超音波画像の取得は、気道の標準的な超音波評価を導くことができます。超音波は、臨床検査だけでは不可能な動的な解剖学的気道評価を臨床医に提供することができます。テクニックは簡単で、自分自身や同僚に練習することができます。約20回の試験でコンピテンシーを取得することが可能です。まず、患者に枕なしで仰臥位で横になるように頼みます。標準化と最良の測定を確実にするために、患者に頭頸部をニュートラルな位置に維持するように依頼します。患者に下切歯に舌を当てるように頼みます。一貫性を保つために、舌は超音波検査全体を通して同じ位置にとどまる必要があります。患者を正しい位置に置いたら、トランスデューサーにゲル培地を塗布して、皮膚とトランスデューサーの間に空気がないことを確認します。トランスデューサーを患者の前頸部に横向きに置き、皮膚との接触を維持しながら、最小限の圧力で配置します。次に、トランスデューサーの正中線を横方向の中心軸に配置します。そして、ゆっくりとした細かい動きで顎下腔から始めて、トランスデューサーを心を込めて動かします。顎下画像の場合は、トランスデューサーを身体の中心軸に沿ってメンタル下空間に縦方向に置きます。freezeをクリックし、続いてmeasureをクリックし、メンタムの外側の境界から舌骨までの距離を測定します。センチメートル単位の距離の値が画面にポップアップ表示されるので、[取得]をクリックします。トランスデューサーを横方向に回転させ、首の中心軸の上に置きます。探触子を細かくゆっくりとした動きで丁寧に操作し、目的の構造を視覚化します。甲状腺舌膜(THM)を視覚化するには、甲状軟骨と舌骨を触診し、トランスデューサーを頸部の中心軸に留まらせながら、横方向の位置の間にトランスデューサーを配置します。順番に、皮膚から中央の喉頭蓋の前縁までの距離を測定する前に、凍結をクリックして測定します。センチメートル単位の距離が画面にポップアップ表示されたら、[取得]をクリックします。次に、探触子を右に1センチ動かし、前に示したように、測定を繰り返します。距離の値が画面に表示されたら、データを取得します。次に、探触子を中心から左に1センチメートル移動させ、測定値を取得するプロセスを繰り返します。3つの測定値を平均してTHMを求めます。探触子を同じ位置に保ち、次の中心軸にとどまり、低エコー曲線喉頭蓋を暗い空間として見ます。freezeをクリックし、続いてmeasureをクリックします。次に、皮膚から明るい白い線の中心までの距離を測定します。センチメートル単位の距離の値が画面にポップアップ表示されるので、[取得]をクリックします。プローブを正中線から左に1センチメートル動かし、測定値を再度取得します。次に、探触子を正中線の右に1センチメートル動かして、3回目の測定値を取得します。3つの測定値を平均して、皮膚から喉頭蓋またはDSCまでの距離を取得します。探触子のテールを約20度下に傾けます。舌骨を触診し、プローブを首の中心軸に保ちながら、トランスデューサーを舌骨の真上に置きます。順番に、凍結をクリックして測定します。低エコー影の上で逆さまに湾曲した明るいエコー源線として見られる、皮膚から舌骨の中心までの距離を測定し、データを取得します。プローブを左側の正中線から1センチ横に動かして、もう1つの測定値を取得します。3回目の測定では、トランスデューサーを正中線の右に1センチメートル移動させ、3回の測定値を平均して、皮膚から舌骨またはSHBまでの距離を取得します。皮膚から声帯またはSVCまでの距離を測定するには、首の中心軸にとどまりながら、トランスデューサーを甲状軟骨の上に横向きに配置します。フリーズをクリックしてから、メジャーをクリックします。次に、皮膚から右声帯の上縁までの距離を測定し、データを取得します。左の声帯で同じ測定を繰り返し、2つの測定値を平均してSVCを取得します。30件の研究では、喉頭鏡検査の困難を予測するいくつかの異なる超音波パラメータが分析された。2件のメタアナリシスにより、喉頭直接鏡検査の簡易なビューと難しいビューで有意に異なる、最も研究されている5つのパラメータが同定された。中立位置では、3.4〜4.5センチメートルのHMD値は、直接喉頭鏡検査の困難と関連していました。このパラメータの感度と特異度はそれぞれ100%と71.4%であり、一般集団と肥満集団で予測的であった。TMH値が2.8センチメートルを超えると、喉頭鏡検査が困難になることが予測されました。このパラメータの感度は64.7%、特異度は77.1%でしたメタアナリシスでは、DSEがパラメータとして、困難な直接喉頭鏡検査と最も相関していることがわかりました。2〜2.5センチメートルを超える平均DSE値は、約30%〜50%の陽性適中率を示しましたが、パラメータの陰性適中率は95%〜97%の範囲でした1.28センチメートルを超えるSHBは、直接喉頭鏡検査の困難と相関していました。パラメータの感度と特異度はそれぞれ85.7%と85.1%でした。困難な直接喉頭鏡検査では、約0.9〜1.3センチメートルのSVC測定値が報告された。頭をニュートラルな位置に保ち、舌を下切歯に置くことが重要です。トランスデューサーは首の中心軸上に置く必要があり、ゆっくりと動かす必要があります。超音波検査は、気道の臨床検査の補助として日常診療に組み込むことができます。
