胸部大動脈瘤および解離(TAAD)は、高い罹患率および死亡率に関連する大動脈壁の局所的な拡張または層間剥離です1。ヒト大動脈平滑筋細胞(HASMC)は、TAADの病因において重要な役割を果たしています。HASMCは終末分化細胞ではなく、HASMCは高い可塑性を保持しているため、異なる刺激に応答して表現型を切り替えることができます2。HASMCは主に生体内でリズミカルな引張ひずみを受けており、これは平滑筋の形態学的変化、分化および生理学的機能を調節する重要な因子の1つです3,4。したがって、HASMCの研究における周期的ひずみの役割は無視できません。しかし、従来の2D細胞培養では、HASMCが経験する環状株の生体力学的刺激をin vivoで再現することはできません。さらに、動物TAADモデルの構築は、二尖大動脈弁(BAV)関連のTAADなど、一部のタイプのTAADには適していません。さらに、臨床ヒト研究と動物実験の間の種のギャップは無視できません。それは臨床現場での医薬品翻訳を妨げます。したがって、大動脈疾患の研究において、in vivo生体力学的環境をシミュレートするためのより複雑で生理学的なシステムが緊急に必要とされています。

従来の二次元細胞培養技術および動物モデルは、ヒト胸部大動脈瘤および解離(TAAD)の研究に使用されてきた。しかし、ヒトTAADは動物モデルによって特徴付けられないことがある。臨床ヒト試験と動物実験の間には、治療薬の発見を妨げる可能性のある明らかな種のギャップがあります。対照的に、従来の細胞培養モデルは、 in vivo の生体力学的刺激をシミュレートすることができません。この目的のために、微細加工およびマイクロ流体技術は近年大きく発展しており、生体力学的微小環境を再現するオルガノイドオンチップモデルを確立するための新しい技術を提供しています。本研究では、ヒト大動脈平滑筋細胞オルガンオンチップ(HASMC-OOC)モデルを開発し、TAADで重要な役割を果たすヒト大動脈平滑筋細胞(HASMC)が経験する周期的ひずみの振幅と頻度を含む、大動脈バイオメカニクスの病態生理学的パラメータをシミュレートしました。このモデルでは、HASMCの形態は形状が細長く、ひずみ方向に対して垂直に整列し、静的な従来の条件下よりもひずみ条件下でより収縮的な表現型を示しました。これは、天然のヒト大動脈壁における細胞の配向および表現型と一致していた。さらに、二尖弁大動脈弁関連TAAD(BAV-TAAD)および三尖弁大動脈弁関連TAAD(TAV-TAAD)患者由来の原発性HASMCを用いて、TAADのHASMC特性を再現するBAV-TAADおよびTAV-TAAD疾患モデルを確立しました。HASMC-OOCモデルは、TAADの病因をさらに探索し、治療標的を発見するための動物モデルを補完する新しい in vitro プラットフォームを提供します。

このモデルは、ヒト大動脈の平滑筋細胞が経験する生体力学的刺激を刺激する。患者由来の細胞と組み合わせることで、大動脈疾患の薬物スクリーニングや個別化医療に活用できます。このモデルは、ヒト大動脈の平滑筋細胞の裸の機械的パラメータを模倣し、正確に制御することができ、大動脈疾患の研究と病因のための新しいin vitroプラットフォームを提供します。手順を実演するのは、私たちの研究室のポスタードクターであるミエラディリジャンアブドゥバットです。上行大動脈の右側領域を滅菌PBSで1〜2回洗浄することから始めます。2つの眼科用鉗子で組織の内膜層と外膜層を取り除き、培地層を保持して細胞を採取します。メディア層を10センチの皿に置き、2〜3ミリメートルに切ります。次に、10%fbsと1%ペニシリンストレプトマイシンを含む5ミリリットルのSMCMを組織サンプルに追加します。