갑상선 결절은 가장 흔한 내분비 질환 중 하나이며 그 중 갑상선암이 14%-21%를 차지합니다1. 갑상선암에 대한 선호되는 치료법은 수술입니다. 그러나 갑상선은 전방 경추 부위에 위치하기 때문에 부갑상선, 기관, 식도, 경추 대혈관 및 신경과 같은 수술 부위의 갑상선 가까이에 중요한 조직과 기관이 있습니다 2,3 수술이 상대적으로 어렵고 위험합니다. 가장 흔한 수술 합병증은 부갑상선 기능 손상 또는 잘못된 절제술로 인한 부갑상선 기능 저하와 재발성 후두 신경 손상으로 인한 쉰 목소리이다4. 위에서 언급 한 수술 합병증의 감소는 항상 외과 의사의 목표였습니다. 갑상선 수술 전 가장 일반적인 영상 방법은 초음파 영상이지만 부갑상선과 신경의 표시는 매우 제한적입니다5. 또한, 갑상선 수술 부위의 부갑상선과 재발 성 후두 신경의 위치 변화가 매우 커서 식별을 방해합니다 6,7. 수술 중 실시간으로 모델을 통해 각 환자의 해부학적 위치를 외과의에게 명확하게 표시할 수 있다면 갑상선 수술의 수술 위험을 줄이고 합병증 발생률을 줄이며 갑상선 수술의 효율성을 높일 수 있습니다.
또한 수술 전에 환자에게 수술 과정을 철저히 설명하는 것도 어렵습니다. 경험이 부족한 일부 외과 의사는 특히 갑상선과 그 주변 구조의 복잡성 때문에 수술의 정확한 세부 사항을 환자에게 설명하고 전달하는 데 어려움을 겪습니다. 환자마다 고유한 해부학적 구조와 개인적 필요가 있다8. 따라서 환자의 실제 해부학을 기반으로 한 개인화된 3D 갑상선 모델은 환자와 임상의를 효과적으로 도울 수 있습니다. 현재 시장에 나와있는 대부분의 제품은 평면 다이어그램을 기반으로 대량 생산됩니다. 3D 프린팅 기술을 활용하여 각 환자의 개별 의학적 요구를 반영하는 환자별 모델을 생성함으로써 이 모델은 갑상선암 환자의 실제 상태를 평가하고 외과의가 환자와 질병의 특성을 더 잘 전달하는 데 사용할 수 있습니다.
3D 프린팅(또는 적층 제조)은 컴퓨터 지원 설계 모델 또는 디지털 3D 모델(digital 3D model)9을 기반으로 구축된 3차원 구조이다. 이는 의료 기기, 해부학적 모델 및 약물 제형과 같은 많은 의료 응용 분야에서 사용되어 왔다10. 기존 이미징에 비해 3D 프린팅 모델은 더 잘 보이고 읽기 쉽습니다. 따라서 3D 프린팅은 현대 외과 수술에서 점점 더 많이 사용되고 있습니다. 일반적으로 사용되는 3D 프린팅 기술에는 통 중합 기반 프린팅, 분말 기반 프린팅, 잉크젯 기반 프린팅 및 압출 기반 프린팅이 포함됩니다 11. 통 중합 기반 인쇄에서는 특정 파장의 빛이 광경화 수지 배럴에 조사되어 수지를 한 번에 한 층씩 국부적으로 경화시킵니다. 그것은 재료 절약과 빠른 인쇄의 장점이 있습니다. 분말 기반 인쇄는 더 조밀한 구조를 위해 분말 재료를 융합하기 위해 국부적 가열에 의존하지만, 인쇄 시간과 비용의 상당한 증가로 이어지며 현재 제한적으로 사용되고 있다12. 잉크젯 기반 인쇄는 층별 공정으로 기판에 물방울을 정밀하게 분사합니다. 이 기술은 가장 성숙하고 높은 재료 호환성, 제어 가능한 비용 및 빠른 인쇄 시간의 장점이 있습니다13. 압출 기반 인쇄는 노즐을 통해 용액 및 현탁액과 같은 재료를 압출합니다. 이 기술은 세포를 활용하므로 가장 높은 연조직 모방 능력을 가지고 있습니다. 높은 비용과 생체 친화력으로 인해 주로 조직 공학 분야에서 사용되며 수술 장기 모델에서는 덜 사용된다14.
