Die konventionelle Beleuchtung (z.B. durch eine Dentaleinheitslampe) ist ein unbefriedigendes diagnostisches Werkzeug zur Identifizierung von Kompositharzrestaurationen. Für eine bessere Diagnostik mit herkömmlicher Beleuchtung ist eine Vergrößerungshilfe, Trocknung oder auch mühsame Reinigung der Zähne notwendig. Selbst unter idealen Umständen scheint die konventionelle Beleuchtung nicht ausreichend zu sein. Eine Studie zeigte, dass konventionelle Beleuchtung zu einer Fehlerkennung von Kompositharzrestaurationen ...

Die Anwendung von FIT erleichtert die Unterscheidung von Kompositharzmaterialien von gesunder Zahnsubstanz im Vergleich zu herkömmlicher Beleuchtung, beispielsweise durch eine Dentaleinheitslampe 1,2. Fluoreszenz tritt auf, wenn ein Material das Licht mit einer höheren Wellenlänge emittiert, als es absorbiert wurde. Durch diese Beleuchtung erscheint das Material heller als der Zahn3. Die maximale Fluoreszenz von Verbundharzmaterialien tritt auf, wenn sie von einer Wellenlänge von 398 ± 5 Nanometernbeleuchtet werden3. Fluoreszenz in Verbundharzmaterialien tritt aufgrund von Seltenerdoxiden auf, die den Glasfüllstoffen zugesetzt werden, einige der Hauptkomponenten von Verbundharzen 4,5. Die Zugabe dieser fluoreszierenden Substanzen beabsichtigt, die optischen Eigenschaften von Kompositharzen an die Zahnstruktur anzupassen, um die ästhetischen Eigenschaften von Kompositharzen 4,5 zu verbessern. FIT ist auf vielen Verbundharzmaterialien anwendbar, da sie diese Fluoreszenzeigenschaften aufweisen3. Die Fluoreszenz nimmt jedoch mit der Alterung der Verbundharzmaterialien 6,7,8,9 ab.
Die Unterscheidung von Verbundharzwerkstoffen von Zahnstrukturen mit herkömmlicher Beleuchtung ist eine Herausforderung, da moderne Verbundharzwerkstoffe den optischen Eigenschaften der Zahnsubstanz nahezu perfekt entsprechen10,11. Die Fehldiagnose von Kompositharz führt zu ungenauen Zahndiagrammen, falscher Karies-Risikobewertung und unangemessener Behandlungsplanung11. Darüber hinaus werden epidemiologische Daten gefälscht12.
Kompositharz ist aufgrund seiner einfachen Handhabung, seiner ästhetischen Eigenschaften und seiner klinischen Leistung das Material der Wahl für direkte Restaurationen13. Dennoch müssen viele Kompositrestaurationen aufgrund von sekundärer Karies, Frakturen oder anderen Gründen erneuert werden14,15. Die Entfernung von Harzresten kann jedoch unter herkömmlichen Lichtbedingungen anspruchsvoll sein. Selbst mit dem Einsatz einer Vergrößerungshilfe und dem Einsatz von taktilen Sonden oder ausgiebigem Trocknen der Zähne sind Kompositreste manchmal schwer von solider Zahnstruktur zu unterscheiden. Reste von Kompositresten bei der Entfernung der Klebstoffrestauration verringern die Qualität weiterer Restaurationen und haben eine ästhetische Beeinträchtigung durch mögliche Verfärbung der Ränder 1,16,17,18,19,20,21,22 . Im Gegenteil, eine Überpräparation aufgrund einer Fehldiagnose von Kompositharz versus Zahnstruktur kann zu unnötigem Substanzverlust führen 1,2.
In der dentalen Traumatologie ist die Fixierung der verletzten Zähne mittels Traumaschienen häufig und in vielen Fällen zwingend erforderlich23. Die Traumaschienen werden in der Regel mit einem fließfähigen Verbundharzmaterial auf den Zähnen fixiert. Eine unvollständige Entfernung des Verbundharzmaterials kann in diesem Szenario zu den oben beschriebenen Wertminderungen führen. Da Zahntraumata meist in Frontzähnen auftreten, sind eine Beeinträchtigung der Ästhetik und eine ausreichende Haftung weiterer Rekonstruktionen entscheidend. Ziel des Artikels ist es daher, die Anwendung der FIT-Methode als effizienten und unkomplizierten Ansatz zur Detektion und Entfernung von Verbundharzmaterialien zu demonstrieren.

Die Erkennung und Entfernung von zahnfarbenen Füllungsmaterialien ist eine große Herausforderung für jeden Zahnarzt. Die Fluorescence-aided Identification Technique (FIT) ist ein nichtinvasives Werkzeug, um die Unterscheidung von Kompositharzmaterial von gesunder Zahnsubstanz zu erleichtern. Im Vergleich zur herkömmlichen Beleuchtung ist FIT eine sehr genaue, zuverlässige und schnelle Diagnosemethode. Wenn Kompositharz mit einer Wellenlänge von etwa 398 ± 5 nm beleuchtet wird, lassen bestimmte fluoreszierende Komponenten das Kompositharz heller erscheinen als die Zahnstruktur. Für dieses Verfahren kann jede fluoreszenzinduzierende Lichtquelle mit der entsprechenden Wellenlänge verwendet werden. Optimalerweise wird diese Technik ohne zusätzliche natürliche oder künstliche Beleuchtung eingesetzt. Die Anwendung von FIT kann für diagnostische Zwecke, z. B. Zahnkarten, und zusätzlich für die vollständige und minimalinvasive Entfernung von Kompositharzrestaurationen, Bracket-Debonding und Traumaschienenentfernung verwendet werden. Die Beurteilung volumetrischer Veränderungen nach der Kompositentfernung kann durch überlappende prä- und postoperative Scans und anschließende Berechnung mit geeigneter Software erfolgen.

