関節軟骨は、軟骨細胞と細胞外マトリックス1からなる高度に分化した高密度組織である。関節軟骨の表層は硝子軟骨の一種であり、表面が滑らかで、摩擦が少なく、強度と弾力性が高く、機械的ストレス耐性に優れています2。細胞外マトリックスはコラーゲンプロテオグリカンと水からなり、コラーゲン全体の約90%を占めるII型コラーゲンがコラーゲンの主成分です3。軟骨組織には血管や神経が存在しないため、損傷後の自己修復能力が不足しています4。したがって、外傷によって引き起こされる軟骨欠損は、診療所では常に難治性の関節疾患でした。さらに、この関節疾患は若者を襲う傾向があり、世界的な発生率は増加しています5,6。膝関節は軟骨欠損の最も一般的な部位であり、ここでの欠損は関節痛、関節機能障害、関節軟骨変性を伴い、最終的には変形性膝関節症(kOA)につながります7。膝関節の軟骨欠損は、患者に経済的および生理学的負担をもたらし、患者の生活の質に深刻な影響を及ぼします8。この病気は、差し迫った解決策のない重大かつ緊急の臨床的課題をもたらします。現在、軟骨欠損の治療は手術が主力であるが、長期的な転帰は依然として満足のいくものではない9。
臨床的な軟骨欠損は最終的にkOAにつながるため、kOA動物モデルは軟骨欠損の病理学的研究や医薬品開発に一般的に使用されています。軟骨欠損修復の病態生理学的過程を理解するためには、軟骨の再生や線維軟骨と硝子軟骨の変化を観察するために、動物モデルの確立が重要です10。しかし、前十字靭帯離断(ACLT)、内側半月板の不安定化(DMM)、卵巣摘出術(OVX)、ハルスなどの一般的に使用されるkOA動物モデルは、通常、長期的なモデリングを必要とし、病理学的および疼痛の評価のみを可能にするため、医薬品開発の効率に限界があります11。手術モデルに加えて、モノヨードアセテート(MIA)やパパイン注射などの化学モデルも軟骨欠損を引き起こしますが、欠陥の程度はうまく管理できず、状態は臨床的現実からかけ離れています11。衝突は、より大きな動物の軟骨欠損をモデル化するための別のアプローチですが、この方法は特定の器具の使用に依存し、めったに適用されません12。

外傷による膝関節の軟骨欠損は、クリニックで一般的なスポーツ関節損傷であり、これらの欠陥は関節痛、運動障害、そして最終的には変形性膝関節症(kOA)を引き起こします。しかし、軟骨欠損やkOAに対する効果的な治療法はほとんどありません。治療薬の開発には動物モデルが重要ですが、軟骨欠損の既存のモデルは不十分です。本研究では、ラットの大腿滑車溝に穴を開けて全層軟骨欠損(FTCD)モデルを確立し、その後の疼痛行動と病理組織学的変化を読み出し実験として使用した。手術後、機械的離脱閾値が低下し、損傷部位の軟骨細胞が失われ、マトリックスメタロプロテイナーゼMMP13発現が増加し、II型コラーゲン発現が減少し、ヒト軟骨欠損で観察された病理学的変化と一致しました。この方法論は簡単で簡単に実行でき、損傷直後の肉眼的観察を可能にします。さらに、このモデルは臨床的な軟骨欠損をうまく模倣することができ、軟骨欠損の病理学的過程を研究し、対応する治療薬を開発するためのプラットフォームを提供する。

このプロトコルは、臨床FTCDの発生と開発を模倣するだけでなく、FTCDに対する治療的治療を評価するための信頼できる動物モデルも提供します。しかし、この技術は実行が容易であり、損傷直後の成長観察を可能にします。この技術は、臨床的軟骨欠損をうまく模倣することができ、軟骨欠損の病理学的過程を研究し、対応する治療薬を開発するためのプラットフォームを提供する。この方法は、外傷性軟骨欠損、すなわち心的外傷後変形性関節症の研究に適用できます。このテクニックを初めて試すときは、脛骨と腓骨を90度の角度で曲げて、大腿骨顆の蝸牛を完全に露出させ、ドリルビットを軟骨表面に垂直に保つことが重要です。麻酔をかけたラットを仰臥位で手術台に置くことから始めます。ラットの上に滅菌ドレープを置き、剃って消毒した膝関節を露出させます。11番のメスの刃を使用して、膝関節の中央に1センチの垂直切開を行います。そして、内側膝蓋骨縁に沿って関節包と大腿四頭筋腱を切断します。次に、膝蓋骨を外側に向け、脛骨と腓骨を90度の角度で曲げ、大腿骨顆蝸牛を露出させます。1.6ミリメートルの円形ドリルビットを使用して、大腿骨顆蝸牛に全厚0.1ミリメートルの深さの軟骨欠損を作ります。0.9%生理食塩水に浸した綿球で手術部位を拭き、膝蓋骨を交換します。次いで、膝を伸ばした位置に保ち、非吸収性の4-0縫合糸を使用して切開部を層ごとに縫合する。ラットの機械的離脱閾値またはMWT"を測定するには、動物を金網プラットフォーム上の単一のプラスチックチャンバーに入れます。金網のベースをテーブルの上に50センチ置きます。適応の30分後にMWT測定を開始します。次に、Von Freyフィラメントをラットの後足のプランター表面を垂直に押します。次に、足の中央部分の最も厚い部分を避けて、ブラシを2秒間曲げます。足の離脱や舐めなどの肯定的な反応が発生するまで、刺激の重量を4グラムから徐々に増やします。モデル群のラットをモデル化してから3日後、偽群と比較してMWTの減少を示した。モデル群における痛覚過敏を示唆する。モデル群のMWTは17日後も低いままであった。疼痛感受性の長さを示す。偽群の病理組織染色では,関節および無傷の軟骨表面が明瞭で,軟骨細胞の分布が均一で,II型コラーゲンの高発現が認められた.対照的に、モデル群は、軟骨表面の落ち込み、軟骨細胞の喪失、マトリックスメタロプロテイナーゼMMP13の発現の増加、およびII型コラーゲンの発現の低下を示しています。覚えておくべき最も重要なことは、縫合する前に膝蓋骨を縮小しなければならないということです。
