השמנת יתר היא מצב מתפתח ואנדמי עם שכיחות הולכת וגוברת, המשפיעה על בערך 1 מתוך 20 מבוגרים ברחבי העולם1. ניתוח בריאטרי הפך לאפשרות הטיפול היעילה ביותר עבור המבוגרים המושפעים בשנים האחרונות, שיפור הן ירידה במשקל והפרעות מטבוליות2,3, עם תוצאות משתנות בהתאם לסוג ההליך הכירורגי בשימוש.
ישנם שני מנגנונים עיקריים המעורבים בהשפעות הפרוצדורות הבריאטריות: הגבלה שמטרתה להגביר את השובע (כמו בכריתת קיבה בשרוול (SG) שם מסירים 80% מהקיבה), ותת ספיגה. בין ההליכים המרמזים הן על הגבלה והן על תת-ספיגה, מעקף אנסטומוזיס דואודנו-איליאלי יחיד עם כריתת קיבה בשרוול (SADI-S) הוצע כחלופה למעקף קיבה Roux-en-Y (RYGB), שבו נצפתה עלייה חוזרת במשקל בכ -20% מהחולים 4,5. בשיטה זו, כריתת שרוול קיבה קשורה לסידור מחדש של המעי הדק, ומחלקת אותו לגפה מרה ולגפה משותפת קצרה (שליש מאורך המעי הדק הכולל) (איור 1A). מבחינה טכנית, ל- SADI-S יש יתרון על פני ה- RYGB בכך שהוא דורש אנסטומוזה אחת בלבד, מה שמקצר את זמן הפעולה בכ -30%. בנוסף, שיטה זו משמרת את הפילורוס, אשר מסייע להפחית את הסיכון למחלת כיב פפטי ומגביל דליפה אנסטומטית.