滅菌スポイトで小さな組織を培養ボトルに移動し、組織を均等に広げます。培養液はできるだけ廃棄してください。次に、ボトルを逆さまにします。倒立培養ボトルに2ミリリットルのSMCMを加え、5%二酸化炭素を入れたインキュベーターに摂氏37度で1〜2時間入れます。次に、ゆっくりと右側を上にして、さらに2ミリリットルのSMCMを追加します。5〜7日間のインキュベーション後、SMCMを4ミリリットルの新鮮なSMCMと交換します。メディアを交換するときは、SMCMをゆっくりと破棄して追加してください。一般的に、散発的な平滑筋細胞は約2週間で上昇します。顕微鏡ステーションに移動するときは、培養ボトルをそっと運びます。さもなければ、組織は浮遊し、細胞はゆっくり成長するでしょう。細胞が成長したら、4ミリリットルの新鮮なSMCMを入れた2つの新しい培養ボトルに分けます。平滑筋細胞の4つの特異的マーカーの免疫蛍光分析により細胞を同定します。PDMSを重合するには、塩基またはA成分に硬化剤またはB成分を加え、複合体を5分間混合します。量は研究の必要性によって異なります。調製したPDMSゲルを真空抽出タンクに30〜60分間入れ、圧力をマイナス0.8ミリパスカル以下に維持します。コンピュータ支援設計ソフトウェアを使用して、金型を設計します。高精度のコンピューター数値制御彫刻機を使用して、3層の金型をカスタム作成します。PMMAプレートを使用して金型のフレームとマイクロチャネルを切り開いた後、それらを別のプレートに接着します。準備したPDMSゲルを設計型の型に注ぎます。その後、摂氏70度で1〜2時間架橋します。架橋PDMSスラブを金型から剥がし、製品化したPDMSメンブレンを100ミリメートル×40ミリメートルの切片に切断します。1ミリメートルの穴あけ機を使用して3つのPDMスラブに穴を開け、PDMSチップ上の空気と中程度のマイクロチャネルの入口と出口を作ります。準備した3枚のPDMSスラブと2枚のPDMSメンブレンを酸素プラズマで5分間処理し、PDMの表面を活性化します。ガスを処理する室内空気、圧力を負の100キロパスカル、電流を180〜200ミリアンペアピア、電圧を200ボルト、処理時間を5分に設定します。3つのPDMSスラブと2つのPDMSメンブレンを実体顕微鏡で接着し、最上層が空気チャネルPDMSスラブで構成され、次にPDMSメンブレンで構成されるようにします。媒体チャネルはPDMSスラブ、次にPDM膜で構成され、最下層は空気チャネルPDMSスラブです。組み立てたPDMSチップを摂氏70度のインキュベーターに1時間入れます。内径1ミリ、長さ3センチのラテックスホースを数本用意します。用意したホースの一端に外径1ミリ、長さ1センチのステンレス針を差し込みます。次に、ホースのもう一方の端にルアーを挿入して、PDMSチップの空気および中程度のマイクロチャネルに接続されたチューブを作成します。準備したチューブをPDMSチップ上の空気および中程度のマイクロチャネルの出口と入口に挿入します。2ミリリットルのシリンジを使用して、マウスコラーゲン1ミリリットルあたり80ミリグラムをPDMsチップの培地マイクロチャネルに注入し、室温で30分間インキュベートします。インキュベーション後、2ミリリットルの注射器でチャネルからコラーゲンを取り除きます。コラーゲンコード化されたPDMSチップを摂氏60度のインキュベーターに一晩入れます。PDMSチップをUV滅菌器に1時間以上入れます。滅菌したPDMSチップを超クリーンベンチに置き、細胞実験の準備をします。2%FBSおよび1%ペニシリンストレプトマイシンを含むSMCMを用いた患者由来の5つの初代ヒト大動脈平滑筋細胞を摂氏37度に設定した5%二酸化炭素インキュベーター内で10〜5回培養する。