卵円孔は、通常、生後数年以内に閉じる胚学的心臓発達の残骸です1。以前は、965人の正常な心臓の剖検研究で27.3%の症例で卵円孔(PFO)が見つかり、経食道心エコー検査(TEE)研究3では581人の被験者の25.6%で発見されました。性別や人種・民族に関して有意差はなく2,3,4、剖検データによると、成人のPFO直径は1mmから19mm(平均:4.9mm)まで変化し、5歳とともに増加する。

経食道心エコー検査は、PFO診断のゴールドスタンダードと見なされており、PFO閉鎖の手続き計画に利用されています。この手順は、透視法、TEEガイダンス、および生理学的モニタリングを使用して、標準的な心臓カテーテル検査室で低侵襲的に経皮的に行われます。心臓内心エコー検査(ICE)は、経験豊富なオペレータによってTEEの代替として考慮され得る14。

このプロトコルは、PFOの介入閉鎖のための標準化された手順のすべてのステップを学ぶのに役立ちます。安全で実行可能であり、ほぼすべてのPFOを閉じることができる標準化された手法を示します。まず、末梢静脈アクセスラインを確立し、500 CCの塩化ナトリウムに接続してから、患者の腕と脚を固定します。ECG、末梢酸素飽和度、脈拍測定を開始します。ヨウ素ベースの溶液を使用して右鼠径部の滅菌を行います。カテーテル助手または看護師が必要とする材料を使用して滅菌機器を準備します。滅菌した大腿骨アクセス部位を透明なホイル石膏で覆います。口腔咽頭領域に局所麻酔を5〜10回適用し、次に咬合保護を適用します。無菌条件下で超音波ガイダンスを準備します。触診、透視、または超音波ガイダンスを介して右大腿静脈にアクセスします。皮下麻酔を提供します。鼠径部の静脈穿刺を行います。次に、J-Tipと5つのフランスの多目的カテーテルを備えた標準ワイヤーを上大静脈に導入します。大腿動脈の動脈穿刺を行い、圧力監視のために4つのフレンチシースを導入します。多くのインターベンショナル心臓専門医は、安定した患者で非侵襲的血圧モニタリングが信頼できると感じたり、ラジアルアクセスを使用したりする場合があります。動脈血圧測定が確立されたら、テキスト原稿に記載されているように、ミダゾラムとプロポフォールで患者の意識的鎮静を開始します。右心房の圧力を測定します。CVPは、水銀柱の約6〜12ミリメートルの通常の範囲にある必要があります。メインCVPが低い場合は、手順中の空気塞栓症を避けるために、CVPが最低6ミリメートル水銀柱に達するまで塩化ナトリウムを注入します。TEEプローブを導入し、既存の心嚢液貯留の可能性を4室ビューで評価し、PFOを含むようにバイカバルビューを設定します。J-Tip標準ワイヤーと多目的カテーテルを左心房に進み、ワイヤーをわずかに前方に押し出し、透視とTEEガイダンスの下で左上部肺静脈に配置します。30度から110度までスイープすることで最適なTEEイメージングを実現します。これは役立つかもしれないので、45度の左前方斜めのビューを取得します。カテーテルが左心房に導入されたら、体重1キログラムあたり100IUのヘパリンのボーラス注射を投与して、約250秒の活性化凝固時間に到達します。活性化凝固時間は、ポイントオブケアACTアナライザーによって約5分後に制御する必要があります。多目的カテーテルを介して左心房圧を測定します。水銀柱が5ミリメートル未満の場合は、IV液を投与して、水銀柱の平均左心房圧が5〜10ミリメートルに達するようにします。上部肺静脈をターゲットにして、カテーテルに沿って柔らかい先端を持つ硬くて長いガイドワイヤーを左心房に進めます。次に、患者の外側で4分の1から4分の2の造影剤と生理食塩水を混ぜてバルーンを注意深く除気します。多目的カテーテルを24ミリメートルサイズのバルーンに交換します。バルーンをPFOの向こう側に配置し、造影剤溶液中の0.9%塩化ナトリウムを3対1の比率で膨らませます。PFO測定には、2つの垂直TEE投影を使用します。血管造影画像で寸法を確認します。どちらの場合も、最小のバルーンテールの測定が重要です。サイジングチャートを使用して、製造元の指示に従ってTEEまたはTTEによって評価されたPFOサイズとセプタム全長を使用して、PFO閉塞デバイスのサイズとタイプを選択します。テキスト原稿に記載されているように、適切なサイズの閉鎖装置を準備します。そして、気泡を除去するために適切にフラッシュします。サイジングバルーンを収縮させ、デリバリーシースと交換します。硬いガイドワイヤーを介してデリバリーシースを導入します。次に、イントロデューサーとワイヤーを取り外します。血液の吸引後、シースとPFO閉鎖装置の間の濡れた接続を確保して、装置の気泡のない前進を保証します。サイドポートから生理食塩水を洗い流し、シースからの逆出血を確実にすることにより、ウェットツーウェット接続を介してシースを介して閉鎖装置を導入します。次に、デバイスをシースに前方に押し込みます。デバイスがシース内に見えたら、デバイスの前に気泡がないかどうかを確認します。次に、デバイスを約2センチ引き戻して、デバイスが配送ケーブルに接続されていることを確認します。デバイスを左心房に前方に押し込み、シースを左心房中央に慎重に引っ込めて、プッシュアンドプル運動と心エコーガイダンスにより、左側の椎間板を広げることができます。ここでは、血管造影の代わりにTEEガイダンスで十分であり、放射線量を減らすことが推奨されます。微妙な抵抗が感じられ、左側の椎間板がTEEによって中隔に見えるまで、左側の椎間板を心房中隔に向かって引きます。控えめな張力の下で、シースを右心房に引き戻し、右側の椎間板を広げます。椎間板が付着するまで、デバイスを心房中隔にそっと押し付けます。TEEと血管造影で閉鎖装置の正しい位置を評価します。後者の場合、Pacman記号は正しいデバイス配置を示します。局所的な圧迫損傷につながる可能性のある大動脈または左心房の屋根に関するずれがないか、デバイスのエッジを徹底的に検査します。プッシュプルタグテストは、デバイスの安定性を示すのに役立ちます。デバイスの位置が正しいら、配送ケーブルを外し、シースを取り外します。皮下組織材料の8針の字または血管閉鎖装置を用いて吸収縫合糸で大腿静脈を閉じる。局所的な出血を防ぐために、しっかりと座っている圧力包帯を使用してください。動脈血圧監視装置を取り外した後、総大腿動脈を手動で圧迫するか、閉鎖装置を使用してからきつい包帯を適用します。血圧と心拍数の非侵襲的モニタリングを24時間続けます。テキスト原稿に記載されているように、ヘパリンを24時間提供します。65歳未満の若年患者では、内科的治療のみと比較して、介入PFO閉鎖後に再発性脳卒中および死亡のリスクが低いことが観察された。PFO閉鎖におけるインターベンション周辺合併症はまれであった。心膜タンポナーデは、RESPECTおよびREDUCE試験でそれぞれ0.4%および0.2%の割合でめったに発生しなかった。心臓穿孔は重篤な合併症であり、CLOSURE-I試験では患者の0.25%、RESPECT試験では0.2%に発生した。可能な限り最良の結果を得るには、デバイスの開閉と位置決めをTEE制御の下で行うことが重要です。TEEが利用できない場合は、造影剤注入によってデバイスの位置を確認することもできます。
