Osteonecrose van de heupkop (ONFH) is een veel voorkomende invaliderende ziekte die voorkomt bij jonge volwassenen1. Klinisch is het noodzakelijk om de stadiëring van ONFH te bepalen op basis van röntgenfoto's, CT en MRI om de behandelingsstrategie te bepalen (figuur 1). Voor onfh in een vroeg stadium wordt meestal therapie voor heupbehoud toegepast2. Core decompressie (CD) chirurgie is een van de meest gebruikte heupconserveringsmethoden voor ONFH. Bepaalde curatieve effecten van kerndecompressie met of zonder bottransplantatie bij de behandeling van ONFH in een vroeg stadium zijn gemeld, wat latere totale heupartroplastiek (THA) gedurende lange tijd kan voorkomen of vertragen 3,4,5. Het succespercentage van CD met of zonder bottransplantatie werd echter anders gerapporteerd in eerdere studies, van 64% tot 95%6,7,8,9. De chirurgische techniek, met name de nauwkeurigheid van de boorpositie, is belangrijk voor het succes van heupbehoud10. Vanwege de blindheid van de punctie- en positioneringsprocedure hebben de traditionele technieken van CD verschillende problemen, zoals meer fluoroscopietijd, herhaalde punctie met Kirschner-draad en letsel van normaal botweefsel 11,12.
In de afgelopen jaren is de augmented reality (AR)-geassisteerde methode geïntroduceerd in de orthopedische chirurgie13. De AR-techniek kan de anatomie van het chirurgische veld visueel weergeven, de chirurgen begeleiden bij het plannen van de operatieprocedure en bijgevolg de moeilijkheidsgraad van de operatie verminderen. De toepassingen van de AR-techniek bij pedikelschroefimplantatie en gewrichtsarthroplastiekchirurgie zijn eerder gemeld 14,15,16,17. In deze studie willen we de AR-techniek toepassen op de CD-procedure en de veiligheid, nauwkeurigheid en haalbaarheid ervan in de klinische praktijk verifiëren.
Hardwarecomponenten van het systeem De belangrijkste componenten van het op AR gebaseerde chirurgische navigatiesysteem zijn het volgende: (1) Een dieptecamera (figuur 2A) die direct boven het operatiegebied is geïnstalleerd; de video wordt hiervan geschoten en teruggestuurd naar de werkplek voor registratie en samenwerking met de beeldgegevens. (2) Een prikinrichting (figuur 2B) en een niet-invasief frame voor het markeren van het lichaamsoppervlak (figuur 2C), beide met passieve infraroodreflectoren. Een speciale reflecterende coating van markeerballen (figuur 3) kan worden opgevangen door infraroodapparatuur om nauwkeurige tracking van chirurgische apparatuur in het operatiegebied te bereiken. (3) Een infrarood positioneringsapparaat (figuur 2D) is verantwoordelijk voor het volgen van markeringsmarkeringen in het operatiegebied, waarbij het markeringsframe van het lichaamsoppervlak en de punctievoorziening met hoge nauwkeurigheid worden afgestemd (figuur 4). (4) Het hostsysteem (figuur 2E) is een 64-bits werkstation, geïnstalleerd met het onafhankelijk ontwikkelde AR-geassisteerde orthopedische chirurgiesysteem. Augmented reality-weergave van heupgewricht en femurkoppunctieoperatie kan met zijn hulp worden voltooid.

Osteonecrose van de heupkop (ONFH) is een veel voorkomende gewrichtsaandoening bij jonge en middelbare leeftijd patiënten, die hun leven en werk ernstig belast. Voor onfh in een vroeg stadium is kerndecompressiechirurgie een klassieke en effectieve heupconserveringstherapie. Bij traditionele procedures van kerndecompressie met Kirschner-draad zijn er nog steeds veel problemen, zoals blootstelling aan röntgenfoto's, herhaalde punctieverificatie en schade aan normaal botweefsel. De blindheid van het prikproces en het onvermogen om real-time visualisatie te bieden zijn cruciale redenen voor deze problemen.
Om deze procedure te optimaliseren, ontwikkelde ons team een intraoperatief navigatiesysteem op basis van augmented reality (AR) technologie. Dit chirurgische systeem kan intuïtief de anatomie van de chirurgische gebieden weergeven en preoperatieve beelden en virtuele naalden in realtime weergeven op intraoperatieve video. Met de gids van het navigatiesysteem kunnen chirurgen Kirschner-draden nauwkeurig in het beoogde laesiegebied inbrengen en de nevenschade minimaliseren. We hebben 10 gevallen van kerndecompressiechirurgie uitgevoerd met dit systeem. De efficiëntie van positionering en fluoroscopie is sterk verbeterd in vergelijking met de traditionele procedures en de nauwkeurigheid van de punctie is ook gegarandeerd.

