甲状腺結節は最も一般的な内分泌疾患の1つであり、その中で甲状腺がんは14%〜21%1を占めています。甲状腺がんの好ましい治療法は手術です。しかし、甲状腺は前頸部に位置するため、手術領域には副甲状腺、気管、食道、頸部大血管や神経など、甲状腺に近い重要な組織や臓器があり2,3、手術は比較的困難で危険です。最も一般的な外科的合併症は、副甲状腺機能障害によって引き起こされる副甲状腺機能の低下、または反回神経損傷によって引き起こされる切除ミスと嗄声です4。上記の外科的合併症の減少は、常に外科医の目的でした。甲状腺手術前の最も一般的な画像診断方法は超音波画像ですが、副甲状腺と神経の表示は非常に限られています5。さらに、甲状腺手術領域における副甲状腺と反回神経の位置の変動は非常に大きく、識別を妨げています6,7。手術中に各患者の解剖学的位置をモデルを通じてリアルタイムで外科医に明確に表示できれば、甲状腺手術の運用リスクを減らし、合併症の発生率を減らし、甲状腺手術の効率を向上させます。
また、手術前に患者さんに手術過程を徹底的に説明することも困難です。経験の浅い外科医の中には、特に甲状腺とその周囲の構造が複雑なため、手術の正確な詳細を患者に説明して伝えるのが難しいと感じる人もいます。各患者は独自の解剖学的構造と個人的なニーズを持っています8.したがって、患者の実際の解剖学的構造に基づいてパーソナライズされた3D甲状腺モデルは、患者と臨床医を効果的に支援できます。現在、市場に出回っている製品の大部分は、平面図に基づいて大量生産されています。3D印刷技術を利用して、各患者の個々の医療ニーズを反映した患者固有のモデルを作成することにより、このモデルを使用して甲状腺がん患者の実際の状態を評価し、外科医が病気の性質を患者とよりよく伝えるのに役立ちます。
3D印刷(または積層造形)は、コンピュータ支援設計モデルまたはデジタル3Dモデル9から構築された3次元構造です。医療機器、解剖学的モデル、薬物製剤など、多くの医療用途で使用されています10。従来のイメージングと比較して、3D印刷モデルはより見やすく、読みやすくなります。したがって、3D印刷は現代の外科手術でますます使用されています。一般的に使用される3D印刷技術には、バット重合ベースの印刷、粉末ベースの印刷、インクジェットベースの印刷、および押出ベースの印刷が含まれます11。バット重合ベースの印刷では、特定の波長の光が光硬化性樹脂のバレルに照射され、樹脂が一度に1層ずつ局所的に硬化します。材料の節約と高速印刷という利点があります。粉末ベースの印刷は、局所加熱に依存して粉末材料を融合させてより緻密な構造にしますが、印刷時間とコストの大幅な増加にもつながり、現在使用が制限されています12。インクジェットベースの印刷では、層ごとのプロセスで基板に液滴を正確に噴霧します。この技術は最も成熟しており、高い材料適合性、制御可能なコスト、および高速印刷時間という利点があります13。押し出しベースの印刷は、溶液や懸濁液などの材料をノズルから押し出します。この技術は細胞を利用しているため、軟部組織を模倣する能力が最も高いです。コストとバイオアフィニティが高いため、主に組織工学の分野で使用され、外科用臓器モデルではあまり使用されません14。
その結果、甲状腺とその周囲の構造の複雑さと手術スケジュールに基づいて、「ホワイトジェットプロセス」印刷技術を選択しました。この技術は、バット重合ベースの印刷とインクジェットベースの印刷の利点を兼ね備えており、高精度、高速印刷、低コストを提供し、甲状腺手術に適しています。このプロトコルの目的は、3Dプリントされた甲状腺がんモデルを作成し、患者の解剖学的構造とバリエーションに関する十分な情報を提供することにより患者の予後を改善し、医師と患者に手術プロセスに関連するすべての状態についてよりよく知らせることです。
