肝細胞がん(HCC)は、世界で6番目に診断されたがんであり、がんによる死因として3番目に多く、2020年には90万人以上の新規症例と80万人以上の死亡があります1。主な危険因子は肝硬変であり、これはウイルス感染(B型およびC型肝炎ウイルス)、アルコール乱用、糖尿病、および非アルコール性脂肪性肝炎2の結果として発生します。HCCの管理はかなり複雑であり、疾患の病期分類に応じて、外科的切除、熱的または化学的アブレーション、移植、経動脈化学塞栓術、放射線、化学療法など、いくつかの治療オプションが利用可能です2,3。HCCは化学療法に難治性の腫瘍であり、治癒目的の治療を受けた患者の最大70%に疾患が再発します2。
高度の腫瘍の不均一性にもかかわらず、HCCは2つの一般的な結果に関連しています:(i)HCCは非常に低酸素であり、(ii)腫瘍低酸素症は腫瘍の攻撃性および治療の失敗の増加に関連しています。HCC細胞の制御されない増殖は、血管新生に先行する高い酸素消費率をもたらし、したがって低酸素微小環境を作り出す。腫瘍内酸素レベルが低いと、腫瘍の攻撃性と治療反応に影響を与えるさまざまな生物学的反応が引き起こされます。低酸素誘導因子(HIF)は、低酸素症への応答において必須の転写調節因子として認識されることがよくあります2,3。したがって、低酸素症を検出する能力は、腫瘍性組織を視覚化し、侵襲的な手順を必要とするアクセスできない部位を特定するために重要です。また、腫瘍の攻撃性につながる分子変化をよりよく理解し、患者の治療結果を改善するのにも役立ちます。
陽電子放出断層撮影(PET)を使用した分子イメージングは、HCCを含む多くの癌の診断と病期分類に一般的に使用されています。特に、[18 F]フルオロデオキシグルコース([18F]FDG)と[11C]アセテートを含むデュアルトレーサーPETイメージングを組み合わせて使用すると、HCC診断における全体的な感度が大幅に向上する可能性があります4,5。一方、低酸素症のイメージングは、一般的に使用される低酸素マーカー[18 F]フルオロミソニダゾール([18F]FMISO)を使用して達成できます。臨床診療では、低酸素症の非侵襲的評価は、放射線治療計画のためにさまざまな種類の腫瘍と領域を区別するために重要です6。
前臨床イメージングは、さまざまな疾患のマウスモデルの非侵襲的および縦断的評価に不可欠なツールになっています。堅牢で再現性の高いHCCモデルは、ヒトHCCの病態生理学および新規治療法の評価に関する前臨床およびトランスレーショナル研究のための重要なプラットフォームです。PETイメージングとともに、in vivoの挙動を解明して、任意の時点で分子レベルで重要な洞察を提供することができます。ここでは、[18F]FMISOおよび[18F]FDG PET/MRを使用した肝動脈結紮(HAL)同所性HCC異種移植片の生成とそれらのin vivo腫瘍代謝の分析のためのプロトコルについて説明します。HALの組み込みは、HALが腫瘍内低酸素症を誘発するために動脈血供給を効果的に遮断することができるので、in vivoで腫瘍低酸素症を研究するためのトランスジェニックまたは化学的に誘導されたHCCマウス異種移植片の適切なモデルを作る7,8。さらに、ピモニダゾールを用いたex vivo免疫組織化学染色とは異なり、低酸素の結果としての腫瘍代謝の変化は、PETイメージングを使用して非侵襲的に容易に視覚化および正確に定量することができ、治療反応の縦断的評価または耐性の出現の測定を可能にします3,7,8.ここに示す方法は、PET/MRイメージングを用いた腫瘍低酸素の非侵襲的モニタリングとともに、堅牢な低酸素HCCモデルの作成を可能にし、in vivoでのHCC生物学を研究することができます。

ヒト疾患を再現する肝細胞癌(HCC)の前臨床実験モデルは、腫瘍形成を研究し、新しい治療アプローチを評価するための重要なツールとなります。陽電子放出断層撮影(PET)を使用した非侵襲的な全身イメージングは、組織のin vivo特性に関する重要な洞察を分子レベルでリアルタイムで提供します。ここでは、腫瘍低酸素症を誘発するための肝動脈結紮(HAL)の有無にかかわらず、同所性HCC異種移植片を作成するためのプロトコルと、[18 F]フルオロミソニダゾール([18 F]FMISO)および[18F]フルオロデオキシグルコース([18F]FDG)PET /磁気共鳴(MR)イメージングを使用したin vivoでの腫瘍代謝の評価を紹介します。腫瘍低酸素症は、低酸素マーカー[18 F]FMISOを使用して容易に視覚化でき、[18 F]FMISOの取り込みは、HALを受けたHCCマウスで非HAL群よりも高かったが、[18F]FDGは2つのグループ間で腫瘍低酸素症を区別できなかったことがわかりました。HAL腫瘍はまた、低酸素に応答してより高いレベルの低酸素誘導因子(HIF)-1α発現を示した。HAL腫瘍の定量化は、標準化値取り込み(SUV)アプローチに基づく[18F]FMISO取り込みの2.3倍の増加を示した。

