胸腰椎後弯変形は、一般的に椎骨骨折、椎体の発達、強直性脊椎炎、および脊髄結核によって引き起こされる原発性または二次性疾患です1,2,3,4。重度の脊柱後弯変形は、脊髄圧迫または重度の腰痛を引き起こすことがよくあります。保存的治療が効果がなくなると、変形によって引き起こされる合併症のために外科的アプローチが必要です。しかし、適切な外科的治療は依然として物議を醸しています。
重度の脊柱後弯変形を治療するための外科的アプローチには、通常、グレード3以上の骨切り術5が必要です。椎弓根減算骨切り術(PSO)は、30°から40°6の間の矯正で報告される3列の骨切り術を達成できる技術です。脊柱後弯変形が40°を超える場合は、脊柱切除術が推奨されますが、脊椎が短くなり、脊髄バルクを誘発する可能性があります7。体と椎間板の部分切除(BDBO;グレード4)および後脊柱切除術(PVCR;グレード5)は、脊椎の前柱と中柱の完全な遮断を必要とし、手術または術後のインプラント沈降における脊椎の不安定性による重度の神経学的合併症を伴う脊椎に大きな損傷を引き起こす可能性があります7,8.一般のオペレーターにとって、この技術は習得するのが難しく、患者にとって外科的損傷は甚大です。したがって、より少ない損傷でより簡単に実行できる方法が必要です。
本報告では,胸腰椎後弯症を片側から除去し,自家骨を有するチタンメッシュを同じ側に配置することで,トレフィンを修飾した洗練された手術手技を紹介する.この技術は、良好な結果を達成しながら、患者への損傷を最小限に抑えることを目的としています。私たちの以前の研究では、洗練された外科的処置は有意に大きな結果を示し、対側茎と椎体の一部を温存することにより脊椎への損傷を減らしました9。
