変形性関節症(OA)のような関節疾患は非常に蔓延しており、衰弱させ、世界中の障害の主な原因となっています1。米国だけでも、OAは2,700万人の患者に影響を及ぼし、60歳以上の成人の12.1%に発生していると推定されています2。残念ながら、現在関節疾患の管理に使用されているほとんどの薬は緩和薬であり、効果的な疾患修飾OA薬(DMOAD)は入手できません3。この満たされていない医療ニーズは、主に、疾患メカニズムを研究し、潜在的なDMOADを開発するための効果的なモデルがないことに起因しています。従来の2次元(2D)細胞培養は、関節組織の3D特性を反映しておらず、組織外植片の培養は、著しい細胞死によって妨げられることが多く、通常、動的な組織相互接続を再現できません4。さらに、遺伝的および解剖学的差異は、動物モデルの生理学的関連性を有意に低下させる4。

臨床試験中に人間が薬物にどのように反応するかを正確に予測する能力は、生理学的に関連する前臨床モデルがないために制限されています。この微生理学的システムは、その格差に対処することができます。このヒト細胞由来システムは、ヒト膝の変形性関節症の関節疾患の性質全体を正確に模倣しているため、臨床試験の前に疾患修飾変形性関節症薬の試験が可能になり、数百万ドルを節約できる可能性があります。500ミリリットルの蒸留水に17グラムのゼラチンタイプBを加えて、メタクリレートゼラチンまたはGelMAの調製を開始します。摂氏37度のシェーカーで30分間混合し、シェーカーで一晩混合する前に13ミリリットルの無水メタクリル酸塩を加えます。翌日、約60ミリリットルのGelMAのアリコートを透析バッグに調製します。すべての透析バッグを攪拌子付きの蒸留水に7日間入れます。一日に複数回水を交換し、バッグを摂氏4度で一晩放置します。7日目に、GelMAを皿に注ぎ、凍結乾燥の前に摂氏マイナス80度で凍結します。次いで、GelMAを凍結乾燥機の真空チャンバーに入れて凍結乾燥する。凍結乾燥機から取り出す前に、完全に乾燥させてください。次に、凍結乾燥GelMAを15%濃度のハンクスバランス塩溶液に溶解し、少量の水酸化ナトリウムを使用してpHを7.4に調整します。取得した量に基づいて、抗生物質抗真菌剤と0.15%フェニルホスフィン酸リチウム、またはLAPを溶液に補給します。GelMA溶液を光から保護し、さらに使用するまで摂氏マイナス20度で保管してください。オートクレーブ3Dプリントされたデュアルフローバイオリアクターチャンバー、蓋、インサートをオートクレーブバッグに摂氏121度で蒸気で20分間入れます。そして、乾熱で20分間。滅菌後、バイオリアクターチャンバー、蓋、インサートをバイオセーフティキャビネット内の15ミリリットルの滅菌PBSに一晩浸した後、完全に乾燥させます。1000マイクロリットルのピペットを用いて、15%GelMA溶液に1ミリリットル細胞あたり6番目のヒト骨髄由来間葉系幹細胞(MSC)を10回20回再懸濁する。次に、滅菌手袋を使用して、滅菌済みの乾燥したシリコン型をペトリ皿に押し付けます。インサートの穴側を下に向けて、鉗子でシリコンモールドの各穴に1つのインサートを入れます。完了したら、200マイクロリットルのピペットを使用して、インサートごとに約50マイクロリットルの細胞懸濁液を追加します。上部を波長395ナノメートルのUV懐中電灯に1.5分間さらし、次に反対側を30秒間照らして、インサート内のゲルを架橋します。滅菌鉗子を使用して、インサートを非組織培養6ウェルプレート内の8ミリリットルの増殖培地に直ちに移し、細胞を一晩回復させます。脂肪組織の分化を開始するには、インサートを8ミリリットルの脂肪生成培地に移し、毎日培地を交換しながら28日間培養します。骨軟骨ユニットを設計するには、インサートをデュアルフローバイオリアクターチャンバーに配置し、ウェルに蓋をし、35ミリリットルの骨形成培地と35ミリリットルの軟骨形成培地を毎分5マイクロリットルの流速で2つのストリームに別々に注入します。それぞれの培地タイプで隔週のシリンジ交換を行うことにより、分化のために細胞を28日間維持します。線維芽細胞を誘導するには、20ミリリットルの線維形成培地を入れたT150平方センチメートルの組織細胞培養フラスコ中で21日間2D培養でMSCを分化させる。メディアの毎週の変更を維持します。21日後、4ミリリットルのトリプシンを使用して細胞を剥離します。