발목 염좌는 전 세계적으로 가장 흔한 스포츠 부상 중 하나입니다. 미국에서는 매일 10,000명이 부상을 입는 것으로 추정되며1 그 중 스포츠 관련 부상이 15%-45%2를 차지합니다. 미국에서 발목 염좌 치료와 관련된 의료 비용은 연간 42억 달러에 달합니다 3,4,5. 만성 발 불안정은 발목 염좌 후 흔히 발생하는 문제로, 발목 염좌의 약 74%에서 발생하며, 발목 또는 거골하 불안정성을 포함한다6. 그러나 임상적 증상 및 징후가 유사하기 때문에 의료진은 클리닉에서 만성 발목 불안정성이 만성 거골하 관절 불안정을 동반하는지 여부를 구별하기 어렵고, 그 결과 만성 거골하 불안정성을 쉽게 놓칠 수 있습니다. 따라서 만성 발목-거골하 복합 관절(ASCJ) 불안정성(만성 발목 불안정성과 만성 거골하 불안정성을 모두 포함하는 특정 유형의 만성 발 불안정성)의 실제 발생률은 보고된 것보다 높을 수 있습니다 7,8,9. 치료하지 않고 방치하면 만성 발목-거골하 복합 관절 불안정이 반복되어 발목 염좌와 만성 발목-거골하 복합 불안정성의 악순환으로 이어질 수 있습니다. 장기간의 만성 발목-거골하 복합체 불안정은 ASCJ의 퇴행과 외상 후 골관절염으로 이어질 수 있으며, 심한 경우 인접 관절에 영향을 미칠 수 있다10. 이러한 질환에 대한 현재의 임상적 치료는 주로 보존적 치료이며, 인대 재건 및 인대 재건과 같은 수술적 치료 방법11,12.
ASCJ는 발의 핵심 구조이며 동작13 동안 신체의 균형을 유지합니다. 발목 관절과 거골하 관절의 구조에 대한 광범위한 연구가 수행되었습니다14,15,16,17. 그러나 발목-거골하 관절 전체에 대한 연구는 드뭅니다. 발목 부상의 약 1/4은 거골하 관절 손상과 관련이 있다18. ASCJ 불안정성의 복잡한 손상 메커니즘으로 인해 임상 환경에서 진단 및 치료에 대한 합의가 이루어지지 않았습니다. 현재 임상에서 발목 부상의 현황을 고려할 때, 발목과 거골하 관절을 전체적으로 연구하여 발 질환 연구에 대한 새로운 이해를 제공하기 위해서는 보다 과학적인 방법이 필요합니다.
근골격계 수준에서 쥐 뒷발의 해부학적 구조는 인간의 발과 비슷하기 때문에19 여러 연구에서 발/발목 연구를 위한 마우스 모델이 이미 구현되었다10,19. Chang et al.19은 발목 골관절염에 대한 세 가지 다른 마우스 모델을 성공적으로 개발했습니다. 마우스 모델에서 발목 불안정성을 성공적으로 확립한 것에서 영감을 받아 발목-거골하 복합 불안정성에 대한 마우스 모델을 확립하여 마우스 뒷발의 부분 인대 절개가 ASCJ의 기계적 불안정성을 초래하여 ASCJ의 외상 후 골관절염(PTOA)으로 이어질 것이라는 가설을 세웠습니다. ASCJ 불안정성 동물모델은 발목 불안정성과 거골하 불안정성 모두에 사용될 수 있으며, 이는 현재 사용되는 단순 발목 불안정성 모델 7,8,9,19보다 실제 임상 상황에 더 부합합니다. 이 가설을 테스트하기 위해 ASCJ의 인대 절개로 인한 불안정성의 두 마우스 모델을 설계했습니다. 균형 빔 테스트, 발자국 분석 및 열 통각 평가와 같은 감각 운동 기능 결과는 모델의 타당성을 평가하는 데 사용되었으며 마이크로 컴퓨터 단층 촬영(CT) 및 조직학적 염색은 마우스 관절 연골의 손상 및 변성을 평가하는 데 사용되었습니다.
