surgical approach, challenges, and resolutions for uterus transplantation in rats

השתלת רחם ימין שייכת לאחד ההליכים המיקרוכירורגיים המורכבים ביותר בהם שולטים רק מעטים. הפרוטוקול שלנו מעביר השתלת רחם ימנית משלב ניסיוני למתודולוגיה ישימה. הפרוטוקול שלנו מוביל לשיעור גבוה של לידות חי לאחר השתלת רחם.בקרוב זה יקדם את היישום של השתלת רחם ימין למחקר בפועל. סבלנות וסיבולת הן תכונות מפתח במהלך כל ההליך. לאחר ביצוע לפרוטומיה מדיאנית, על חולדה מורדמת, לקצור את קרן הרחם הימנית עם חלל הרחם המשותף וצוואר הרחם יחד עם פדיקור כלי הדם.לצרוב את הפצע ולקשור את קרן הרחם השמאלית בתפר פוליגלקטין 4-0. כעת יש לנתק את קרן הרחם השמאלית הצמודה להסתעפות מחלל הרחם המשותף. להפריד את החלק הצווארי והנרתיקי של השתל העתידי מהחיבור הרקטלי.להפריד את השתל העתידי מן הרצועות paravaginal ו paracervical ligaments. באמצעות דיאתרמיה, לנתח את הנרתיק סביב שניים עד שלושה מילימטרים caudal של צוואר הרחם. אתר את עורק הרחם והוורידים במקורותיהם.Ligate כל כלי הדם caudal של כלי הרחם עם 8-0 מקום פוליאמיד. מקום 8-0 ליגטורות פוליאמיד ישירות סביב העורק האיליאק המשותף הימני ווריד פרוקסימלי לביפורקציות אבי העורקים והקאוול. בצע חתך באורך 0.5 עד מילימטר לתוך העורק האיליאק המשותף הימני הסמוך ישירות לביפורקציה.הכנס מחט ישרה וקהה של 25 מד לתוך הלומן כדי לשטוף אותו החוצה. אבטח את המחט עם ליגטורה פוליאמיד 6-0. כדי לאפשר זרימה החוצה במהלך השטיפה, יש לנתח את הווריד האיליאק המשותף לליגטורה בווריד האיליאק המשותף הימני.באמצעות מזרק של שלושה מיליליטר, שטפו את הרחם בתמיסת רינגר קרה, ואחריה תמיסת שימור איברים בתוספת הפרין וקסילזין. חותכים את עורק האיליאק המשותף של הרצועה בביפורקציה של אבי העורקים הבטני. לאחר מכן באמצעות דיסקציה קהה, לנתק את כלי הדם האיליאק הנפוצים מן אבי העורקים vena cava bifurcation בחלוקת כלי הרחם.ולהסיר את ההשתלה כאשר רקמת הרחם הופכת חיוורת. הכניסו את ההשתלה לתמיסת שימור איברים קרים כדי להקל על הכנת השולחן האחורי ואחסון לפני ההשתלה. לאחר הנחת השתל בחלל הבטן ומיקום וריד השתל, יש להניח תפר אחד של פוליאמיד 10-0 בפינת הגולגולת ותפר רופף אחד בפינה הקאודלית של החריץ בווריד האיליאק המשותף הימני.Anastomose צד אחד של וריד השתל לתוך הווריד של המקבל באמצעות שש עד שמונה לולאות של תפר רציף. באופן דומה, anastomose את הצד השני של הכלי החל מבחוץ. לאחר מכן באמצעות 10-0 פוליאמיד, לקשור קשר אחד בתפר השהייה הגולגולתית וקשר נוסף בתפר השהייה הקאודלי.בצע אנסטומוזה עורקית באמצעות 10-0 תפרים קטועים פוליאמיד בארטריה האיליאק המשותפת הימנית עם שמונה עד 10 לולאות. כדי להתחיל בזילוח השתל, כאשר האתרים מופיעים כפטנט, שחררו את מהדקי כלי הדם על כלי השתל ובדקו אם יש סימנים של רפרפוזיה בשתל. חבר את שרוול הנרתיק של ההשתלה לקמרון הנרתיק באמצעות שישה עד שבעה תפרים קטועים של 6-0 פוליגלקטין.באמצעות חמישה עד שבעה תפרים קטועים של פוליאמיד 7-0, אנסטומוז את קרן הרחם של הגרף מקצה לקצה למקטע הרחם הגולגולתי הנותר של הנמען. עקוב אחר פעולת פוסט החולדה. לאחר השתלת הרחם, שתי חולדות מקבוצה 1 הראו סימני הריון.עם זאת, בדיקה גילתה רחם מורחב, מה שמרמז על כך שהגורים התפתחו בצורה תקינה, אך החולדות לא הצליחו ליילד את הגורים. בדיקה היסטולוגית בעוברים הראתה כי הגורים התפתחו עד ללידה. חמש חולדות מקבוצה 2 הציגו הריון ושלוש המליטו גורים.נקבת לואיס המליטה פעמיים. כל הגורים בהמלטה הראשונה שרדו, בעוד שרק שניים מתוך שלושת הגורים מההמלטה השנייה שרדו. הנקבה הנורווגית החומה המליטה גם היא פעמיים וכל הגורים ששרדו הראו התפתחות תקינה.התאמת הפרוטוקול שיפרה הן את התוצאות הניתוחיות הישירות והן את שיעור לידות החי לאחר השתלת רחם. הוא הגדיל את שיעור הישרדות השתל במשך שבועיים מ-50% בקבוצה 1 ל-75% בקבוצה 2. חשוב לשמור על קשר התפרים בפינה הקאודלית משוחרר לצורך התאמה טובה יותר ומניעת השפעות של מיתרים קפוצים.השתלת רחם ימין מתאימה במיוחד להערכת גישות לפני ההשתלה שעשויות לשפר את תוצאות השתלת הרחם כגון זילוח איברים היפודרמי ex vivo או טכניקות אחרות להפחתת פגיעה באיברים. באופן כללי, השתלת רחם בבעלי חיים הייתה וחובה להציג כדי לבסס השתלת רחם במרפאה.
