世界保健機関(WHO)によると、世界で10億人以上が難聴のリスクにさらされており、2050年までに4人に1人が難聴に苦しむと推定されています1。過去20年間、CIは、永続的な重度で重度の感音難聴(SNHL)を持つ人々にとって最も効果的な介入でした。CIは、音の物理的信号を生体電気信号に変換し、有毛細胞をバイパスしてらせん状神経節ニューロン(SGN)を刺激します。時間の経過とともに、CIの適応症は拡大され、現在では残存聴、片側性難聴、および非常に高齢者または若年者が含まれるようになりました2,3,4。一方、完全に埋め込み可能なCIと高度なアレイが開発されました5。しかし、CIを用いて内耳の電気生理学および組織病理学を調査するための経済的に実現可能な大型動物モデルはありません。この大型動物モデルの欠如は、CIを改善し、内耳に対するCIの電気生理学的影響についての洞察を得ようとする研究を制限します。
CI研究では、マウス6、スナネズミ7、ラット8、モルモット9など、いくつかのげっ歯類動物モデルが適用されています。しかし、形態や電気生理学的応答の特徴はヒトとは異なります。ネコ、モルモット、その他の動物など、CI研究に伝統的に使用されている動物モデルの蝸牛構造は、ヒトの蝸牛構造のそれとは大きく異なります10。アレイ挿入はネコ11とウサギ12で行われてきましたが、蝸牛が小さいため、これは人間で使用するように設計されていないアレイで行われました。CIのためにいくつかの大型動物モデルも調査されています。子羊は非外傷性人工内耳のトレーニングモデルとして適していますが、蝸牛のサイズが小さいため、完全なアレイ挿入が不可能になります13。霊長類は、人間と解剖学的に類似しているため、CI研究に最も適した動物である可能性があります14,15;しかし、サルの性的成熟は遅れ(4〜5年)、妊娠期間は最大約165日であり、各女性は通常、年間16人の子孫しか産みません。これらの理由と高価なコストは、CI研究における霊長類の広範な適用を妨げています。

人工内耳(CI)は、重度から重度の感音難聴の人を治療するための最も効果的な方法です。CIは世界中で使用されていますが、CIを持つ患者または動物モデルの電気生理学および組織病理学を調査したり、電極アレイの新しいモデルを評価したりするための標準モデルは存在しません。ヒトと同様の蝸牛特性を有する大型動物モデルは、ヒトで使用する前に、高度および改変アレイの研究および評価プラットフォームを提供する可能性がある。