Werkingskenmerken Het chirurgische navigatiesysteem werd toegepast in continu 10 heupen van negen patiënten. De gemiddelde totale positioneringstijd van de operatie was 10,1 min (mediaan 9,5 min, bereik 8,0-14,0 min). De gemiddelde C-ARM fluoroscopieën waren 5,5 keer (mediaan 5,5 keer, bereik 4-8 keer). De gemiddelde fout van leknauwkeurigheid was 1,61 mm (mediaan 1,2 mm, bereik -5,76-19,73 mm; Tabel 1). De resultaten laten zien dat de positioneringstijd en fluoroscopietijden duidel...

Ons systeem kan het slagingspercentage en de efficiëntie bij kerndecompressiechirurgie voor femorale hoofdnecrose verbeteren. Het vermindert ook aanzienlijk de intraoperatieve positioneringstijd en de fluoroscoopbron. Met VR-technologie kunnen we het punctieproces gebruiken, de moeilijkheidsgraad van de operatie verminderen en minder extra schade aan de patiënt veroorzaken dan traditionele chirurgische resultaten.Het systeem kan worden uitgebreid naar andere orthopedische chirurgie met punctieprocedures. Het is gebruikt om percutane endoscopische transforaminale discectomie procedure te krijgen. Beweeg het prikapparaat willekeurig in het operatiegebied om de overeenkomende mate van virtuele naald en trackingvertraging te detecteren.Vraag de patiënt in de operatiekamer om in rugligging te gaan liggen en de onderste ledemaat van de aangedane kant te fixeren. Bereid de operatieplaats voor met jodium en 75% alcohol. Verplaats de C-arm fluoroscoop naar de zijkant van de operatietafel en plaats de bron boven het heupgewricht.Lijn de bron uit met de dieptecamera en noteer de positie van de operatietafel als positie één. Klik in het AR-geassisteerde orthopedische chirurgiesysteem op de voorste röntgenfoto van het bestand en selecteer afbeelding één. Het systeem identificeert automatisch het markeringsframe en plaatst dit beeld op het heupgewricht in de chirurgische video.Gebruik de AR-weergave van het röntgenbeeld en op basis van de realtime video om het prikpad te plannen. Ga aan de aangedane kant staan voor het uitvoeren van de procedure, houd het prikapparaat vast en bepaal de beste inbrenghoek. Markeer het inbrengpunt op het huidoppervlak, geleid door de virtuele Kirschner-draad en het röntgenbeeld van het heupgewricht in de chirurgische video.Prik met behulp van een Kirschner-draad door het insteekpunt. Observeer de invoegdiepte en -hoek in de video en pas deze tijdig aan. Wanneer de virtuele naald het doelgebied van necrose heeft bereikt, stopt u het punctieproces en bewaart u de schermafbeelding als afbeelding twee voor de daaropvolgende evaluatie van de punctienauwkeurigheid.Trek na de punctie de boor eruit en laat de Kirschner-draden tijdelijk in het bot achter. Verplaats de operatietafel naar de positie voor de tweede fluoroscopie om de werkelijke punctieconditie van de Kirschner-draad te controleren. Neem het afbeeldingsbestand op.Punctie is succesvol wanneer de locatie van de Kirschner-draad aan alle eisen van de chirurg voldoet. Gebruik vervolgens de Lancet om de huid rond de naald te snijden en elk niveau van zacht weefsel te scheiden totdat het subtrochanterbot wordt blootgesteld aan ongeveer een diepte van drie centimeter. Boor in het necrotische gebied langs de Kirschner-draad met een trephine van vijf millimeter om de volgende bewerkingen te voltooien.Na het voltooien van alle procedures, sluit u de huid met drie oh zijden draad en bedekt u deze met steriel verband. Het chirurgische navigatiesysteem werd toegepast in continue tien heupen van negen patiënten. Met een gemiddelde totale positioneringstijd van 10,1 minuten tijdens de operatie waren de gemiddelde C-arm fluoroscopieën 5,5 keer.De gemiddelde fout van de leknauwkeurigheid was 1,61 millimeter. Door de heupen geëvalueerd, waren twee heupen in ARCO I-stadium, vier heupen waren in ARCO IIA-stadium en vier in ARCO IIB-stadium. De gemiddelde preoperatieve VAS-score was zes en de gemiddelde postoperatieve score was 3,75.De gemiddelde preoperatieve Harris-score was 77,5 en de gemiddelde postoperatieve score was 85,5. Er werden geen postoperatieve complicaties zoals infectie, hematoom of zenuwbeschadiging gevonden. Het is ook mogelijk om de procedure te combineren met CT driedimensionale reconstructietechnologie, zodat de punctie vanuit een driedimensionaal perspectief kan worden gerealiseerd, wat ons toekomstige werk is.We bieden een innovatief idee om AI-technologie en orthopedische chirurgie te combineren. Dit idee kan ook worden gebruikt om de precisie van andere orthopedische operaties in de toekomst te verbeteren.
