최근 몇 년 동안 관상 동맥 심장 질환 사망률이 크게 감소했지만, 미국 중년 성인의 절반이 매년 허혈성 심장 관련 증상을 나타내며 노인의 1/3이 관상 동맥 심장 질환으로 사망합니다1. 관상동맥우회술(CABG)은 심근 허혈을 개선하는 효과적인 수술 방식이며, 더 중요한 것은 다혈관 관상동맥 질환 치료를 위한 대체할 수 없는 수술 방식입니다2. 그러나 시간이 지남에 따라 혈관 이식편은 염증, 내막 증식 및 진행성 죽상동맥경화증을 일으켜 정맥 이식 실패 또는 정맥 이식 질환(VGD)을 유발하는 것으로 알려져 있습니다3. CABG 후 환자에서 재협착이 발생하면 경우에 따라 병든 혈관만 교체할 수 있다2. 고령 환자와 추가된 동반 질환으로 인해 관상동맥 우회술을 다시 하는 것이 상당히 어렵습니다. 이식된 혈관과 관련된 병리학적 문제를 지연시키거나 조절하는 것은 해결해야 할 시급한 문제이다. CABG-VGD의 대형 동물 모델은 질병 메커니즘의 조사 및 치료 전략의 개발에 필요합니다. 연구자들은 생쥐4, 쥐5, 토끼6, 돼지7과 같은 크고 작은 동물에서 동물 VGD 모델을 성공적으로 확립했다. 작은 동물에 비해 돼지와 같은 큰 동물은 인간과 유사한 해부학적 구조와 생리학적 특성을 가지고 있으며 수명이 더 깁니다 8,9. 따라서 대형 동물은 정맥 이식편 질환의 장기적인 병리학 적 변화를 탐구하고 약물 또는 장치의 전임상 시험에 더 적합합니다. 우리와 우리의 협력 팀은 돼지 심부전 모델을 확립하기 위해 수술 기법을 성공적으로 적용했으며 이 모델의 심장 병리학적 변화를 설명했습니다10.
CABG 수술은 임상에서 표준화되어 있지만, VGD 동물 모델 구축에 적용하면 종 간의 차이, 동물 장비 및 시설 확보, 동물 외과 수술, 동물 급여 및 간호 등이 연구자에게 큰 과제입니다. 임상 실습에서와 같이, VGD 동물 모델을 확립하기 위해 사용되는 CABG 수술에 대한 접근법은 정중선 흉골 절개술11 및 좌측 측면 개흉술12을 포함한다. 정중선 흉골 절개술이 더 일반적으로 사용됩니다13,14. 그러나 이 접근 방식은 인간과 동물 모두에게 높은 위험을 초래합니다.
