대퇴골 두부의 골괴사 (ONFH)는 젊은 성인에서 발생하는 일반적인 비활성화 질환입니다1. 임상적으로, 치료 전략을 결정하기 위해서는 X선, CT 및 MRI를 기반으로 ONFH의 스테이징을 결정할 필요가 있습니다 (그림 1). 초기 단계 ONFH의 경우, 고관절 보존 요법이 일반적으로 채택됩니다2. 코어 감압 (CD) 수술은 ONFH에서 가장 자주 사용되는 고관절 보존 방법 중 하나입니다. 초기 단계 ONFH를 치료하는데 있어서 골 이식술의 유무에 관계없이 코어 감압의 특정 치료 효과가 보고되었으며, 이는 장시간 동안 후속적인 총 고관절성형술(THA)을 회피하거나 지연시킬 수 있다3,4,5. 그러나, 골 이식술의 유무에 관계없이 CD의 성공률은 이전 연구들 사이에서 64%에서 95%로 다르게 보고되었다 6,7,8,9. 수술 기술, 특히 드릴링 위치의 정확성은 고관절 보존(10)의 성공을 위해 중요하다. 펑크 및 위치 결정 절차의 실명으로 인해, CD의 전통적인 기술은 더 많은 형광 검사 시간, Kirschner 와이어를 사용한 반복적 인 펑크 및 정상 뼈 조직의 손상11,12와 같은 몇 가지 문제를 가지고 있습니다.

대퇴골 두부의 골괴사 (ONFH)는 젊은 및 중년 환자에서 흔히 발생하는 관절 질환으로, 삶과 업무에 심각한 부담을줍니다. 초기 단계 ONFH의 경우, 코어 감압 수술은 고전적이고 효과적인 고관절 보존 요법입니다. Kirschner 와이어를 사용한 전통적인 코어 감압 절차에는 X 선 노출, 반복적 인 펑크 검증 및 정상 뼈 조직의 손상과 같은 많은 문제가 여전히 있습니다. 펑크 프로세스의 실명과 실시간 시각화를 제공 할 수 없다는 것은 이러한 문제의 중요한 이유입니다.
이 절차를 최적화하기 위해 우리 팀은 증강 현실 (AR) 기술을 기반으로 수술 중 내비게이션 시스템을 개발했습니다. 이 수술 시스템은 수술 영역의 해부학을 직관적으로 표시하고 수술 전 이미지와 가상 바늘을 수술 중 비디오로 실시간으로 렌더링 할 수 있습니다. 내비게이션 시스템의 가이드를 통해 외과의는 Kirschner 와이어를 표적 병변 영역에 정확하게 삽입하고 부수적 인 손상을 최소화 할 수 있습니다. 우리는이 시스템으로 핵심 감압 수술의 10 사례를 실시했습니다. 위치 지정 및 형광 검사의 효율성은 전통적인 절차에 비해 크게 향상되었으며 펑크의 정확성도 보장됩니다.
