단순한 열상에서 사지 전체 절단에 이르기까지 다양한 상처에는 다양한 정도의 말초 신경 손상이 동반됩니다. 이러한 신경 손상은 발아 신경 섬유의 무질서한 얽힘인 신경종의 형성을 초래할 수 있습니다. 신경종은 환자의 8%-30%에서 통증이 나타나 삶의 질에 심각한 영향을 미칩니다 1,2,3,4,5. 사지 절단 후, 신경 종 통증은 환자의 50 %에서 발생합니다 6,7,8. 보고된 증상으로는 압통, 자발성 통증, 이질통, 통각과민, 신경분포 부위의 기계적 또는 열적 과민증등이 있다 9. 1년 이내에 적절하게 치료하지 않으면 신경종 통증이 만성 통증 상태로 진행되어 높은 사회적 부담과 관련 의료 비용이 발생할 수 있습니다10,11,12,13,14. 현재의 약리학적 개입의 빈약한 효능으로 인해, 신경종 통증은 바람직하게는 문헌15에 기재된 바와 같이 통증성 신경종의 외과적 제거에 의해 치료되고, 신경은 다양한 외과적 기법에 의해 치료된다. 완전한 통증 완화는 드물고, 통증은 종종 시간이 지남에 따라 악화되며, 환자의 40%가 수술의 혜택을 받지 못하므로 새로운 치료법이 필요하다는 점에 유의해야 합니다 1,16.

경골 신경종 전위(TNT)는 신경종 부위(경골 신경)의 이질통이 온전한 비골 신경에 의해 신경분포된 뒷발의 발바닥 표면에서 이질통과 독립적으로 평가될 수 있는 쥐 모델입니다. 이 TNT 모델은 클리닉에서 이미 사용되고 있는 특정 수술 요법의 잠재적 우월성과 같은 신경종 통증 치료법을 테스트하거나 동일한 동물의 두 통증 양식에 대한 신약 및 그 효과를 평가하는 데 적합합니다. 이 모델에서는 경골 신경에 원위 병변(신경망)이 만들어지고 근위 신경 말단이 전치되고 피하 및 경골로 고정되어 15g Von Frey 모노필라멘트로 신경종 부위를 평가할 수 있습니다. 비복 신경에 대한 이질통을 평가하기 위해 Von Frey 모노필라멘트는 뒷발의 발바닥 측면 영역에서 상하 방법을 통해 사용할 수 있습니다. 경골 신경을 절단한 후 수술 후 1주일 이내에 신경종 부위에 기계적 과민증이 발생하고 수술 후 최소 12주까지 지속됩니다. 비복 신경 분포 발바닥 표면의 이질통은 반대쪽 사지에 비해 수술 후 3주 이내에 발생합니다. 12 주에 절단 된 경골 신경의 근위 끝에 신경종이 형성되며, 축삭의 분산과 소용돌이로 나타납니다. TNT 모델 수술의 경우 여러 중요한 (미세) 수술 단계를 따라야 하며 말기 마취 하에 일부 수술 실습이 권장됩니다. 예비 신경 손상 모델과 같은 다른 신경병성 통증 모델과 비교하여 신경종 부위에 대한 이질통은 TNT 모델에서 비뇨 신경 과민증으로부터 독립적으로 테스트할 수 있습니다. 그러나 신경종 부위는 생쥐가 아닌 쥐에서만 검사할 수 있습니다. 이 프로토콜에 제공된 팁과 지침은 통증에 대해 연구하는 연구 그룹이 시설에서 TNT 모델을 성공적으로 구현하는 데 도움이 될 수 있습니다.
