肥満は、有病率が増加している新たな風土病の状況であり、世界中の成人の約20人に1人が罹患しています1。肥満手術は、近年、影響を受けた成人にとって最も効果的な治療選択肢となり、体重減少と代謝障害の両方を改善し2,3、使用される外科的処置の種類に応じて結果はさまざまです。
肥満手術の効果に関係する2つの主要なメカニズムがあります:満腹感を高めることを目的とした制限(胃の80%が除去されるスリーブ状胃切除術(SG)など)、および吸収不良。制限と吸収不良の両方を意味する手順の中で、スリーブ状胃切除術を伴う単一吻合十二指腸回腸バイパス術(SADI-S)は、約20%の患者で体重回復が観察されるRoux-en-Y胃バイパス(RYGB)の代替として提案されています4,5。この技術では、スリーブ状胃切除術は小腸の再配置に関連し、胆道と短い総四肢(小腸全長の3分の1)に分割されます(図1A)。技術的には、SADI-SはRYGBに比べて単一の吻合のみを必要とするという利点があり、操作時間を約30%短縮します。さらに、この方法は幽門を保存し、消化性潰瘍疾患のリスクを軽減し、吻合部漏出を制限します。SADI-Sはまた、代謝改善率が高く、過去数年間の使用を強く支持しています6,7。
代謝効果は肥満手術の基礎となるものになりつつあるため、そのメカニズムを解明することは重要であるように思われます。したがって、肥満手術のための動物モデルの使用は、それらの代謝効果と関与する細胞および分子経路をよりよく理解するために最も重要です8。これらのモデルは、例えば、制御された環境でのSGまたはRYGB後の食物摂取の変化のより良い理解9および腸関門を通るグルコースまたはコレステロールフラックスの研究に貢献した10,11;これらの情報は、臨床試験ではほとんど入手できません。この知識は、最適な外科的適応症を定義するのに役立つ可能性があります。我々は以前にSGおよびRYGB12のマウスモデルについて説明しました。しかし、臨床診療における有望な結果にもかかわらず、SADI-Sはラット13、14、15でのみ開発および記載されています。しかし、その遺伝的可鍛性を考えると、マウスモデルは過去にそのような手順のさまざまな代謝効果を研究するために有用であり16、17、18、SADI-Sマウスモデルは、技術的な困難にもかかわらずSADI-Sの効果を評価するのに役立つ可能性があります。
