1. 患者准备
<ol><li>讨论该手术的益处和风险，以及并发症的风险，例如需要输血的失控出血、转为剖腹手术、术后感染和 POP 复发。</li>
	<li>根据先前的报告，使用全身麻醉和气管插管进行手术7.在诱导麻醉前30分钟向每位患者施用预防性抗生素，例如静脉注射头孢呋辛（1.5g，见 材料表）。</li>
	<li>根据医院方案，使用依诺肝素钠注射液（4，000 AxaIU，见 材料表）开始围手术期血栓形成预防。</li>
</ol>2. LNMCP前期准备
<ol><li>皮肤灭菌后，使用标准技术7建立端口（见材料表）：一个10毫米的脐带端口，右侧一个5毫米的端口，以及下腹部左侧的两个左侧端口，每个5毫米。</li>
	<li>在手术过程中，在麻醉师的帮助下仔细监测患者的生命体征。 注意：在麻醉期间，不断评估患者的氧合、通气、循环和体温。</li>
	<li>保持腹内压 12 mmHg，气体流速 20 L/min7。</li>
</ol>3. LNMCP程序

有几种程序可用于恢复顶端支撑。腹腔镜骶骨阴道固定术是根尖脱垂的主要治疗方法，成功率高，经久耐用3。然而，网状物桥接了携带骶骨和阴道的骨盆空间，减少和限制了结肠的运动，导致排便时疼痛13。腹腔镜胸腔固定术可代替骶骨阴道固定术治疗根尖脱垂。该技术需要使用髂外侧韧带的外侧组件来修复脱垂的组织。Pectop固定术通过确保直肠的盆腔空间不减少来减少排便障碍的频率13。
然而，这两种手术都使用合成网片作为支撑，导致膀胱、尿路和肠道网状穿孔、感染和盆腔疼痛等并发症5。此外，合成网价格昂贵，限制了其在资源有限的地区的应用。因此，我们开发了一种称为LNMCP的非网状悬浮程序。
LNMCP基于以下主要功能：
子宫颈的保存 子宫颈是骨盆悬吊系统的关键结构。它为主韧带和子宫骶韧带提供附着。宫颈保留优选以增加顶端区域固定的稳定性14。在宫颈上子宫切除术中，保留一部分子宫下段会导致形成蘑菇状的宫颈残端，并将宫颈环扎牢固地固定在子宫颈上。
髂外韧带的识别 髂外韧带从与圆形韧带表面相邻的手术切口开始制备，该切口位于髂外血管和解剖学上闭孔神经旁边15。切除该区域的软组织以露出髂外韧带，而不会撕裂电晕榫，这是髂外静脉和闭孔静脉之间的吻合口16。
宫颈环扎术 宫颈峡部周围的环形缝合线确保宫颈环扎牢固地固定在子宫颈上，并防止颈残端撕裂。
缝合线嵌入圆形韧带 悬吊在颈残端和髂外韧带之间产生明显的张力。具有高拉伸强度的裸缝线可能会损害邻近器官，例如肠道和膀胱。因此，嵌入周围韧带的缝合线可以防止这种并发症。
无张力悬架 子宫颈升高到 POP-Q 阶段 0，以避免过度矫正并确保无张力锚固。
这项试点研究提出了一种称为LNMCP的新程序，仅使用缝合线来治疗根尖脱垂。LNMCP表现出优异的安全性、有效性和可行性。目前的数据表明结果良好，估计的失血量、手术时间以及成功的客观和主观参数与腹腔镜胸腺固定术13 期间获得的参数相当。此外，LNMCP避免使用合成网，从而消除了相关并发症的任何风险并减少了患者的医疗费用。因此，即使在资源有限且无法使用合成网格的区域也很容易执行。
然而，这项试点研究涉及少量病例，并报告短期结果，这是一个局限性。需要涉及大型病例系列和广泛随访的其他研究来证实令人鼓舞的结果。

盆腔器官脱垂 （POP） 主要见于女性，尤其是有多个孩子的女性。POP对患者的生活质量产生不利影响和显着影响1。根尖支撑对于确保盆腔器官的整体健康至关重要。然而，顶端支撑的丧失往往被忽视。一旦子宫颈下降到 ≥-3 cm 或阴道袖带下降到 ≥-4 cm，必须考虑根尖缺损2，并且需要进行根尖支撑手术以解决 POP。
骶骨阴道固定术是根尖脱垂的金标准治疗3.然而，骶骨阴道固定术与出口梗阻引起的 新排 便障碍有关4.眼前固定术是一种新的根尖修复技术，在临床疗效方面与骶骨阴道固定术相似，排便障碍极少4。在骶骨阴道固定术和骶骨固定术中使用合成网片进行顶端支撑提高了成功率5。
然而，植入合成网片与膀胱、尿路和肠道穿孔等并发症有关，导致感染和疼痛6。美国食品和药物管理局 （FDA） 于 2019 年 4 月禁止生产用于脱垂的经阴道网片，这表明即将限制在 POP6 的腹部修复中使用网片。因此，此类患者需要其他泌尿妇科管理选择。
本研究报告了腹腔镜非网片环扎胸肌固定术（LNMCP）在不使用合成网片的情况下用于根尖脱垂治疗的发展，从而消除了网片相关并发症的风险。

在过去的一年中，LNMCP在16名患者中成功进行了。计算出的平均年龄为58.5岁（SD 8.0）。所有妇女都同意在平均6.6个月后到医院进行随访（SD 3.8）。平均估计失血量为73.1毫升（SD 51.1）。平均手术时间为67.8分钟（SD 15.5）。没有并发症。只有一名患者偶尔主诉轻微的下腹痛。结果显示，在最后一次随访之前，客观成功率为100%，而主观成功率为93.8%（表1）。
