足首の捻挫は、世界中で最も一般的なスポーツ傷害の1つです。米国では毎日10,000人が負傷していると推定されており1、そのうちスポーツ関連の負傷は15%〜45%を占めています2。米国における足首の捻挫の治療に関連する医療費は、年間42億ドルにのぼります3,4,5。慢性的な足の不安定性は、足首の捻挫に続く一般的な問題であり、足首や距骨下の不安定性を含む足首の捻挫の約74%で発生します6。しかし、臨床症状や徴候が似ているため、慢性的な足首の不安定性が慢性的な距骨下関節の不安定性も伴うかどうかを医療スタッフが区別することは困難であり、その結果、慢性的な距骨下不安定性は簡単に見逃される可能性があります。したがって、慢性足首距骨下複合関節(ASCJ)不安定性(慢性足首不安定性と慢性距骨下不安定性の両方を含む特定のタイプの慢性足の不安定性)の真の発生率は、報告されている7,8,9よりも高い可能性があります。慢性的な足首-距骨下複合関節の不安定性は、治療せずに放置すると、足首の捻挫を繰り返し、足首の捻挫と慢性的な足首-距骨下複合体の不安定性の悪循環につながる可能性があります。長期にわたる慢性的な足首-距骨下複合体の不安定性は、ASCJの変性および心的外傷後変形性関節症につながる可能性があり、重症の場合、隣接する関節に影響を与える可能性があります10。これらの疾患に対して、現在の臨床治療は、靭帯修復や靭帯再建などの外科的治療法に加えて、主に保存的である11,12。
ASCJは足のコア構造であり、動き13の間、体のバランスを維持します。足首関節と距骨下関節の構造について、別々に広範な研究が行われています14,15,16,17。しかし、足首と距骨下関節全体の研究はまれです。足首損傷の症例の約4分の1は距骨下関節損傷に関連している18。ASCJの不安定性の複雑な損傷メカニズムのために、臨床現場での診断と治療に関するコンセンサスはありません。クリニックにおける足関節損傷の現状を考えると、足関節と距骨下関節全体を研究するためのより科学的な方法が必要であり、それによって足の病気を研究するための新しい理解を提供します。
筋骨格レベルでのマウス後足の解剖学的構造は人間の足の解剖学的構造に匹敵するため19、いくつかの研究では、足/足首研究用のマウスモデルがすでに実装されています10,19。Changら19は、変形性足首関節症の3つの異なるマウスモデルの開発に成功しました。マウスモデルにおける足首の不安定性の確立に成功したことに触発され、マウス後足の部分靭帯の切断がASCJの機械的不安定性をもたらし、ASCJの心的外傷後変形性関節症(PTOA)につながるという仮説を立てて、足首-距骨下複合体の不安定性のマウスモデルを確立しました。ASCJ不安定性動物モデルは、足首の不安定性と距骨下不安定性の両方の治療に使用でき、現在使用されている単純な足首不安定性モデル7,8,9,19よりも実際の臨床状況と一致しています。この仮説を検証するために、靭帯離断によるASCJの不安定性を示す2つのマウスモデルを設計しました。感覚運動機能(平均台試験、フットプリント解析、熱侵害受容評価)の結果を用いてモデルの実現可能性を評価し、マイクロコンピュータ断層撮影(CT)と組織学的染色を用いてマウス関節軟骨の損傷と変性を評価した。ASCJ不安定性のマウスモデルの確立に成功したことは、足の病気を研究するための新しい理解を提供するだけでなく、損傷関連のメカニズムに関する臨床研究のための貴重な参考資料を提供し、足首の捻挫に対するより良い治療オプションを提供し、病気のさらなる研究に役立ちます。
