reduced procedure time and variability with active esophageal cooling during radiofrequency ablation for atrial fibrillation

환자 특성 이 분석에서는 2020 년 1 월부터 2022 년 1 월 사이에 고주파 PVI를받은 총 164 명의 환자가 확인되었습니다. 환자는 PVI 만 받았는지 또는 지붕 선, 바닥 선, 승모판 협부 선 등과 같은 추가 병변을 받았는지 여부에 관계없이 포함되었습니다. LET 모니터링은 단일 센서 온도 프로브로 수행되었으며 능동 냉각이 있는 경우와 동일한 팀 및 동일한 실험실에서 수행되었습니다. 연구 기간 동안 PVI에 대한 LET 모니터링을받은 환자는 63 명, 식도 보호를 위해 능동 식도 냉각을받은 환자는 101 명이었다. 두 그룹 모두에서 비슷한 비율의 AF 유형이 있었습니다(표 1).
절차 기간 및 절차 변동성 절차 기간은 첫 번째 외피가 배치된 시점부터 마지막 외피 제거 시간까지의 시간으로 정의되었다. LET 모니터링을받은 환자의 평균 시술 시간은 176 분± 52 분이었다. 능동 냉각 그룹에서, 평균 절차 시간은 156분 ± 40분이었고, 이는 절차 기간의 전체 20분 감소를 나타낸다(P =0.012). 절차 시간의 중앙값은 LET 모니터링 그룹에서 172분(사분위수 범위[IQR] = 144 - 198)이었고, 151분(IQR = 129 - 178; P = 0.025)에서 능동 식도 냉각기. 전체적으로 중앙값은 21분 감소했습니다(그림 1). 연산자의 차이 외에는 사용된 식도 보호 유형 외에 그룹 간에 다른 요인이 없었습니다. 따라서 절차 기간의 차이는 전적으로 LET 모니터링에 필요한 일시 중지, 온도 상승에 대한 반응, 폐정맥 주위를 절제하는 동안 반복적으로 위치를 변경해야 할 필요성 때문인 것으로 생각됩니다. 이 임상 부위에서 장기 효능 분석이 아직 수행되지는 않았지만 다른 곳의 데이터는 LET 모니터링과 비교할 때 냉각 시 개선된 효능을 보여주었습니다. 이는 국소 과열 경보의 중단 없이 완료될 수 있는 개선된 점대점 병변 시퀀싱 때문인 것으로 여겨집니다.
