1. 动物

</ol>3. 外科手术
<ol><li>消毒并覆盖手术区域。从木骨到耻骨联合进行中线切口。用左侧切口延长手术区域，以获得更好的暴露。</li>

	<li>取回胰腺移植物和一段腔静脉或髂静脉以延伸门静脉。 注意：胰腺移植物与脾脏一起切除。</li>
	<li>将器官放在一个器官袋中，该器官袋放置在装满冰的盆中。</li>
</ol>4.胰腺移植物的背台准备（图2A）
<ol><li>从主动脉远端取下冲洗线并用扎带闭合。用剩余的UW溶液填充器官袋。将胰腺从粘附组织中解放出来，包括脾脏。</li>
	<li>使用先前恢复的腔静脉或髂静脉与6-0 Prolene进行门静脉延伸。用 1/4 英寸 x 3/8 英寸的缩小器插管门静脉和近端主动脉。</li>

	<li>记录移植物的重量。将移植物保存在静态冷库（SCS）中，直到NEVPP开始。</li>
</ol>5. 正常离 体 胰腺灌注 （NEVPP）

<ol><li>将相应的管子连接到氧合器和静脉储液器，并将动脉管连接到氧合器的流出处，并将气泡过滤器放在其支架中。连接从气泡过滤器到静脉储液器的吹扫管路。打开气泡过滤器盖，让所有空气排出。</li>
	<li>将静脉管线连接到静脉储液器的入口。连接透析过滤器和将注入透析液的管道。连接流量计传感器、压力管路和温度探头。将动脉和静脉采样管连接到样品端口。</li>
	<li>将胰腔室（图3）放在妙佑医疗国际的桌子上，并通过用于此目的的孔引入动脉和静脉管。连接并打开外部加热器单元。</li>
	<li>将吸入管放在滚轮泵内，并将一端连接到从腔室出来的管子中以收集液体，另一端连接到静脉储液器以收集所有器官丢失的灌注液。</li>

</ol>6. 灌注液的制备和电路的启动
<ol><li>用灌注液填充静脉储液器（表1）。</li>
	<li>使用一个注射泵以 8 mL/h 的速度将血管扩张剂（依前列醇）连续施用到动脉管路中。使用第二个注射泵将酶抑制剂直接连续施用到静脉储液器中（15 mg，10 mL / h）。</li>
	<li>打开心肺机 （HLM） 并启动压力、温度和计时器面板。打开加热器泵，将灌注溶液加热至38°C。 打开 O 2/CO2 电源。</li>

本研究的背台制备与人类移植物制备不同（图2A）。由于胰腺是从猪身上获得的唯一器官，我们能够取出包括乳糜泻躯干和肠系膜上动脉在内的主动脉部分。至于门静脉，使用髂静脉进行了伸展。在人类移植的情况下，背台准备必须以与移植相同的方式进行，使用髂骨移植物进行动脉重建和门静脉延长19。
此方法可能受到设置复杂性的限制。我们决定在注意到移植物在没有透析的情况下出现严重水肿后添加透析盒。还为这些实验建造了一个定制的器官室，其中包含一个外部加热源，该加热源被证明有助于移植物的最佳灌注。
很少有研究描述正常变温离 体 胰腺灌注。在大多数这些研究中，水肿似乎是主要的限制因素。据我们所知，这种方法是使用透析盒控制水肿的唯一报告。
与其他器官相比，胰腺的正常 离体 灌注仍处于起步阶段。目前的方案侧重于扩展标准供体（DCD）、灌注液改善、更长的灌注时间和生物标志物，以评估灌注过程中的移植物损伤。淀粉酶和脂肪酶水平似乎不是可靠的标志物，因为我们使用的是封闭系统，并且似乎与组织病理学无关20。到目前为止，我们小组还设法在灌注后移植胰腺同种异体移植物，效果良好18。
随着这项技术的不断改进，我们希望这项技术将适用于临床移植，并允许评估和修复胰腺同种异体移植物。这有望最终导致更多的移植物利用率，减少患者的等待时间，并改善患者的预后。

根据国家糖尿病统计报告，2019年美国共有2870万人被诊断为糖尿病。其中约有180万人被诊断为1型糖尿病1。PTx是目前最有效和唯一的治疗方法，可以治愈复杂的1型糖尿病2，并且是一种既能延长这些患者的预期寿命又能提高生活质量的手术3。
胰腺是从已故捐献者那里取回后最常丢弃的器官4.随着持续的器官短缺和等待名单时间的增加，移植中心正在使用更多来自ECD的胰腺移植物，包括循环死亡后捐赠（DCD）5。需要安全保存、灌注、评估和修复来自扩展标准供体的同种异体移植物的策略。
NEVP已被证明在保存肺6，肝脏7，8和肾脏移植物9，10方面是成功的。然而，由于移植物水肿和损伤，研究胰腺机器灌注的小组数量（低温或正常体温）以及出版物数量很少且有限11，12，13，14。
本研究的目的是提出一种使用猪模型的常温 离体 胰腺灌注（NEVPP）方案，目的是最终为移植前的长期保存，器官评估和修复提供一个平台。这将允许其他研究小组建立用于胰腺同种异体移植物研究的灌注模型。

normothermic ex vivo pancreas perfusion for the preservation of pancreas allografts before transplantation
