve scans te exporteren als oppervlakte-vlakvullingstaal in de hoogste resolutie. Open vervolgens geschikte software en druk op Recombine.Upload de pre- en postoperatieve scans naar de software door op Importeren te drukken. Druk vervolgens op Analyse om de volumetrische veranderingen van pre- tot postoperatieve scans te visualiseren. Selecteer de tandplaatsen waar volumetrische veranderingen vermoedelijk hebben plaatsgevonden door Regio te kiezen onder de sectie Hulpmiddelen.Analyseer de volumetrische veranderingen met behulp van de lineaire en volumetrische meetsoftwaretools Afstanden en Volumeanalyse. Druk afstanden onder de sectie Hulpmiddelen voor lineaire kwantificering van tandsubstantieverlies en composietharsresten in kleur. Gebruik de kleurenbalk aan de linkerkant om lineaire volumetrische veranderingen te kwantificeren.Zoek bovendien de cursor op de relevante tandlocaties en observeer de exacte cursorafstand in het vak aan de linkerkant. Het representatieve tandmodel met verschillende composietharsrestauraties en traumaspalk gefixeerd met composiethars wordt hier getoond. Het gebruik van de fluorescentie-ondersteunde identificatietechniek maakte de meeste composietharsmaterialen helderder dan gezonde tandstructuren.Een tandmodel na traumaspalkverwijdering onder conventionele verlichting en met behulp van de fluorescentie-ondersteunde identificatietechniek wordt hier getoond. Een volumetrische beoordeling toonde kwantificering van de samengestelde resten en het verlies van tandstoffen. De discrepantie van de pre- en postoperatieve scan onthulde samengestelde resten en stofverlies in tanden 13, 12 en 11.Tanden 21, 22 en 23 vertoonden daarentegen geen oppervlakteveranderingen na het verwijderen van traumaspalk. Bovendien werden de samengestelde restanten onopgemerkt onder conventionele lichtverlichting en werden ze zichtbaar met behulp van de fluorescentie-ondersteunde identificatietechniek. Elke fluorescentie-inducerende lichtbron is van toepassing op de FIT-methode.Maak de kamer donkerder om interferentie met andere lichtbronnen te voorkomen. FIT is niet alleen toepasbaar voor de detectie en verwijdering van composietharsrestauraties en traumaspalken. Het kan ook worden toegepast in bracket debonding en tandheelkundig forensisch onderzoek.
