静脈移植片疾患(VGD)は、冠状動脈バイパスグラフト(CABG)障害の主な原因です。CABG-VGDの大型動物モデルは、疾患メカニズムの解明と治療戦略の開発に必要です。 
手術を行うには、第3肋間腔から心腔に入り、内乳腺静脈を慎重に解剖し、通常の生理食塩水に浸します。その後、右主冠状動脈の虚血の治療が行われます。標的血管を切開し、シャントプラグを配置し、移植静脈の遠位端を吻合する。上行大動脈は部分的に塞がれており、移植静脈の近位端は穿孔後に吻合されています。移植静脈の開存性をチェックし、近位右冠状動脈を結紮します。 
CABG手術は、血管移植片として使用するために左内乳静脈を採取するためにミニブタで行われます。血清生化学的検査は、手術後の動物の生理学的状態を評価するために使用されます。超音波検査は、移植血管の近位、中端、および遠位端が塞がれていないことを示しています。手術モデルでは、CABG手術後の組織学的検査で移植片内の乱流が観察され、移植片には内膜過形成に伴う静脈グラフト狭窄が観察されます。ここでの研究は、再現性のあるCABG誘発VGDモデルを確立するための詳細な外科的処置を提供します。

このモデルは、冠動脈移植疾患の動物モデルを確立するための標準プロトコルを提供します。この方法で確立された動物モデルの手順は標準的であり、結果は安定しています。まず、体重30〜35キログラムの生後3か月のオスのミニブタを、それぞれ5匹の動物の偽グラフト疾患グループと静脈移植片疾患グループに分けます。動物に麻酔をかけた後、留置静脈カテーテルを辺縁耳静脈に挿入して耳のアクセスを確立します。次に、豚を仰臥位の操作テーブルに移します。7.0〜7.5フレンチゲージチューブで気管挿管を行い、麻酔呼吸回路に接続します。包帯で手足を固定し、滅菌ドレープで頭を上げます。筋弛緩のために臭化ベクロニウムを静脈内注射した後、心電図を使用して心拍数、血中酸素濃度、体温を監視します。手術部位を剃って滅菌した後、その周りに滅菌手術用ドレープを置きます。本文記載のとおり切開を行い内乳腺静脈を露出させた後、胸骨左側に内乳静脈と左内乳動脈を配置し、血管鉗子で内乳静脈の鈍的解剖を行う。電気ナイフで左内乳静脈枝の電気凝固によって止血を行う。止血が不完全な場合は、綿糸を使用して、収穫中に静脈の両端を結紮してマークを付けます。静脈が除去されたら、前処理のためにヘパリン、通常の生理食塩水を静脈に注入します。次に、静脈を通常の生理食塩水に入れ、バックアップのために保管し、同様に偽グループの内乳腺静脈を切開します。次に、心膜を開き、胸壁を閉じ、冠状動脈バイパス移植なしで内乳静脈を病理学的制御に使用します。心膜を電動ナイフで約7センチ切開し、右冠動脈幹を露出させます。心膜を吊り下げ、1-0の外科用縫合糸で同側側の皮膚を縫います。次に、右冠状動脈幹を周囲の組織から分離します。次に、ワイヤーフックを使用して、大動脈近くの孤立した右冠状動脈の近位端の下の遮断バンドをバイパスします。次に、ブロッキングバンドを締めて弛緩させることにより、2分間の虚血と5分間の再灌流の3サイクルで心筋を治療します。また、虚血再灌流プレコンディショニング中に心電図で心臓の電気的活動を監視します。次に、バンドを締めた後に血管を覆う心外膜を切断して右冠動脈血流を遮断します。冠状動脈壁を露出させ、血管の前壁の中心に対して外科用刃の先端で縦方向に切断する。内腔を切断し、切開部を拡大し、冠状動脈シャントを配置した後、シャントの一端をコイルで涙液を通して遠位冠状動脈に挿入します。次に、冠状動脈の血液を中空の冠状動脈シャントにシャントして明確な手術野を確保し、6-0ポリプロピレン縫合糸を使用して内乳静脈と大動脈壁の間でエンドツーエンドの連続縫合を行います。上行大動脈の中央で、上行大動脈の左前外側壁を半閉塞クランプで閉塞する。外膜を切断した大動脈壁に手術刃で小切開を行った後、パンチの頭端にあるスライドシャフトの頭端をこの切開を通して大動脈腔に挿入します。次に、スライドシャフトを外側に収縮させ、円形ナイフが動脈壁の一部を切り取ります。シャントが引き抜かれたら、6-0ポリプロピレン縫合糸を使用して、内乳静脈と右冠状動脈の間でエンドツーエンドの連続縫合を行います。半閉塞クランプを開いた後、超音波を用いて吻合部位の近位の右冠動脈幹のバイパス流を記録する。前に示したように、手術のために動物を準備します。そして、電動ナイフで、10センチの胸骨中央切開を行い、胸骨を分割し、術後30日目に静脈を採取します。分離している間、主血管、心臓を避け、移植された血管を層ごとに分離します。心臓につながる大きな血管をすばやく切断し、心臓と上行大動脈をアイスチップの上に置きます。移植血管ブリッジ、接続された大動脈、および右冠状動脈が除去されたら、摂氏4度の生理食塩水ですべてのサンプルをすすぎます。約3〜4センチメートルのグラフト容器全体を4〜5等分した後、それをCryoTubeに移して液体窒素中で素早く瞬間凍結させる。分析のために、氷冷した生理食塩水ですすいだグラフトを4%パラホルムアルデヒド溶液に固定します。12時間の組織固定後、切片を50ミリリットルのヘマトキシリン水溶液で3分間染色する。次に、50ミリリットルの0.5%塩酸エタノールと0.2%アンモニア水でそれぞれ10秒間洗浄して切片を分離します。切片を流水で1時間すすぎたら、蒸留水に3分間浸して洗浄します。切片を70%および90%エタノールで10分間脱水した後、50ミリリットルの0.5%アルコールエオジン染色液に2〜3分間入れます。染色した切片を純粋なエタノールで脱水したら、サンプルを純粋なキシレンに10分間浸して透明にします。最後に、透明な部分を中性接着剤で滴下してから、カバースリップで覆います。病理切片を光学顕微鏡で40倍の倍率で観察する。超音波検査の結果,全体的な血流方向はグラフト血管の近位端,血管腔,遠位端で正常であった。しかしながら、移植血管の近位端および遠位端は、いくらかの逆行性血流を示した。内乳静脈の組織学的分析により、偽群では内膜、中膜、静脈移植片の静脈壁が正常に見えることが明らかになりました。しかし、冠動脈移植の30日後、血管の内膜が肥厚し、内腔が狭くなった。偽および静脈移植疾患群のうち、アスパラギン酸アミノトランスフェラーゼ、血清ビリルビン、総ビリルビン、クレアチニンの4つの生化学的指標が統計学的に有意な変化を示した。外科医は、内乳腺静脈を完全に露出させ、周囲の組織に付着していないことを確認する必要があります。この技術は、動物モデルにおける静脈移植疾患を研究するための標準化されたモデリングプロセスを提供する。
