이 연구는 뇌졸중 환자의 상지 운동 관련 신경 활동을 측정하기 위한 EEG 실험을 도입했습니다. EEG의 획득 및 분석의 실험 패러다임과 방법은 ipsilesional 및 contralesional motor cortex의 ERD 패턴을 결정하기 위해 적용되었습니다.

상지 운동 장애는 뇌졸중의 가장 흔한 결과 중 하나이며 일상 생활 활동의 제한과 관련이 있습니다 1,2. 알파(8-13Hz) 및 베타(13-30Hz) 밴드 리듬은 움직임과 밀접한 관련이 있는 것으로 알려져 있습니다. 특히, 연구에 따르면 장애가 있는 사지의 움직임 중 알파 및 하위 베타(12-20Hz) 주파수 대역의 신경 활동 변화는 뇌졸중 환자의 운동 장애 정도와 상관관계가 있는 것으로 나타났습니다 3,4,5. 이러한 연구 결과를 바탕으로 뇌파 검사(EEG)는 운동 장애의 중증도와 운동 회복 가능성을 모두 반영하는 잠재적인 바이오마커로 부상했습니다 6,7. 그러나 이전에 개발된 EEG 기반 바이오마커는 작업 유도 EEG 데이터보다는 휴식 상태 EEG 데이터에 주로 의존하기 때문에 뇌졸중 환자의 운동 장애 특성을 조사하는 데 부적합한 것으로 판명되었습니다 8,9,10. 이층 반구와 반대 반구 사이의 상호 작용과 같은 운동 장애와 관련된 복잡한 정보 처리는 휴식 상태 EEG가 아닌 작업 유도 EEG 데이터를 통해서만 밝힐 수 있습니다. 따라서 신경 활동과 운동 장애 특성 사이의 관계를 탐구하고 뇌졸중 환자의 운동 장애에 대한 잠재적 바이오마커로서 손상된 신체 부위의 움직임 중에 생성된 EEG의 유용성을 명확히 하기 위한 추가 연구가 필요하다11.
행동 효과를 평가하기 위해 EEG를 구현하려면 작업별 패러다임과 프로토콜이 필요합니다. 현재까지 다양한 뇌파 프로토콜이 제안되었는데,12 뇌졸중 환자는 운동 관련 뇌 활동을 유도하기 위해 상상 또는 실제 움직임을 수행했다11,13. 상상 동작의 경우, 참가자의 약 53.7%가 해당 움직임을 명확하게 상상할 수 없었기 때문에("문맹"이라고 함) 움직임과 관련된 뇌 활동을 유도하지 못했다14. 또한, 중증 뇌졸중 환자는 상지 전체를 움직이기 어렵고, 불안정한 움직임으로 인해 데이터 수집 시 불필요한 아티팩트가 발생할 가능성이 있습니다. 따라서 업무와 관련된 고품질 뇌파 데이터와 신경생리학적으로 해석 가능한 결과를 얻기 위해서는 전문가의 노하우를 바탕으로 한 지도가 필요합니다. 본 연구에서는 뇌졸중 환자가 비교적 간단한 손 운동 작업을 수행할 수 있도록 실험 패러다임을 종합적으로 설계하고, 상세한 지침과 함께 실험 절차를 제공하였다.
이 기사에서 시각화된 실험 프로토콜을 간략하게 설명함으로써 EEG 시스템을 사용하여 상지의 움직임과 관련된 신경 활동의 획득 및 분석에 사용되는 특정 개념과 방법을 설명하는 것을 목표로 했습니다. 편마비 뇌졸중 참가자의 마비성 상지와 비마비성 상지 간의 EEG를 통한 신경 활동의 차이를 입증하기 위해 이 연구는 단면 맥락에서 뇌졸중 환자의 운동 장애 중증도에 대한 잠재적 바이오마커로 설명된 프로토콜을 사용하여 EEG의 타당성을 제시하는 것을 목표로 했습니다.
