As injeções intravítreas são um dos procedimentos cirúrgicos mais comuns na oftalmologia humana e provaram ser eficazes na administração de terapias genéticas mediadas por AAV para a retina de ovinos. Vários métodos podem ser usados para fornecer agentes terapêuticos à retina, incluindo administração intravítrea (IVT), sub-retiniana, supracoroidal, periocular ou tópica. Cada via de administração envolve a superação de barreiras como a barreira sangue-retina ou as membranas limitantes internas e externas e tem taxas variadas de eficácia dependendo do fármaco a ser administrado e do alvo específico da retina 1,2.
A administração da droga IVT envolve uma injeção no humor vítreo do olho, uma substância gelatinosa que ocupa a câmara posterior do olho. A principal função do humor vítreo é manter a forma do globo ocular e manter os tecidos oculares, como a lente e a retina, no lugar. O humor vítreo é composto em grande parte de água, com pequenas quantidades de colágeno, ácido hialurônico e outras proteínas não colágenas3. A injeção de TVI é um procedimento simples e comum usado rotineiramente para tratar uma ampla gama de condições oculares, incluindo degeneração macular relacionada à idade, edema macular diabético, retinopatia diabética, oclusão da veia retiniana e várias distrofias hereditárias da retina 4,5.
Lipofuscinoses ceroides neuronais (NCL; Doença de Batten) é um grupo de doenças fatais de armazenamento lisossômico que causam degeneração grave do cérebro e da retina. Atualmente, existem 13 variantes conhecidas de NCL resultantes de mutações em diferentes genes (CLN1-8, CLN10-14) que afetam predominantemente crianças, mas têm idades variadas de início e gravidade da doença6. As NCLs compartilham sintomas progressivos comuns, incluindo declínio cognitivo e motor, convulsões e perda de visão. Não há cura para a NCL; no entanto, a terapia de reposição enzimática dirigida pelo cérebro está atualmente em ensaios clínicos para a doença CLN27,8, e a terapia gênica mediada por AAV tem se mostrado grande promessa em estudos pré-clínicos, com um ensaio clínico para a terapia gênica CLN5 esperado para começar em 2022 9,10.
Muitas outras espécies desenvolvem formas naturais de NCL, incluindo gatos, cães, ovelhas e vacas. Dois modelos ovinos de NCL estão atualmente em estudo ativo na Nova Zelândia: um modelo de doença CLN5 em ovinos de Borderdale e um modelo de doença CLN6 em ovinos de South Hampshire. Os ovinos afetados apresentam muitas das características clínicas e patológicas da doença humana, incluindo atrofia retiniana e perda de visão10,11. Embora a terapia gênica CLN5 dirigida pelo cérebro em ovinos com doença CLN5 possa prevenir ou deter a atrofia cerebral e o declínio clínico, as ovelhas tratadas ainda perdem a visão9. O olho de ovelha representa um bom modelo do olho humano devido à sua semelhança nas dimensões do globo ocular, volume vítreo e estrutura da retina10,12,13. Este trabalho detalha o protocolo cirúrgico para a administração de TVI de um pequeno volume (≤100 μL) de vetor viral terapêutico ao olho ovino.
