様々な要因が肺に異常な粘液の産生をもたらし得る。COPDでは、タバコの煙の慢性吸入は、杯細胞の化生の結果として粘液過分泌を引き起こし、嚢胞性線維症膜貫通コンダクタンス調節因子(CFTR)チャネルのダウンレギュレーションを介した気道脱水を引き起こし、粘液過集中および小さな気道閉塞を引き起こす7,8。同様に、CFTR遺伝子の変異に関連する遺伝性疾患であるCFは、輸送には不十分である粘性のある付着性粘液の産生によって特徴付けられる8、9。簡単に言えば、CFTR機能障害は、気道表面の液体枯渇、ポリマームチン絡み合い、および生化学的相互作用の増加を誘発し、慢性炎症および細菌感染をもたらす。さらに、静置粘液に閉じ込められた炎症細胞は、別の大きな分子であるDNAをゲルマトリックスに添加することによって粘液の粘弾性をさらに悪化させ、気道閉塞を悪化させる5。肺の全体的な健康に対する粘液レオロジーの重要性の最良の例の1つは、嚢胞性線維症患者の治療における組換えヒトDNFase(rhDNase)の例によって提供される。rhDNaseの効果は、去痰性痰に対するエクスビボで最初に実証され、粘性粘液から流動液体への移行を数分以内に示した10、11。CF患者を対象とした臨床試験では、rhDNase吸入による気道粘液粘弾性の低下が肺悪化の速度を低下させ、肺機能および患者全体の幸福を改善することが実証された12,13,14。その結果、クリアランスを容易にすることを目的としたrhDNase吸入は、20年以上にわたってCF患者の標準的なケアとなった。同様の臨床的利益は、CFにおける粘液水和のための吸入高張生理食塩水の使用によって観察され、これはレオロジー特性の変化と相関し、粘膜繊毛クリアランス加速および肺機能の改善をもたらした15,16。したがって、臨床現場で粘液粘弾性特性を測定するための迅速かつ信頼性の高いプロトコルは、治療アプローチを最適化するために重要です。
ここで試験されたベンチトップレオメーターは、粘液/喀痰サンプルの包括的な粘弾性測定を実行するための迅速かつ便利な代替手段を提供します。制御された角度変位を備えた動的振動を使用して、この装置は調整可能な平行プレート(例えば、粗いまたは滑らかな形状)のペアを介して変形を提供し、15nNの分解能でトルクと変位を測定します。mおよび150nmは、それぞれ17である。デフォルトの標準化された校正と、レオロジー以外の専門家向けに調整されたユーザーガイドラインを組み合わせることで、簡単な測定が可能になり、オペレータのエラーのリスクが軽減されます。このデバイスは、リアルタイム(約5分以内)で処理および分析される歪みスイープ曲線を生成し、線形粘弾性(G '、G"、G*、および黄褐色δ)とゲル化点(γ c、およびσc)特性の両方を自動的に提供します(表1を参照)。弾性率または貯蔵弾性率(G ́)は、サンプルが応力にどのように応答するか(すなわち、元の形状に戻る能力)を表し、粘性弾性率または損失弾性率(G")は、正弦波変形のサイクルごとに散逸するエネルギー(すなわち、分子の摩擦によって失われるエネルギー)を記述する。複素弾性率または動的弾性率(G*)は、ひずみに対する応力の比であり、せん断変位に応答して蓄積する内部力の量(すなわち、全体的な粘弾性特性)を記述する。減衰係数(黄褐色δ)は、粘性弾性率と弾性率の比であり、サンプルがエネルギーを放散する能力を示す(すなわち、低い黄褐色δは弾性優勢/固体様挙動を示し、高い黄褐色δは粘性優勢/液体様挙動を示す)。ゲル化点特性の場合、クロスオーバーひずみ(γc)は、試料が固体状から液体状に遷移し、定義上、G'=G"または黄褐色δ=1の振動ひずみで生じるせん断ギャップ高さに対するたわみ経路の比によって計算される剪断ひずみの尺度である。クロスオーバー降伏応力(σc)は、弾性弾性率と粘弾性率が交差するデバイスによって加えられる応力量の尺度である。健康な喀痰では、弾力性がひずみに対する機械的反応を支配する(G'>G")。粘液閉塞性疾患では、病理学的粘液変化の結果としてG'およびG"の両方が増加する17、18、19。この装置の操作上のシンプルさにより、オンサイト測定が容易になり、分析のためにオフサイト施設へのサンプルの保管/輸送/出荷の必要性を回避し、これらの生物学的サンプルの特性に対する時間と凍結融解の影響を回避します。
この研究では、異なる濃度(1%〜3%)の8つのMDaポリエチレンオキシド(PEO)溶液を使用して、市販のベンチトップレオメーター(材料表)の測定範囲を検証し、得られた濃度依存曲線を従来のバルクレオメーター(材料表)で取得した測定値と直接比較しました。).次に、レオロジー測定の再現性を、気管支痙攣、好酸球性炎症、および環境または感染性因子に応答した粘液過産生を特徴とする喘息悪化の極端な形態であるステータス喘息(SA)に罹患している挿管患者から気管支鏡的に採取された粘液を用いて評価した8,20.この場合、SA患者は重度の呼吸不全のために挿管されており、積極的な標準的な喘息治療にもかかわらず、機械的換気だけで患者を効果的かつ安全にサポートすることができないため、ECMO(体外膜酸素化)を必要としていた。葉椎虚脱のための臨床的に示された気管支鏡検査の間、厚く、透明で粘り強い分泌物が葉細気管支を妨害していることが指摘され、生理食塩水洗浄を用いて吸引された。収集の直後に、吸引液から余分な生理食塩水を除去し、残りのSAサンプルの粘弾性特性をベンチトップ装置を用いて分析した。追加のサンプルアリコートを還元剤であるトリス(2−カルボキシルエチル)ホスフィン塩酸塩(TCEP)で処理し、このプロトコールがエクスビボでの治療化合物有効性を特徴付けるために使用され得るかどうかを決定する。
その結果、このプロトコルとベンチトップ装置が臨床現場で効果的に使用できることが示されました。PEO濃度依存曲線(図1A)から決定されたレオロジー特性は、試験済みのベンチトップ装置と従来の平行平板レオメータ(図1B)とでは区別がつかなかった。SA粘液の3連測定は反復可能で、G*、G'、およびG"エンドポイントの変動係数は10%であり、この患者の症例で臨床的に明らかであった粘液粘弾性の実質的な異常を反映していました(図1D)。最後に、TCEPによる エキソビボ 治療は、G'およびG"の有意な減少、および黄褐色δの増加をもたらし、ムチンネットワークの変化による治療に対する応答性を実証した(図2)。結論として、ベンチトップレオメーターを使用するこのプロトコルは、診療所から得られた粘液サンプルの粘弾性特性を評価するための簡単で効果的なアプローチを提供します。この機能は、臨床医が承認された粘膜活性薬の有効性を現場でテストできるため、ケアへの精密医療アプローチを促進するために使用され、代替治療選択肢を特定するのに役立ちます。さらに、このアプローチは、治験薬の効果を調べるための臨床試験で使用することができる。
