上肢の運動障害は、脳卒中の最も一般的な結果の1つであり、日常生活動作の制限に関連しています1,2。アルファ(8-13 Hz)とベータ(13-30 Hz)のバンドリズムは、動きと密接に関連していることが知られています。特に、研究によると、障害のある手足の運動中のアルファおよび下部ベータ(12〜20 Hz)周波数帯域での神経活動の変化は、脳卒中患者の運動障害の程度と相関しています3,4,5。これらの知見に基づき、脳波検査(EEG)は、運動障害の重症度と運動回復の可能性の両方を反映する潜在的なバイオマーカーとして浮上しています6,7。しかし、以前に開発された脳波ベースのバイオマーカーは、主にタスク誘発性脳波データではなく安静時の脳波データに依存しているため、脳卒中患者の運動障害の特徴を調査するには不十分であることが証明されています8,9,10。イプシレシオナル半球と対病変半球の相互作用など、運動障害に関連する複雑な情報処理は、安静時の脳波ではなく、タスク誘発の脳波データによってのみ明らかにすることができます。したがって、脳卒中患者の運動障害の潜在的なバイオマーカーとして、神経活動と運動障害特性の関係を探り、障害のある身体部分の運動中に生成される脳波の有用性を明らかにするために、さらなる研究が必要である11。
行動効果を評価するためのEEGの実装には、タスク固有のパラダイムとプロトコルが必要です。現在までに、脳卒中患者が想像上または実際の動きをして、運動関連の脳活動を誘発するさまざまな脳波プロトコルが提案されています11,13。想像上の動きの場合、参加者の約53.7%が対応する動きを明確に想像できず(「リテラシー」と呼ばれる)、したがって運動に関連する脳活動を誘発できなかった14。また、重度の脳卒中患者は上肢全体を動かすことが難しく、動きが不安定なため、データ取得時に不要なアーチファクトが発生する可能性があります。そのため、課題に関する高品質な脳波データや神経生理学的に解釈可能な結果を得るためには、専門家のノウハウに基づいた指導が必要です。本研究では、脳卒中患者が比較的単純な手の動きの課題を行なうための実験パラダイムを包括的に設計し、詳細なガイダンスを伴う実験手順を提供した。
本稿では、可視化された実験プロトコルの概要を説明することで、脳波システムを用いた上肢の動きに関連する神経活動の獲得と解析に使用される具体的な概念と方法を説明することを目的とした。片麻痺性脳卒中の参加者における麻痺性上肢と非麻痺性上肢の間の脳波を介したニューロン活動の違いを実証するにあたり、この研究は、脳卒中患者の運動障害の重症度の潜在的なバイオマーカーとして、記述されたプロトコルを使用して脳波の実現可能性を提示することを目的としていました 横断的な文脈での脳卒中患者の運動障害。

障害肢のタスク固有の動作中の脳波(EEG)信号の変化は、運動障害の重症度および脳卒中患者の運動回復の予測のための潜在的なバイオマーカーとして報告されています。脳波実験を実施する場合、堅牢で解釈可能な結果を得るためには、詳細なパラダイムとよく整理された実験プロトコルが必要です。このプロトコルでは、上肢の動きと、EEGデータの取得と分析に必要な方法と技術を備えたタスク固有のパラダイムを示します。パラダイムは、1 分間の休息とそれに続く 10 回の試行で構成され、それぞれ 5 秒と 3 秒の休息とタスク (手の伸展) 状態を 4 回のセッションで交互に行います。脳波信号は、32個のAg/AgCl頭皮電極を用いて、サンプリングレート1,000Hzで取得し、四肢運動に伴う事象関連スペクトル摂動解析と、低ベータ(12-20Hz)周波数帯における全球レベルでの機能的ネットワーク解析を行った。代表的な結果は、障害上肢の運動中に低ベータ脳波周波数帯の機能ネットワークの変化を示し、機能ネットワークの変化は慢性脳卒中患者の運動障害の程度と関連していました。この結果は、脳卒中患者の上肢運動時の脳波測定における実験パラダイムの実現可能性を実証している。運動障害と回復のバイオマーカーとしての脳波信号の潜在的な価値を決定するには、このパラダイムを使用したさらなる研究が必要です。
