精巣上体閉塞は、閉塞性無精子症の最も一般的な原因です。顕微手術精巣上体切除術(MVE)は、精巣上体閉塞の主な外科的治療法です。様々なMVE技術1,2,3,4が以前に説明されているが、Chanら5,6によって記述された2本針縦断腸重積血管上体吻切開術(LIVE)技術は、優れた開存率を達成するためのゴールドスタンダードとして認識されている7。.精管血管は通常、LIVEで結紮されます。血管の結紮は手順を簡素化し、吻合の緊張を軽減し、緊張のない吻合は成功した結果を達成するために重要です。
精管動脈は主に精巣上体と精管に供給します8。輸精管動脈の温存が術後の開存率を改善するかどうかは不明である。ただし、MVE中に血管血管を保存すると、正常な生理学的構造をより適切にシミュレートできる可能性があります。また,精索静脈瘤摘出術を受けた患者にとっては,精索静脈瘤摘出術後の血液供給や精巣の静脈還流に血管血管系の完全性が重要な役割を果たすため,血管温存MVEを行うことは有意義であると考えられる。いくつかの研究9,10,11は、MVE中に血管を保存しようと試みており、この手順には特定の利点がある可能性があることが示唆されています。しかしながら、血管血管を保存することは吻合の操作をより困難にし、そして吻合の緊張を増大させる。さらに、輸精管の緊張が血管血管に伝達され、血管の小さな枝の術後出血を引き起こす可能性があります。その上、既存の血管温存手順は、輸精管の血液供給に影響を与える可能性があります。そこで、血管温存MVEの手術技術を改善し、より安全なものにしました。この新しい手順を、逆行性順行性解離を使用して血管血管を保存することにより、顕微手術精巣上体吻合術と名付けました。

顕微手術精巣上体切除術(MVE)は、精巣上体閉塞の主な外科的治療法です。血管血管はMVE中に結紮されます。しかし、MVE中に血管血管を保存することは、正常な生理学的構造をよりよくシミュレートし、精索静脈瘤摘出術を受けた患者にとって意味があるかもしれない。それにもかかわらず、血管血管を保存すると、吻合の緊張が高まり、開存率に影響を及ぼし、術後出血の遅延につながるリスクが高まる可能性があります。そこで、より安全にするために、船舶を節約する新しいMVEを開発しました。手順の改善点の概要を次に示します。1)近位精巣側の血管の逆行性解離を主な方法として採用し、遠位精巣側の血管の順行性解離を、保存する家臣血管を解離するための補足として採用した。この改善により、吻合に使用される輸精管への血液供給が確保され、血管血管が長くなり、吻合の緊張が低下します。2)吻合する輸精管と輸精管の壊れた端を固定することにより、自由な血管血管が固定され、精管血管への輸精管張力の伝達の問題が解決され、血管血管出血のリスクが軽減されます。3)膣中膜を開いた後の輸精管の解離は、輸精管の動員を増加させ、新しい手順を完了しやすくします。本手技のアウトカムを評価した結果,術後合併症の有意性は認められず,開存率は従来と変わらなかった。したがって、この新しい改善された手順は、術後に満足のいく結果をもたらし、安全であると見なすことができます。
