갈색 지방 조직(BAT)은 백색 지방 조직(WAT)과 달리 저장하지 않고 에너지를 소비합니다. 교감신경 자극 시 BAT는 지방산과 포도당을 이용하고 분리 단백질 1(UCP1)의 활성화에 의해 열을 생성합니다. UCP1의 기능은 두 개의 미토콘드리아 막 사이에 H+ 구배를 사용하여 ATP 대신 열을 생성하는 것입니다. BAT의 기능은 추운 조건에서 열 생산을 증가시켜 에너지 소비를 증가시키는 것입니다1. 추위에 노출된 후 피부의 감각 입력은 시상하부 전광학 영역(POA)의 중앙 전시(MnPO) 핵에서 따뜻하고 민감한 뉴런을 억제하여 봉선(rRPa)에 대한 POA 뉴런의 억제 효과를 감소시킵니다. rRPa 뉴런의 활성화는 교감신경 활동을 증가시키고, 그 다음에는 BAT 활성이 증가한다 2,3. 감기에 의한 박쥐 박쥐(BAT) 활성화는 인간의 인슐린 감수성을 향상시키며4, 체질량 지수(BMI, body mass index, BMI)가 증가하고 연령이 1,5,6,7 이상인 사람에게서 인슐린 감수성이 감소한다.
감기 유발 열 발생에서의 역할 외에도 식사 후 BAT의 포도당 흡수는 마른 남성 인구에서 증가하여 식이 유도 열 발생(DIT)에 기여하며, 이는 BAT 양성 남성 피험자에서 더 높습니다 8,9. BAT 활성을 측정하는 데 사용되는 최첨단 기술은 PET-CT로 알려진 양전자 방출 단층 촬영 컴퓨터 단층 촬영입니다. 이 방법은 방사성 추적자 플루오로데옥시글루코스(18F-FDG)의 축적을 측정하여 BAT 활성을 결정합니다. 그러나 PET-CT는 식사 후 BAT의 활성화를 감지하기 위해 선택하는 방법으로 실패합니다. 그 이유 중 하나는 식사 후 18F-FDG가 동일한 포도당 수송체에 대해 식후 고혈당증과 경쟁하기 때문에 특히 건강한 참가자와 당뇨병 참가자의 BAT 활성을 혈당 농도의 가능한 차이와 비교할 때 식사 후 BAT 활성화를 결정하는 데 적합하지 않기 때문입니다. 또한 BAT는 지방산을 열 생산을 위한 에너지원으로 사용하는데, 이는 PET-CT에서는 볼 수 없습니다. 18 식사 후 BAT에 F-FDG가 축적되는 것은 거의 눈에 띄지 않으며10 따라서 대부분의 경우 음성 결과로 해석된다. 놀랍지 않게도, 최근에, BAT의 활성화는 우리가 이전에 생각했던 것보다 인간 인구에서 더 두드러진다는 것이 제안되었습니다. 따라서 BAT 활동과 대사 장애에 대한 관련성을 감지하는 새로운 접근법이 필요합니다7. 이러한 문제를 해결하기 위한 시도는 당뇨병 전단계 환자와 인슐린 저항성이 있는 제2형 당뇨병(T2DM) 환자에서 자기공명영상(MRI)으로 BAT의 부피를 측정하는 것이다11. 그러나 MRI로 측정한 BAT 부피는 BAT에 의한 포도당과 지방산의 일상적인 기능과 사용량을 추정하기에 충분한 지표가 아닙니다. 따라서 건강한 환자와 T2DM 환자에서 BAT 활성의 실제 차이를 추정하기 위해서는 T2DM 환자에서 BAT 오작동의 병리학적 메커니즘을 찾을 수 있는 가능성을 제공하는 새로운 접근법이 필요합니다.
BAT의 활성화를 확인하기 위해 적외선(IR) 서모그래피를 사용하여 식사 전후에 BAT 열 생성을 측정했습니다(그림 1)12,13. 건강하고 비만인 개인 또는 당뇨병 환자의 식사 후 BAT 활성을 측정하기 위한 선택 방법으로 IR 열화상 촬영을 설정하는 것은 현장에 큰 영향을 미칠 것입니다. 오늘날까지 IR 열화상 촬영은 BAT13,14,15의 저온 유도 활성화를 측정하는 데 사용됩니다. 최근 인류 역사에서 감기로 인한 BAT 활동은 더 이상 두드러지지 않는 반면(서식지의 적절한 난방, 적절한 의복으로 인해) 식사 후 BAT 활성화는 매일 발생합니다. 또한, 시상 하부를 통한 이 두 BAT 기능의 생리학적 조절은 완전히 다릅니다. 식사 후 시상하부 아치형 핵(Arc)에서 프로오피오멜라노코르틴(POMC) 발현 뉴런의 활성화는 rRPa16을 통해 교감 신경 활동의 증가로 이어집니다. IR 열화상 촬영 또는 PET-CT로 측정한 BAT의 저온 유도 활성화는 일상적인 BAT 활동에 대한 측정으로 사용될 때 부적절합니다. 식사 후 BAT 활동이 증가하면 포도당 이용이 이루어지며, 이는 궁극적으로 포도당 항상성, 인슐린 감수성 및 포도당 농도의 일일 조절을 유지하는 데 중요합니다. 식후 BAT 활성화는 식후 포도당 소비를 증가시키고 열 생성 및 체온(DIT)을 증가시킵니다. 이는 성별, 연령, BMI에 의존하는 것으로 나타났다12. 식사 후 BAT 활성화의 유사한 성별 차이가 수컷과 암컷 실험용 마우스에서 관찰된다17. 이러한 발견은 최근 발견된 BAT 조절의 성별 차이에 상응하며, Burke et al.은 POMC 뉴런의 하위 집단을 통한 BAT 갈변의 시상하부 조절이 수컷과 암컷 마우스에서 다르다는 것을 보여주었다18. BAT의 식후 활성화는 여성, 노인 인구 및 비만인에서 더 작습니다. 식사 후 BAT 활성화의 부족(포도당 이용 감소)은 여성의 내당능 장애의 유병률을 높일 수 있습니다 19,20,21,22. 불행히도 BAT 활성화에 대한 대부분의 연구는 남성에 대해서만 수행되었습니다. 식사 후 BAT를 활성화하면 마른 남성 인구에서 포도당 흡수가 증가합니다. BAT 활성화 후 DIT가 BAT 양성 남성 피험자에서 더 높다는 것은 놀라운 일이 아닙니다 8,9. 또한, 수컷 쥐의 BAT 이식은 내당능을 개선하고, 인슐린 감수성을 증가시키며, 체중과 체지방량을 감소시킨다23.
PET-CT는 특히 식사 후 BAT 활동을 측정하기 위한 선택 방법으로 실패합니다. 따라서 비침습적이고 보다 민감한 방법이 개발되었습니다. IR 열화상 촬영을 통해 성별, 연령 또는 BAT 활성에 대한 다양한 병리학적 조건의 영향에 관계없이 다양한 실험 동물(녹아웃 마우스)과 인간 참가자의 BAT 활성을 추정할 수 있습니다. 이 방법의 또 다른 이점은 참가자와 실험실 동물을 위한 단순성으로, 이를 통해 BAT 부스터 요법의 잠재적 이점을 추정할 수 있습니다. 저온 노출 또는 식사 후 BAT의 생리적 거동을 결정하기 위해 IR 열화상 촬영을 사용한 최근 연구는 Brasil et al.24의 최근 간행물에 설명되어 있습니다.