私たちの方法は、腫瘍低酸素症の非侵襲的イメージングのための信頼性の高い低酸素肝細胞癌モデルを作成し、in vivoでのHCC生物学を研究し、ヒトの癌発生をよりよく再現することができます。信頼性の高いHCCモデルと再現性の高いイメージング法は、ヒトHCCの病態生理学へのトランスレーショナル研究の重要なプラットフォームとして機能します。腫瘍低酸素症のリアルタイムモニタリングは、腫瘍の攻撃性につながる分子変化に関する洞察を提供し、低酸素症の領域を特定し、治療に対する反応を予測することを可能にします。腹腔内手術は技術的に困難な場合がありますが、適切な計画と動物の世話は、全体的な手術の成功率と小動物の幸福を改善します。まず、麻酔をかけた6〜8週齢のマウスの背側領域を下脇腹の左側または右側に70%エタノールで滅菌します。次に、鉗子を使用して皮膚を持ち上げ、注射針を下にそっと挿入します。針の引き抜き中に注入された細胞懸濁液の偶発的な漏れを避けるために、皮下面に沿って穿刺部位から針を4〜5ミリメートル進めます。鉗子を解放した後、反対側を貫通せずにシリンジの内容物を慎重に排出します。皮下担癌マウスを安楽死させた後、メスの刃で腫瘍部位の皮膚を切開する。その後、腫瘍ブロックを切除し、直ちに培地に移します。次に、鋭利な外科用ハサミを使用して、腫瘍ブロックの中心領域を避けて、腫瘍ブロックを小さな1立方ミリメートルの立方体に切断します。すべてのキューブを培地に入れます。同所性肝移植の場合は、麻酔をかけたマウスの手足を伸ばし、テープで固定して、腹側の腹部と胸部を最大限に露出させます。次に、腹部の皮膚全体を10%ポビドンヨード溶液で滅菌し、続いて70%エタノールで滅菌します。次に、最初に鋭いハサミを使用して10ミリメートルの正中線切開を行い、腹膜にアクセスして開腹術を行います。次に、鉗子で剣状突起を頭に向かってクランプして引っ張り、肋骨下開創器を使用して手術野を開きます。湿ったガーゼ綿棒を使用して、正中葉と左肝葉を上向きに引っ込めます。次に、腹部の外側の腸をマウスの左側に移動し、湿ったガーゼ綿棒で覆います。次に、最適な視覚化のための拡大鏡の下で、膵臓の頭から肝茎の根元まで発生する総肝動脈に肝動脈結紮を行います。濡れたガーゼ綿棒で中央葉と左葉の背中を引っ込めて左葉を露出させます。次に、滅菌メス刃を用いて、左葉に長さ約2ミリ、深さの切開を行い、直ちに滅菌鉗子で肝切開部に腫瘍片を挿入します。腫瘍を肝臓にしっかりと埋めてから、8の字縫合糸を適用し、切開部位に6-0ナイロン縫合糸を付けて、適切な腫瘍移植と止血を確保します。次に、中断された5-0縫合で腹部切開を閉じます。18-FMISOのアリコートをチューブに分注し、滅菌生理食塩水で希釈して、100マイクロリットル中18〜20メガベクレルの総活性濃度を確保します。次に、1ミリリットルの注射器で溶液を引き出し、線量校正器に示された放射能と時間を記録します。被験者マウスの体重を記録した後、調製した放射性トレーサーを尾静脈から慎重に注入する。崩壊補正を考慮するには、注射器内の注入時間と残留活動を記録します。PETの取得を開始するには、スカウトビューを実行してスキャナー内のマウスの位置を見つけます。マウスのベッド位置を調整して、胴体領域を含むマウス体全体がMRの視野の中心になるようにします。次に、試験リストウィンドウでPET取得を選択します。スカウトビューから所定のマウスベッド位置を使用するには、[前回の収集時のスキャン範囲]を選択します。次に、[準備]をクリックしてマウスベッドをMRからPETに自動的に移動し、[移動]をクリックして取得を開始します。放射性医薬品エディターで適切な放射性ヌクレオチドを選択し、注射用量と時間の詳細を入力します。さらに、[件名情報] メニューで、マウスの重量を入力します。PETスキャンが完了したら、[準備]を選択してマウスをPETからMRに移動し、[実行]をクリックしてMR取得を実行します。すべてのスキャンが終了したら、テーブルアウトを選択してイメージングベッドをデフォルトの位置に戻します。この研究で使用されたPETイメージングは、肝動脈結紮術のあるマウスでは18-FMISOの腫瘍取り込みの増加を明らかにしましたが、対照マウスでは増加しませんでした。しかし、解糖系マーカー18F-FDGの腫瘍取り込みは、肝動脈結紮の有無にかかわらずマウスで類似していた。この標準化された取り込み値ベースのアプローチを使用した定量では、肝動脈結紮のあるマウスの腫瘍による18-FMISO取り込みが対照マウスの2.3倍高いことが明らかになりました。同様に、18-FMISOの取り込みは、対照マウスの肝臓よりも肝動脈結紮を有するマウスの肝臓で高かった。肝動脈結紮はまた、対照マウスからのものよりも肝動脈結紮を有するマウスからの腫瘍分泌物における低酸素誘導因子1-α発現のレベルが高いことによって証明されるように、腫瘍低酸素症の増強をもたらした。18-FMISO PETイメージング後、低酸素下で誘導される代謝遺伝子の役割を特定するためのシーケンシングを実行し、新しい治療戦略につながる可能性があります。この技術は、腫瘍の低酸素症を正確にモニタリングするために使用でき、それによって治療の有効性を迅速に評価し、HCCの低酸素経路を標的とする新しい治療戦略を作成することができます。