Die Verwendung der FIT-Methode lässt die meisten Verbundharzmaterialien heller erscheinen als gesunde Zahnstrukturen (Abbildung 2 und Abbildung 5). Daher ist FIT nicht nur bei der Detektion von Kompositharzmaterial anwendbar, sondern erleichtert auch die Entfernung von Verbundharzmaterialien im Allgemeinen und explizit in Seitenzähnen, während der kieferorthopädischen Bracket-Debonding und bei der Entfernung von Traumaschienen</...

Die Erkennung und Entfernung von zahnfarbenem Füllmaterial ist eine Herausforderung. Die fluoreszenzgestützte Identifikationstechnik, FIT, ist ein diagnostisches Werkzeug, das die Unterscheidung von Kompositharz von Zahnsubstanz erleichtert. FIT ermöglicht die minimalinvasive Entfernung von Kompositharzrestaurationen und kompositgebundenen Traumaschienen.Es ist ein unkomplizierter, schneller und zuverlässiger Diagnoseansatz. Tragen Sie zunächst eine klare oder gelb getönte Schutzbrille mit UV-Schutz und dunkeln Sie dann den Raum ab. Verwenden Sie eine fluoreszenzinduzierende Lichtquelle, um die Zahnsubstanz und die zahnfarbene Kompositharzrestauration zu beleuchten.Für einen präoperativen Scan starten Sie das intraorale Scangerät und öffnen Sie die entsprechende Software für experimentelle Auswertungszwecke. Klicken Sie auf Neuen Patienten hinzufügen, um den Patienten zu registrieren. Füllen Sie die Lücken wie Nachname, Vorname, Geburtsdatum und Patienten-ID aus und drücken Sie dann Fall hinzufügen.Wählen Sie Kieferscan und Abdrücke im Abschnitt Indikationen und klicken Sie dann auf Weiter. Verdunkeln Sie den Raum und trocknen Sie das Arbeitsfeld, um den Scanvorgang zu erleichtern. Starten Sie den Intraoralscanner und führen Sie einen digitalen Oberflächenscan des Einsatzfeldes durch.Um das Verbundharzmaterial zu visualisieren, beleuchten Sie die Zahnsubstanz und die zahnfarbene Kompositharzrestauration. Entfernen Sie das Verbundharzmaterial mit einem Hochgeschwindigkeits-Winkelstück mit Diamantbohrern, einem Bindungsharzentferner und Poliervorrichtungen. Als nächstes führen Sie den postoperativen Scan für die experimentelle volumetrische Beurteilung gemäß den Schritten durch, die zuvor für den präoperativen Scan demonstriert wurden.Für die experimentelle volumetrische Beurteilung wählen Sie den Patienten und den Fall aus, öffnen Sie dann den Fall und drücken Sie Modell. Klicken Sie dann auf Exportieren, um die prä- und postoperativen Scans als Oberflächentesselierungssprache in der höchsten Auflösung zu exportieren. Öffnen Sie dann die passende Software und drücken Sie Rekombinieren.Laden Sie die prä- und postoperativen Scans in die Software hoch, indem Sie auf Importieren klicken. Drücken Sie anschließend Analyse, um die volumetrischen Veränderungen von präoperativen zu postoperativen Scans zu visualisieren. Wählen Sie die Zahnstellen aus, an denen vermutlich volumetrische Veränderungen aufgetreten sind, indem Sie im Abschnitt Werkzeuge Region wählen.Analysieren Sie die volumetrischen Änderungen mit den linearen und volumetrischen Messsoftwaretools Entfernungs- bzw. Volumenanalyse. Presseabstände unter der Rubrik Werkzeuge zur linearen Quantifizierung von Zahnsubstanzverlust und zusammengesetzten Harzresten in Farbe. Verwenden Sie den Farbbalken auf der linken Seite, um lineare volumetrische Änderungen zu quantifizieren.Suchen Sie den Cursor zusätzlich an den entsprechenden Zahnstellen und beachten Sie den genauen Cursorabstand im Kasten links. Das repräsentative Zahnmodell mit mehreren Kompositharzrestaurationen und mit Kompositharz fixierter Traumaschiene wird hier gezeigt. Die Verwendung der fluoreszenzgestützten Identifikationstechnik machte die meisten Verbundharzmaterialien heller als gesunde Zahnstrukturen.Ein Zahnmodell nach Traumaschienenentfernung unter konventioneller Beleuchtung und unter Verwendung der fluoreszenzgestützten Identifikationstechnik wird hier gezeigt. Eine volumetrische Bewertung ergab eine Quantifizierung der Kompositreste und des Zahnsubstanzverlustes. Die Diskrepanz des prä- und postoperativen Scans ergab zusammengesetzte Reste und Substanzverlust in den Zähnen 13, 12 und 11.Im Gegensatz dazu zeigten die Zähne 21, 22 und 23 nach Entfernung der Traumaschiene keine Oberflächenveränderungen. Darüber hinaus waren die Kompositreste unter herkömmlicher Lichtbeleuchtung unentdeckt und wurden mit der fluoreszenzgestützten Identifizierungstechnik sichtbar. Jede fluoreszenzinduzierende Lichtquelle ist für die FIT-Methode geeignet.Verdunkeln Sie den Raum, um Interferenzen mit anderen Lichtquellen zu vermeiden. FIT ist nicht nur für die Erkennung und Entfernung von Kompositharzrestaurationen und Traumaschienen anwendbar. Es kann auch in Bracket Debonding und Dentalforensik angewendet werden.
