副甲状腺ホルモン(PTH)は副甲状腺から分泌されます。カルシウムバランスの主要なモジュレーターであり、リン酸代謝を維持し、骨代謝回転に関与します1,2。副甲状腺機能低下症(HypoPT)は、PTHの分泌低下または機能喪失として現れます。これはまれな内分泌障害であり、有病率は10万人年あたり約9〜37人です3,4,5。HypoPTは、血清PTHおよびカルシウムレベルの低下が特徴であり、血清リンの増加を伴う6,7。HypoPTは、その原因に基づいて分類されます:後天性副甲状腺機能低下症(AHypoPT)または特発性副甲状腺機能低下症(IHypoPT)8。AHypoPTは臨床診療でより一般的に遭遇します。AHypoPT症例の約75%は、甲状腺手術またはその他の頭頸部手術中の副甲状腺の切除または偶発的な損傷によって引き起こされます。その他の原因には、頭頸部腫瘍に対する放射線療法と化学療法、および薬物毒性が含まれます1,8。診断法のアップグレードと甲状腺関連疾患のスクリーニングの増加により、甲状腺外科手術の数が増加しています。これは、関連する副甲状腺合併症の対応する増加につながっています9,10。
AHypoPTをよりよく調べ、新しい治療法の治療効果を検証するためには、安定した特性を持つ容易に確立された動物モデルが必要です。ラットおよびマウスに対して行われた副甲状腺摘出術(PTX)は、以前の研究で報告されています6,11;しかし、副甲状腺のサイズが非常に小さく、解剖学的分布が変動するため、実際には成功率は比較的低いです。したがって、甲状腺・副甲状腺摘出術(TPTX)(すなわち、甲状腺および副甲状腺の全摘出)は、通常、副甲状腺12の切除を確実にするために行われる。しかしながら、結果として生じる低チロキシンレベルは、この動物モデル13を用いた研究を複雑にし得る。薬物刺激や遺伝子編集などの他の方法で確立されたHypoPTモデルは、最も一般的なAHypoPT病因を適切に表すことができません。私たちのグループは以前、ノックアウトマウスモデルを使用して副甲状腺を標識し、甲状腺とその周囲の解剖学的構造に損傷を与えることなく副甲状腺を除去できるようにしました14,15。しかし、この方法はトランスジェニックマウスモデルを利用しており、交配や繁殖の要件から開発期間が長くなります。
そこで、AHypoPTの簡易生成モデルの確立を目指しました。この研究では、カーボンナノ粒子標識を使用したPTXのラットモデルについて説明します。甲状腺手術で一般的に使用される50 mg/mLのカーボンナノ粒子懸濁液は、局所注射後に甲状腺に均等に分布します16。甲状腺は黒くなりますが、副甲状腺は染色されないまま17、副甲状腺と甲状腺を明確に区別し、甲状腺に影響を与えることなくPTXを行うことができます。この方法は、さまざまな年齢のラットに適しています。カーボンナノ粒子懸濁液の注入は安全であり、甲状腺機能18に対する影響はごくわずかである。この研究で生成された炭素ナノ粒子標識PTXラットモデルは、4週間の観察期間中に有意な低カルシウム血症および高リン血症の表現型を示しました。したがって、このAHypoPTモデルは確立が容易であり、再現可能な表現型を有する。

このプロトコルの全体的な目標は、確立が容易で、性格が安定しており、甲状腺に無害な後天性副甲状腺機能低下症のラットモデルを生成することです。このモデルの主な利点は、カーボンナノ粒子の助けを借りて副甲状腺を正確に除去できることです。このモデルが副甲状腺機能低下症の分野におけるいくつかの問題を解決するのに役立つことを願っています。例えば、甲状腺摘出術後の低カルシウム血症のリスクを減らすためのカルシウムとビタミンDの術前使用。この方法は、PTHアナログによって骨代謝回転がどのように影響を受けるか、およびPTHアナログが長期使用で身体にどのように影響するかについての洞察を提供することができます。この手法を初めて実行する人は、実体顕微鏡でマイクロマニピュレーションするのが難しいと感じるかもしれません。実体顕微鏡下での練習が役立つ場合があります。はじめに、麻酔をかけたラットを仰臥位に置き、腹側頸部の毛皮を剃ります。原稿に記載されているように切開を行った後、甲状腺の葉を見つけます。30ゲージのベベルニードルを備えた10マイクロリットルのシリンジを使用して、1マイクロリットルのカーボンナノ粒子懸濁液を甲状腺膜の近心部に注入します。5分後、生理食塩水でその領域を灌漑して、余分な懸濁液をきれいにします。甲状腺が黒くなっていることを確認し、汚れていない副甲状腺を観察します。マイクロサージェリー鉗子とはさみを使用して染色されていない副甲状腺を切断し、中断された水平マットレス縫合糸で切開を閉じます。手術後、ラットを摂氏37度の恒温電気毛布の上に置いて体温を維持します。7日後、尾静脈を使用して10マイクロリットルの血液を採取します。市販のキットを使用して、血清カルシウム、血清無機リン、および血清副甲状腺ホルモンを測定します。副甲状腺摘出ラットは4週間の観察期間にわたって安定した低カルシウム血症と高リン血症を示した。血清副甲状腺ホルモンは術後7日目に副甲状腺摘出ラットでは検出されなかった。血清尿素とクレアチニンは、術後7日後の偽甲状腺摘出群と副甲状腺摘出群の間で同等でした。尿中カルシウムイオンと無機リン酸レベルの低下は、術後2週間で観察されました。28日間にわたって副甲状腺摘出術群と偽群の間で体重に有意差は観察されなかった。血清C-テロペプチドは、副甲状腺摘出ラットで有意に減少しました。.血清オステオカルシン濃度に有意差はなかった。しかし、ナノ粒子懸濁液注入は最も重要なことであり、注射の成功は副甲状腺の同定と現在の除去にとって重要である。他の方法には、蛍光ガイド下手術、PTHcre-iDTRトランスジェニックマウスのジフテリア毒素注射、および甲状腺機能亢進症の分野における質問に答えるのに役立つ可能性のある無菌副甲状腺摘出術が含まれます。