平滑筋細胞密度が80%に達したらSMCMを廃棄し、2ミリリットルのPPSで細胞を洗浄する。1ミリリットルの0.25%トリプシンを使用して細胞を2分間消化し、100 RCFで5分間遠心分離します。上清を除去し、細胞ペレットを1ミリリットルの新鮮なSMCMに再懸濁します。8ミリリットルのSMCMを使用して、10センチメートルの培養皿で細胞を培養します。サイトメーターを用いて、細胞数を算出し、1ミリリットル当たり10〜5細胞の2倍の最終濃度で進める。2ミリリットルのPBSをコラーゲンコード化および滅菌されたPDMSチップ培地マイクロチャネルにゆっくりと注ぎ、後で2ミリリットルのシリンジを使用して廃棄します。2ミリリットルの2ミリリットルを10〜5ミリリットルの平滑筋細胞懸濁液をPDMSチップの培地マイクロチャネルにゆっくりと注ぐ。次に、PDMSチップの入口と出口にあるルアーを閉じます。PDMSチップを摂氏37度に設定したインキュベーターに5%二酸化炭素で1日置きます。インキュベーション後、細胞がPDMSチップ内のPDMS膜に付着したら、PDMSチップ上の空気マイクロチャネルの出口を真空コントローラシステムに接続します。細胞が付着すると、細長い正常な細胞形態が顕微鏡下で見られます。吊り下げられた丸い細胞とは対照的です。電磁弁と真空ポンプをオンにします。真空レギュレーターを開き、ひずみに応じて圧力レベルを調整します。次に、PDMSチップと摂氏37度に設定したインキュベーターを5%二酸化炭素で24時間置きます。いくつかの電磁弁、真空フィルター、真空レギュレーター、真空ポンプ、蠕動ポンプ、および電磁弁を制御するPLCを準備します。PLCコントローラをプログラムし、オンオフ時間間隔を1ヘルツに設定します。電磁弁をプログラムされたコントローラに接続します。真空ポンプの入口を真空フィルターに接続し、次に真空フィルターの出口を真空レギュレーターに接続します。真空レギュレーターの出口をソレノイドバルブに接続します。そして最後に、電磁弁の出口をPDMSチップの空気マイクロチャネルの出口に接続します。蠕動ポンプの出口をPDMSチップの培地マイクロチャネルの入口に接続し、蠕動ポンプの入口を培地マイクロチャネルPDMSチップの出口に接続して、培地の交換と薬物処理を行います。レギュレーターによるひずみの振幅とマイクロコントローラによるひずみ周波数を調整します。ヒト大動脈平滑筋細胞株CRL 1990の生存率は、PDMSチップにおいて、細胞培養の3日目および5日目に生死アッセイを用いて推定した。細胞生存率は、3日目と5日目に90%を超えることがわかりました。CRL 1990の細胞骨格Fアクチン染色は、PDMSチップにおいて、24時間延伸した後に正常な細胞形態および細胞が株に対して垂直に整列したことを示す。収縮マーカーSM22およびCNN1の免疫蛍光は、静的条件と比較してひずみ条件下で高かった。また、SM22およびCNN1遺伝子を平滑筋細胞のひずみ条件下でアップレギュレーションした。BAVおよびTAVのFアクチン染色では、胸部大動脈瘤および患者由来初代ヒト大動脈平滑筋細胞の解離、24時間延伸後に正常な細胞形態および細胞がひずみ方向に対して垂直に整列した状態を示した。BAVTAADおよびTAV胸部大動脈瘤および解離におけるSM22およびCNN1蛍光は、静的条件下よりもひずみ条件下で高かった。SM22およびCNN1の遺伝子発現レベルと同様に、静的条件と比較して株条件でもアップレギュレーションされました。一次HASMCを単離する際の無菌性の原則に注意を払うべきである。PDMSラボと膜をプラズマで処理した後は、表面を汚さないでください。このプロトコルに基づいて、内皮細胞を平滑筋細胞と共培養し、周期的なひずみを持つ細胞に純粋なストレスを与えて、より機械的な力をさらにシミュレートすることができます。