インサート内に3Dゲルをカプセル化します。滑膜様線維組織(SFT)を得るには、同様のプロトコルに従う。内径0.062インチ、外径0.125インチのシリコンチューブを、一方の端のミニジョイントバイオリアクターバーともう一方の端のF 1/16ルアーロックコネクタに接続します。次に、3Dミニジョイントバイオリアクターをシリコンチューブおよび蓋と共にオートクレーブする。ミニジョイント培養に使用する培地35ミリリットルを準備し、培地リザーバーに入れます。ストレート鉗子を使用して、骨軟骨ユニットをデュアルフローバイオリアクターからミニジョイントバイオリアクターの右側のウェルに移します。脂肪組織インサートと線維性組織インサートをそれぞれ左と中央のウェルに移してから、すべてのウェルを滅菌した蓋で覆います。ミニジョイントクリップの入口を培地リザーバーに接続し、出口をシリンジに接続します。次に、シリンジをシリンジポンプに取り付けます。ポンプとチップをインキュベーターに移します。培地リザーバーをインキュベーターの外側の氷の上に置きます。ポンプを引き出しモードで操作し、培地リザーバーからミニジョイントバイオリアクターチャンバーに培地を引き込みます。このミニジョイント培養プロセスを28日間続けます。関節の炎症と軟骨変性をモデル化するには、インターロイキン1ベータを1ミリリットルあたり10ナノグラムで7日間線維形成培地株に追加します。線維形成培地に新鮮なインターロイキン1ベータを供給する前に、3日目にシリンジを必ず交換してください。薬物試験のために、インターロイキン一ベータ治療の3日後、関節内投与を模擬する共有培地中、または全身投与を模倣する全ての培地型のいずれかで薬物を投与する。インターロイキン1ベータ治療の7日後、分析のために個々の組織を採取する。滅菌鉗子を使用してインサートを取り外した後、生検パンチをインサートの中央に押し込んでゲルを除去し、ゲルをPBSに入れます。次に、遺伝子発現を評価しながら骨軟骨ゲルを半分に切断します。各培地源から約1.5ミリリットルの培地を14, 000 Gで10分間収集して遠心分離します。沈殿物を廃棄した後、液体窒素中で馴化培地を瞬間凍結する。組織学的染色および免疫染色では、まず骨軟骨および滑膜様線維組織またはSFTサンプルを10%緩衝ホルマリンで固定し、それらを上昇濃度のエタノールで脱水します。サンプルをキシレンで透明にし、パラフィンに埋め込み、最後にサンプルを6マイクロメートルの厚さに切片化します。脂肪組織の場合は、10%緩衝したフォーマルサンプルと固定サンプルをオイルレッドO溶液またはBODIPYで直接染色します。組織特異的表現型は、個々の微小組織について良好に維持された。ミニジョイントのすべての組織を収集し、28日間の培養後にそれらの表現型を分析しました。骨軟骨ユニットの微小組織の骨成分は、高レベルのオステオカルシンを発現した。骨軟骨の軟骨部分では、組織は2型コラーゲンおよびアグリカンの有意に高レベルを発現した。代表的な脂肪遺伝子であるアディポネクチン、およびレプチンの発現レベルは、脂肪組織で高かった。カルシウムミネラル、アルカリホスファターゼ、およびオステオカルシンタンパク質の沈着は、主に骨軟骨微小組織の骨サイズに見られました。骨軟骨組織の軟骨側は、グリコサミノグリカンおよびコラーゲン2型が良好に保持された。軟骨細胞肥大関連マーカーであるコラーゲンタイプ10およびインドハリネズミの発現は、培養28日後に有意にダウンレギュレーションされた。脂肪組織に豊富な脂肪滴沈着が見られました。免疫蛍光染色は、4週間の培養後にSFTによる堅牢なルブリシンおよびカドヘリン11発現を示した。疾患モデルにおいて、IL一ベータ処置は、SFTにおける細胞アポトーシスおよびMMP-13レベルの上昇をもたらした。軟骨分解はIL一βで治療され、骨軟骨組織とSFTとのクロストークの発生が示唆された。全身投与によるナプロキセンの治療効果は、IL一ベータミニジョイントにおける軟骨分解の減少を示した。リアルタイムPCRは、4つの潜在的な疾患修飾変形性関節症薬が軟骨喪失を部分的に逆転させることを示しました。この手順の最も重要なステップには、インサート内のゲルの架橋とバイオリアクターへのインサートの配置が含まれます。このチップにより、肥満や機械的損傷など、変形性関節症で提供される他の危険因子の寄与を調査できます。また、他の関節疾患の研究にも使用できます。
