冠状動脈疾患(CAD)は、先進国と発展途上国の両方で主要な死因である、典型的かつ代表的な心血管疾患として広く認識され、懸念されています1,2。正常な心臓生理機能のための血液および酸素供給経路として、循環血液は、心筋3,4の表面上の2つの主要な冠状動脈および血液血管網を介して心臓に入り、栄養を与える。冠状動脈のコレステロールおよび脂肪沈着は、心臓の血液供給および血管系の激しい炎症反応を遮断し、アテローム性動脈硬化症、安定狭心症、不安定狭心症、心筋梗塞、または突然の心臓死を引き起こす5,6。冠状動脈の病理学的狭窄に応答して、代償促進された生理学的心拍は、左心室7の出力を増加させることによって心臓自体または身体の重要な器官の血液供給を満たす。長期の冠状動脈狭窄が時間内に緩和されない場合、心臓の特定の領域において広範な新しい血管が発達し得る8。現在、CADの臨床治療は、しばしば薬物血栓溶解または外科的機械的血栓溶解および頻繁な投薬および大きな外科的障害を伴う外因性バイオニック血管バイパスを採用する9。したがって、冠状動脈の生理活性の機能的調査は、心血管疾患10にとって依然として緊急のブレークスルーである。
冠状動脈生理学的活性を検出するための利用可能な技術的手段は、 インビボ冠 状動脈圧、血管張力、血中酸素飽和度、およびpH値11を動的に記録できるワイヤレステレメトリシステムを除いて存在しない。したがって、冠状動脈のテクスチャーの秘密性と複雑さを考慮すると、冠状動脈の正確な同定と単離は、間違いなくin vitro4のCADの複数のメカニズムを探索するための最良の選択です。

このプロトコルは、ラット冠状動脈の局在化、分離、および筋肉環固定を詳細に紹介する。様々な心血管疾患のメカニズム、探索、および治療のための新しい方法を提供する。このシステムは、60マイクロメートルから10ミリメートルまでの直径の読み取り、インビトロ動脈張力の動的変化の安定した記録に適している。初心者はまず各血管の解剖学的位置を正確に理解し、胸部動脈や腸間膜動脈などのわずかに大きい血管から始めることができることを示唆しています。心臓を解離して除去した後、医療用プラスチック鉗子で軽く絞ることによって、すべての心臓室から残留血液を排出することから始める。前処理された心臓を、95%酸素および5%二酸化炭素飽和生理塩溶液を含む4°Cのペトリ皿に素早く置き、pH値は7.40である。冠状動脈の解剖学的位置を正確に特定するために、模式図に従って光学顕微鏡下で隔離された心臓の姿勢を調整する。肺動脈の根元から心室間中隔に沿って左右の心室腔を外科用ハサミとピンセットで切る。心筋組織から左右の冠動脈を解離させるには、光学解剖学的顕微鏡下で右心室を解剖し、右冠動脈枝を十分に露出させる。次に、心臓組織を時計回りに45度回転させることにより左冠動脈の位置を特定する。周囲の粘着性心筋組織を除去した後、脈動する左右冠状動脈を明示的に識別する。すぐに真ん中の冠状動脈を分離し、摂氏4度の生理学的塩溶液に完全に浸す。解剖はさみで剥離した動脈を垂直に切断して約2mmの動脈輪を獲得し、異なる刺激下での血管張力を記録する。2センチメートルのステンレス鋼線を2本用意し、95%の酸素と5%の二酸化炭素で飽和した摂氏4度の生理塩溶液に予め浸します。光学解剖学的顕微鏡下で血管の方向に沿って動脈リングに平行に両方のワイヤを通し、血管腔の両端に等しい長さのワイヤを露出させる。酸素95%と二酸化炭素5%のバブリング生理塩溶液で満たされたワイヤー顕微鏡写真の浴槽に鋼線の表裏で動脈リングを固定する。水平スクリューノブを回転させて、2本のワイヤが水平になり、動脈リングが自然なリラクゼーション状態になるように、適切な前後の間隔を空けます。DMTバスを恒温装置に設置した後、データ収集ソフトウェアを開いて、対応するパス信号が記録されていることを確認します。パラメータを設定します。血管の直径に沿って妥当な張力を加えることによって動脈リングの最適な初期張力を達成する。この時点で、表示された血管張力値をゼロに設定する。その後、浴の螺旋軸を回転させることにより動脈リングに3ミリニュートンパルス刺激を加える。LCDパネルをクリックしてゼロミリニュートンに設定し、1時間維持します。40°Cの酸素飽和生理塩溶液緩衝液中で1時間インキュベートした後、ワイヤー顕微鏡写真の張力コントロールパネル上で再び張力値をゼロミリニュートンに設定した。ワイヤー筋電図技術を用いて冠状動脈リングの収縮活性を行い、60ミリモルのカリウムイオン溶液でそれぞれ10分間刺激することによって3つの別々の操作で検証する。各刺激の後、血管緊張が初期状態に戻るまで、酸素飽和生理塩溶液で浴を洗い流す。0.3マイクロモルU46619を浴に加え、10分間観察する。1つの微小モルアピゲニンを加え、血管効果を観察する。拡張がプラトーに達したら、次の濃度を加えます。60ミリモルのカリウム前収縮溶液に1マイクロモルのアピゲニンを加え、血管効果を観察した。拡張がプラトーに達したら、次の濃度を加えます。28ミリモルのカリウムイオンを休止血管輪に加える。収縮がプラトーに達したら、次の濃度を追加します。0.01マイクロモルU46619を静止血管環に加える。収縮がプラトーに達したら、次の濃度を追加します。動脈リングに最初の3ミリニュートンの張力を加えた後、60ミリモルのカリウムイオン濃度を3回連続してかけることで、その張力は2ミリニュートン以上を超えた。したがって、上記の手順は、優れた生理活性を有する単離された冠状動脈環をもたらした。累積カリウムイオン濃度、またはU46619をDMT620Mの浴に添加し、インビトロ血管緊張の濃度依存的な増加をもたらす。カリウムイオンまたはU46619の次の濃度は、血管収縮効果がプラトーに達したときに添加した。カリウムイオンおよびU46619によって狭窄した単離された冠状動脈環の場合、試験薬中のアピゲニンは、驚くべきことに濃度依存的に血管拡張を引き起こした。心臓は肺動脈の根元から心室中隔に沿って解剖され、2ミリメートルの冠状動脈筋輪を露出させ、分離する。この手順に従って、網膜微小血管、腎機能、腎動脈、および中大脳動脈の血管張力を記録することができます。60 マイクロメートルから 10 ミリメートルまでの動的領域。
